This letter is to inform you that the Division has finalized its review and the comment period is now
closed for NPDES permit NC0075523. A final copy of the permit is attached to this letter. This permit is issued
pursuant to the requirements of the North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection Agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, (6714 Mail Service
Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and
binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. In addition, this permit does not affect the legal
requirement to obtain other permits which may be required by the Division of Water Quality or permits required
by the Division of Land Resources, Coastal Area Management Act or any other Federal or Local governmental
permit that may be required.

The following notes were taken during the review of the application package:
• 303(d) List Review: Beaver Dam Creek is not listed on North Carolina's
1998 303(d) List.
• Review of Basin Plan: No information was reported for Beaver Dam Creek
in the Basin Plan.
• Staff Report (prepared 2/8/99): Region did not provide specific comments
in reference to the subject permit. Region recommends issuance of permit.
• Staff Report (prepared 6/30/93: Facility discharges contact cooling water
from one outfall (Outfall 00t) and air compressor condensate from a second
outfall (Outfall 002). The discharges from the facility do.not enter the
receiving stream by a direct conveyance, rather they empty onto the ground

behind the building and meander through a wooded area owned by the
company. Most of the contact cooling water used by the autoclaves is
recycled. When a discharge is necessary, it is usually on an infrequent basis
(3-5 times per month). Most of the water discharged either soaks into the
ground or is lost to evaporation before reaching the receiving stream.
• DMR Review:
Facility appears to be operating within the constraints of their existing permit
conditions. Oil and Grease levels range from 1.0 Ibs/day to 7.01 Ibs/day
(daily max) and TSS levels range from 1.0 mg/I to 14 mg/1 (daily max).
Compliance evaluation Inspection Report prepared July 2, 1998: Self monitoring
data was reviewed for the period April 1997 to March 1998. No violations were
reported. The subject facility is in compliance with permit conditions. All upstream
and downstream samples were collected properly.
