Jenny Graznak of this office performed a Compliance Evaluation Inspection of the wastewater treatment
system at Alta mahaw-Ossipee Elementary School in Alamance County on April 22, 2015. Matt Smith,
Operator in Responsible Charge (ORC), was present for the inspection. This type of inspection consists of
two basic parts: an in -office review of facility files and self -monitoring data and an on -site inspection of
the facility. This inspection evaluated nine (9) areas, and observations from each area are addressed
below:
1. Permit
The permit became effective May 1, 2011 and expires April 30, 2016. A copy of the current NPDES
permit was available on site during the inspection.
2. Self -Monitoring Program
A review of the monthly Discharge Monitoring Reports (DMRs) from April 2014 through February 2015
showed that all parameters were reported per the frequencies specified in the permit. There were also
no limit violations during this time period.
3. Laboratory
R & A Labs, Inc. in Kernersville, NC provides plant operation and all analysis for monitoring
requirements. ORC Matt Smith analyzes the following field parameters for the facility: temperature and
pH. R & A verifies the thermometer annually (April 9, 2015). Daily calibration records, lab data, and
DMRs were available for review during the inspection.
4. Flow Measurement
Effluent flow is estimated using data from the school's potable water meter.
5. Facility Site Review
The plant consists of a bar screen, aeration basin, clarifier, and ultraviolet disinfection. The two blowers
are set on timer so that each one runs a 12-hour shift. Solids are raked from the bar screen and removed
to the school dumpster.

6. Operations & Maintenance
Alamance-Burlington School System is responsible for most maintenance at this facility, except for the
blowers. The six bulbs for the UV system were all changed on February 4, 2015, according to ORC
documentation. Once again, Ms. Graznak noted that the UV intensity monitor was turned off. Mr. Smith
believes the school maintenance staff keeps it turned off to avoid any alarms. However, the bulb sensors
were on and Ms. Graznak verified that the bulbs were operational. The ORC stated that he regularly
adds soda ash to the aeration basin order to regulate pH. He also adds dog food throughout the
summer.
7. Records/Reports
The DMRs and lab data are maintained according to the permit. The daily operator logbook and the UV
log/maintenance report are maintained in the locked shed at the facility. The facility notebook contains
a copy of the current permit, sludge removal receipts, spill response plan, field parameter certification,
and a list of the operators for the plant. Mr. Smith takes thorough notes in his operator logbook
regarding activities at the plant, including maintenance, cleaning, and sludge removal.
8. Sludge Handling & Disposal
There is no sludge digester at the plant, so Jenkins Waste Management pumps out 3,000 gallons from
the aeration basin approximately every month. The last solids removal occurred on April 16, 2015,
according to ORC records.
9. Effluent/Receiving Waters
The effluent is discharged into the Haw River in the Cape Fear River Basin. The location of the effluent
pipe is still unknown. Effluent samples are collected from the end of the UV trough. The effluent
appeared clear with no noticeable solids at the time of the inspection.

Smith, Operator in Responsible Charge (ORC), was present for the inspection. This type of inspection
consists of two basic parts: an in -office review of facility files and self -monitoring data and an on -site
inspection of the facility. This inspection evaluated nine (9) areas, and observations from each area are
addressed below:
1. Permit
The permit became effective May 1, 2011 and expires April 30, 2016. A copy of the current NPDES
permit was available on site during the inspection.
2. Self -Monitoring Program
A review of the monthly Discharge Monitoring Reports (DMRs) from April 2013 through March 2014
showed one transcription error in September 2013. The ORC recorded the pH as 6.77 on September
23, 2013. However, the DMR reports 6.47 on that day. Please submit an amended DMR with the

Price, also with R & A Labs. Alamance-Burlington School System is responsible for most maintenance at
this facility, except for the blowers. The six bulbs for the UV system are changed every 8,000 hours,
according to a maintenance sticker in the UV shed. Three extra bulbs were observed in the locked
shed. Ms. Graznak noted that the UV intensity monitor was turned off at the time of the inspection,
but the bulbs were on. Ms. Graznak requested that the intensity monitor be turned on, and it read
100%. ORC stated that he regularly adds soda ash to the aeration basin order to regulate pH.

log/maintenance report are maintained in the locked shed at the facility. The facility notebook
contains a copy of the current permit, sludge removal receipts, spill response plan, field parameter
certification, and a list of the operators for the plant. Mr. Smith takes thorough notes in his operator
logbook regarding activities at the plant, including maintenance, cleaning, and sludge removal.
8. Sludge Handling & Disposal
There is no sludge digester at the plant, so Jenkins Waste Management pumps out 3,000 gallons from
the aeration basin every month. The last solids removal occurred on April 10, 2014, according to ORC
records.
9. Effluent/Receiving Waters
The effluent is discharged into the Haw River in the Cape Fear River Basin. The location of the effluent
pipe is unknown. Effluent samples are collected from the end of the UV trough. The effluent appeared
clear with no noticeable solids.

