V. ADDITIONAL REGIONAL STAFF REVIEW ITEMS
Lg& T.StiiAr
The subject application was received on May 20, 2020. While downloading the application from Laserfiche, several
items necessary to complete the requested permitting were found to be missing in the application. An email was sent on
May 21, 2020 to the permit reviewer requesting that an add info be generated to request this information or that the
application be returned as incomplete. Other than the cursory review for completeness while downloading, no
review has been completed for this project. Missing items must be received before review can begin.
To date the application has not been returned as incomplete and no add info request letter has been submitted to the
applicant. We are 30 days into the review period, thus warranting an official staff report be generated to officially
request the missing application items. See attached email for missing items.

I cannot do my review without the soils report. This is an incomplete application package and should be returned, or ask
for the missing report very soon. The timeframe that I have for review, site visits and returning a completed staff report
should not start counting down until we receive a complete application package.

Ranveer,
I wanted to check with you to ensure that you received my email below concerning the Sparta RLAP permit application.
It is missing the soils evaluation report and soil sampling results for the proposed new fields. I need that information
before I can begin my review.
Thanks,
Patrick

Ranveer,
I downloaded the application for the subject modification and renewal project off Laserfiche. I noticed the pdf
application package did not contain a soils report or soil sampling for the proposed new land application fields. I have
not completed a thorough review of the application, so I am not sure if anything else is missing or not. Please double
check to ensure it is not something that I overlooked in Laserfiche. We need to ask for the missing soils report and soil
sampling data for the proposed new fields before I can begin my review.
Thanks,
Patrick
