Dear Mr. Mashburn:
A letter of request for an Authorization to Construct was received August 14,
1987, by the Division and final plans and specifications for the subject facilities
have been reviewed and found to be satisfactory. Authorization is hereby granted for
the construction of a 0.015 MGD wastewater treatment facility consisting of a bar
screen unit, 10,000 gallon aeration tank, two (2) 1800 gallon clarifiers, a 1775
gallon sludge holding tank, an 895 gallon chlorine contact tank and tablet -type
chlorinator; piping and appurtenances to serve the East Bend Industrial Park in Yadkin
County.
This Authorization to Construct is issued in accordance with Part III paragraph C
of the NPDES Permit No. NC0064726 issued December 3, 1987, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0064726.
The Permittee must employ a certified wastewater operator in accordance with Part
I paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston-Salem Regional Office, telephone no. 919/761-2351 shall be notified
at least twenty-four (24) hours in advance of the operation so that an in -place
inspection can be made. Such notification to the Regional Supervisor shall be made
during the normal office hours.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
The blowers shall provide 60 CFM for aeration. Any adjustments necessary to
obtain 60 CFM must be made.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with with the approved
plans and specifications.