A review of the monthly Discharge Monitoring Reports (DMRs) from April 2013 through March 2014 showed
one transcription error in September 2013. The ORC recorded the pH as 6.77 on September 23, 2013.
However, the DMR reports 6.47 on that day. Please submit an amended DMR with the correct pH value for
that date. There were no limit violations for this facility during the time period stated above.

Jenny Graznak of this office performed a Compliance Evaluation Inspection of the wastewater
treatment facility at Altamahaw/Ossipee Elementary School in Alamance County on May 29, 2013.
Matt Smith, ORC, was present for the inspection. This type of inspection consists of two basic parts:
an in -office review of facility files and self -monitoring data and an on -site inspection of the facility.
This particular inspection evaluated nine (9) areas, and observations from each area are addressed
below:
1. Permit
The permit became effective May 1, 2011 and expires April 30, 2016. A copy of an older edition of
the permit is kept in the locked UV shed at the facility. Please make sure that a copy of the
current permit and effluent limitations and monitoring requirements are available on site.

Dear Mr. Rogers:
Jenny Graznak of this office performed a Compliance Evaluation Inspection of the
wastewater treatment facility at Altamahaw/Ossipee Elementary School in Alamance County on
May 30, 2012. Morgan Turner, ORC, was present for the inspection. This type of inspection
consists of two basic parts: an in -office review of facility files and self -monitoring data and an
on -site inspection of the facility. This particular inspection evaluated nine (9) areas, and
observations from each area are addressed below:
1. Permit
The permit became effective May 1, 2011 and expires April 30, 2016. A copy of the permit is
kept in the locked UV shed at the facility.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from April 2011 through March 2012 showed
that all parameters were correctly reported per the frequencies specified in the permit. There
were no limit violations during this time period.
3. Laboratory
R & A Labs, Inc. in Kernersville, NC provides analysis for all permit parameters. R & A Lab's
staff analyzes the following field parameters for the facility: temperature and pH. R & A verifies
the thermometer annually. Calibration records are maintained with the lab data and DMRs and
were available during the inspection.
4. Flow Measurement
Effluent flow is estimated by the school's potable water meter.
5. Facility Site Review
The plant includes an aeration tank, a clarifier, and an ultraviolet disinfection system. School
maintenance staff built a bar screen for the plant. Solids are raked from the screen and placed
in a basket, which is carried by the maintenance staff to the trash dumpster when full. Both
blowers were operational during the inspection. Blowers have been moved onto a concrete pad
so that servicing them is easier. The blowers are on a timer so that each one runs a 12-hour
shift, changing over at 11:00 am and 11:00 pm.

Labs. Alamance-Burlington School System is responsible for maintenance at this wastewater
treatment facility. Bulbs for the UV system are changed every 8,000 hours, according to a
maintenance sticker in the UV shed. The UV bulbs are cleaned every 2 weeks — the UV trough
makes this system easier to clean than at other schools. An extra bulb is maintained in the
locked UV shed. Ms. Graznak noted that the UV intensity monitor was not working at the time
of the inspection, but the bulbs were on.
7. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A copy of this data was available for review during the inspection. The daily operations
logbook and the UV log/maintenance report are maintained in the UV shed at the facility. The
facility notebook contains a copy of the permit, annual reports, sludge removal receipts, spill
response plan, field parameter certification, and a list of the operators for the plant.
8. Sludge Handling & Disposal
There is no sludge digester at the plant, so sludge is periodically pumped from the clarifier.
Jenkins Waste Management works on contract to pump out the clarifier every month.
9. Effluent/Receivinq Waters
The effluent is discharged into the Haw River in the Cape Fear River Basin. The location of the
effluent pipe is unknown. Effluent samples are collected from the end of the UV trough.

Altamahaw/Ossipee Elementary School WWTP — Alamance County
Dear Dr. Bridges:
Jenny Graznak of this office performed a Compliance Evaluation Inspection of the
wastewater treatment facility at Altamahaw/Ossipee Elementary School in Alamance County on
January 27, 2010. Morgan Turner, ORC, was present for the inspection. This type of
inspection consists of two basic parts: an in -office review of facility files and self -monitoring data
and an on -site inspection of the facility. This particular inspection evaluated nine (9) areas, and
observations from each area are addressed below:
1. Permit
The permit became effective July 1, 2006 and expires April 30, 2011. A copy of the permit is
kept in the facility notebook that is stored in the shed at the plant.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from January 2009 through December 2009
showed that all parameters were correctly reported per the frequencies specified in the permit.
3. Laboratory
R & A Labs, Inc. in Kernersville, NC provides analysis for all permit parameters. R & A Lab's
staff analyzes the following field parameters for the facility: temperature and pH. R & A verifies
the thermometer annually. Calibration records are maintained with the lab data and DMRs and
were available during the inspection.
4. Flow Measurement
Effluent flow is estimated from meter readings from the school's drinking water well.
5. Facility Site Review
The plant includes an aeration tank, a clarifier, and an ultraviolet disinfection system. School
maintenance staff built a new bar screen for the plant. Solids are raked from the screen and
placed in a basket, which is carried by the maintenance staff to the trash dumpster when full.
Both blowers were operational during the inspection. The blowers are being placed on a new
12-hour timer, which will be interchanged back and forth.

Labs. Alamance-Burlington School System has designated two staff members to work on
maintenance for the wastewater treatment facilities. Mr. Turner keeps an Equipment
Maintenance log sheet with the Daily Operations Log in order to track and schedule
maintenance activities.
There was a considerable amount of floating grease in the clarifier, which Mr. Turner attributes
to the recent pump -out on January 8, 2010. There is an extra UV bulb maintained on site. UV
bulbs are cleaned every 2 weeks with Lime -Away solution. Operator is regularly adding dog
food as a supplement to the aeration basin.
7. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A copy of this data was available for review during the inspection. Daily operators log is
maintained at the facility. The facility notebook contains a copy of the permit, annual reports,
sludge removal receipts, spill response plan, field parameter certification, and a list of the
operators for the plant.
8. Sludge Handling & Disposal
There is not an operational sludge digester at the plant, so sludge is periodically pumped from
the clarifier. Jenkins Waste Management works on contract to pump out the clarifier every 4-8
weeks. According to receipts, the last pump -out occurred January 8, 2010.
9. Effluent/Receiving Waters
The effluent is discharged into the Haw River in the Cape Fear River Basin. Effluent samples
are collected from the spout at the end of the UV cylinder.

Governor Bev Perdue recently signed legislation to complete passage of rules for the Jordan Lake
Nutrient Management Strategy. The Strategy includes a Wastewater Discharge rule (T15A NCAC 02B
.0270) that applies to your wastewater treatment facility. Pursuant to Sub -Item (6)(d) of the rule, I am
writing to notify you of the nutrient allocations assigned to your facility.
This letter addresses only certain provisions of the Jordan Lake Wastewater Discharge rule. I encourage
you to familiarize yourself with the remainder of this rule.
Nutrient Allocations. The Wastewater Discharge rule establishes the maximum loads of nitrogen and
phosphorus that can be released from wastewater treatment facilities into the Jordan Lake watershed.
The rule provides that these wasteload allocations are to be divided among the existing facilities in
proportion to (with specific exceptions) their 2001 permitted flows. It also provides that facilities
permitted to discharge 100,000 gpd or more are to receive nitrogen and phosphorus limits equivalent to
their individual allocations.
Your facility has been assigned nitrogen and phosphorus allocations. The allocations are expressed as
annual mass loadings delivered to the lake. Due to foreseeable losses between the discharge point and
the lake, they are also expressed as the equivalent discharge loads at the point of discharge. The
delivered and the corresponding discharge allocations assigned to your facility are as follows:

Notification of Jordan Lake Nutrient Requirements

Nutrient Limits. Your facility will not receive new nutrient limits at this time. However, any nutrient
limits already in your NPDES permit will remain in effect.
If the facilities permitted for less than 100,000 gpd, as a group, approach their allocations for nitrogen
or phosphorus, the Division will consider whether nutrient limits are necessary to protect water quality
at that time. In the event that nutrient limits become necessary, the limits would equal your facility's
discharge allocations.
Nutrient Trading. The rule provides that, when one utility connects to another, the allocations held by
the closing facility are transferred to the remaining facility. The rule also provides for the trading of
allocations among dischargers within each of the major Jordan subwatersheds. Be sure to consult with
the Division's NPDES staff before you consider selling allocation: transactions can have profound
implications for your facility and its permit.

Dear Dr. Bridges:
Jenny Graznak of this office performed a Compliance Evaluation Inspection of the
wastewater treatment facility at Altamahaw/Ossipee Elementary School in Alamance County on
March 11, 2009. Morgan Turner, ORC, was present for the inspection. This type of inspection
consists of two basic parts: an in -office review of facility files and self -monitoring data and an
on -site inspection of the facility. This particular inspection evaluated nine (9) areas, and
observations from each area are addressed below:
1. Permit
The permit became effective July 1, 2006 and expires April 30, 2011. A copy of the permit is
kept in the facility notebook that is stored in the shed at the plant.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from January 2008 through December 2008
showed that all parameters were correctly reported per the frequencies specified in the permit.
3. Laboratory
R & A Labs, Inc. in Kernersville, NC provides analysis for all permit parameters. R & A Labs
staff analyzes the following field parameters for the facility: temperature and pH. R & A verifies
the thermometer annually. Calibration records were available during inspection.
4. Flow Measurement
Effluent flow is estimated from meter readings from the school's drinking water well.
5. Facility Site Review
The ultraviolet disinfection system was installed in late January 2008 to replace the chlorination
system. The plant also includes an aeration tank, and a clarifier. Due to the age of the plant,
there is a non-functioning comminutor and bar screen. Solids enter directly into the aeration
basin, and either settle in the clarifier or are skimmed from surface by ORC. Solids are placed
in a trash bucket, which is emptied into the school's dumpster by the maintenance staff. The
skimmer has been rigged with a pulley system to stay level in the clarifier. Both blowers were
operational during the inspection.

6. Operations & Maintenance
The ORC for this facility is Morgan Turner, R & A Labs. Back -Up Operators are also from R & A
Labs. Alamance-Burlington School System has designated two staff members to work on
maintenance for the wastewater treatment facilities. Mr. Turner keeps an Equipment
Maintenance log sheet with the Daily Operations Log in order to track and schedule
maintenance activities.
7. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A copy of this data was available for review during the inspection. Daily operators log is
maintained at the facility. The facility notebook contains a copy of the permit, annual reports,
sludge removal receipts, spill response plan, field parameter certification, and a list of the
operators for the plant.
8. Sludge Handling & Disposal
There is not an operational sludge digester at the plant, so sludge is periodically pumped from
the clarifier. Jenkins Waste Management works on contract to pump out the clarifier every 6-8
weeks. According to receipts, the last pump -out occurred February 17, 2009.
9. Effluent/Receiving Waters
The effluent is discharged into the Haw River in the Cape Fear River Basin. Effluent samples
are collected from the spout at the end of the UV cylinder.

Jenny Graznak of this office performed a Compliance Evaluation Inspection of the
wastewater treatment facility at Alta mahaw/Ossipee Elementary School in Alamance County on
March 28, 2008. Morgan Turner, ORC, and Clifford Cain of R & A Labs, Inc. were present for
the inspection. This type of inspection consists of two basic parts: an in -office review of facility
files and self -monitoring data and an on -site inspection of the facility. This particular inspection
evaluated nine (9) areas, and observations from each area are addressed below:
1. Permit
The permit became effective July 1, 2006 and expires April 30, 2011. A copy of the permit is
kept in the shed.
2. Self-Monitorinq Program
A review of the monthly self -monitoring reports from January 2007 through December 2007
showed no violations of permit limits. All parameters were reported per the frequencies
specified in the permit.
3. Laboratory
R & A Labs, Inc. in Kernersville, NC provides analysis for all permit parameters. R & A Labs
staff analyzes the following field parameters for the facility: total residual chlorine, temperature,
and pH. Chlorine is measured with a Hach test kit, which is annually calibrated by R & A Labs,
Inc. R & A verifies the thermometer annually. Calibration records were available during
inspection.
4. Flow Measurement
Effluent flow is estimated from meter readings from the school's drinking water well. The school
plans to install a flow meter in the next few months to provide a better effluent measurement.
5.-Facility Site Review
The new Total Residual Chlorine limit took effect on December 1, 2007. An ultraviolet
disinfection system was installed in late January 2008. The plant also includes an aeration tank,
and a clarifier. Due to the age of the plant, there is a non-functioning comminutor and bar
screen. Solids enter directly into the aeration basin, and either settle in the clarifier or are
skimmed from surface by ORC.

6. Operations & Maintenance
The ORC for this facility is Morgan Turner, R & A Labs. Back -Up Operators are also from R & A
Labs. Alamance-Burlington School System and R&A Labs are presently negotiating to
determine who should handle maintenance activities at the facility. In particular, it is imperative
to properly dispose of bar screenings to have a removal system in place. A dumpster should be
appropriated for this purpose.
7. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A copy of this data was available for review during the inspection. Daily operators log is
maintained at the facility.
8. Sludge Handling & Disposal
There is not an operational sludge digester at the plant, so sludge is periodically pumped from
the clarifier. Richard Jenkins Septic Tank Service works on contract to pump out the clarifier
every 6-8 weeks.
9. Effluent/Receiving Waters
The effluent is discharged into the Haw River in the Cape Fear River Basin. The location of the
effluent pipe is known, but effluent samples are collected from the end of the UV system.

A letter of request for Authorization to Construct was received May 8, 2007, by the
Division, and final plans and specifications for the subject project have been reviewed and found
to be satisfactory. Authorization is hereby granted for the construction of modifications to the
existing 0.012 MGD Wastewater Treatment Plant, with discharge of treated wastewater into the
Haw River in the Cape Fear River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Installation of 56 GPM open channel ultra violet disinfection system, yard piping,
valves, electrical controls, and other appurtenances necessary to provide a
complete system, in conformity with the project plans, specifications, and other
supporting data subsequently filed and approved by the Department of
Environment and Natural Resources.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NCO045161 issued June 12, 2006, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0045161.
In accordance with 15A NCAC 2T .0105, alternative design criteria has been reviewed
and approved for the design setback from Old NC 87 Road. A new disinfection system is
required to meet NPDES limits for chlorine effective February 1, 2008. The approved alternative
design setback for the new disinfection system from Old NC 87 Road is 33 feet, in lieu of the 50
feet setback established in 15A NCAC 2T .0506 (b).

The sludge generated from these treatment facilities must be disposed of in accordance
with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Water Quality.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number (336) 771-5000, shall be notified
at least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to Cecil G. Madden, Jr., P.E.,
Construction Grants & Loans, DWQ / DENR, 1633 Mail Service Center, Raleigh, NC 27699-
1633.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at
least equivalent to or greater than the classification assigned to the wastewater treatment
facilities by the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class Il, III and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.

Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

The subject permit's expiration date is April 30, 2006. Federal [40 CFR 122] and state (15A NCAC
2H.0105(e)) regulations require that an application for permit renewal be filed at least 180 days prior to expiration

The following items are REQUIRED for all renewal packages:

facility since issuance of the last permit. Submit one signed original and two copies.
o The completed application form (copy attached), signed by the permittee or an Authorized
Representative. Submit one signed original and two copies.
o If an Authorized Representative (such as a consulting engineer or environmental
consultant) prepares the renewal package, submit written documentation showing the
authority delegated to any such Authorized Representative (see Part II.B.11.b of the
existing NPDES permit).
o A narrative description of the sludge management plan for the facility. Describe how
sludge (or other solids) generated during wastewater treatment are handled and disposed.
If your facility has no such plan (or the permitted facility does not generate any solids),

The following item must be submitted by Industrial or Municipal facilities
discharging industrial process wastewater:
o Industrial facilities classified as Primary Industries (see Appendices A-D to Title 40 of the
Code of Federal Regulations, Part 122) must submit a Priority Pollutant Analysis (PPA) in
accordance with 40 CFR Part 122.21. If the PPA cannot be completed by the time you
complete your application package, submit the application package without the PPA.
Submit the PPA as soon as possible after you receive the completed analyses.
The above requirement does NOT apply to non -industrial facilities.

Travel north from Burlington on Hwy 87 approx. 5 miles
to SR 1558. Turn left on SR 1558, travel approx. 0.1
mile. The school is located on the left.

PART IV - EVALUATION AND RECOMMENDATIONS
Jerry Whittum of the WSRO inspected this plant on May
26, 1989. The recommendations made by Jerry were in place
at this school.
The aeration chamber was cleaned out on September 15,
1989. As a result, the sludge was very young and the
aeration chamber was dark grey in color.
Charles Jefferies, Assistant Superintendent of Physical
Affairs, said that the area is growing and the population at
this school is expected to increase substantially in the
next few years.
Since the flow is between 0.004 - 0.005 MGD, it is
recommended that the permit be re -issued with the design
capacity of 0.012 MGD. The treatment plant has been
operating within the limits.
