The NC PIN number for this is 8796169535. Please verify this and submit a corrected LASC Form. There was a signed page 3 from Michael Todd Hadley with the package you submitted. The address the Alamance County Tax Office gives is 8773 Bethel South Fork Road, Snow Camp,NC 27349. V 3. According to the Alamance County Tax Office,Field NC-AM-016-03 belongs to Michael Todd Hadley and not to C. L. Hadley as indicated on the LASC Form you submitted. Moon of 3665 Moon- Lindley Road, Snow Camp, NC 27349 or delete the field. If you decide to delete the field, please provide an updated LASC form reflecting this information. 7. According to the Caswell County Tax Office, Field NC-CS-117-01 belongs to Sandy J. Stone and Carolyn J.Stubblefield(Tax Number: 0044.00.00.0048.0000)and not to Chad Randleman as indicated on the LASC form. Please provide an LOA from either Stone or Stubblefield or delete the field. If you decide to delete the field,please provide an updated LASC form reflecting this information. 8. The LOA for Field NC-CS-117-01 is missing. Please provide an LOA. j9.uffer Maps.Land Owners and Property Lines There need to be new maps with consistent map legends contained on each map and north arrows. There are four different types of map—hand-drawn Synagro maps,GIS drawn Synagro maps and two styles of EMA hand-drawn maps —with different symbols and some of the symbols did not provide adequate information. For example,a solid blue line was a symbol for ditches,ephemeral streams and intermittent/perennial streams;a square box was drawn for structures,but did not differentiate between outbuildings, offsite residences, onsite residences, public assembly, etc. There was confusion about the direction of north on some maps and unless comparison is made to another map,there is no way of orienting the map. 10. According to the Alamance County Tax Office, Field NC-AM-014-05 combines two different pieces of property belonging to James A. Roney, Jr. and Carolyn Rudd Roney. These fields have NC PIN numbers 9807729915 and 9807632176. The property line is not shown on Map NC-AM-14 that you submitted nor is the required 50' setback as required by 15A NCAC 02T.1108(b)(2). Please show the property line and the required 50' setback on Map NC-AM-14 and recalculate acreage of NC-AM-014- 05 and submit a corrected Land Application Site Certification(LASC)Form. 11 According to the Alamance County Tax Office, Field NC-AM-016-01 combines two different pieces of property belonging to William Earl Hadley. These fields are NC PIN numbers 8796385581 and 8796376545. The property line is not shown on Map NC-AM-16;Fields 1-2,4 that you submitted nor is the required 50' setback as required by 15A NCAC 02T.1108(b)(2). Please show the property line and the required 50' setback on Map NC-AM-16; Fields 1-2,4 and recalculate acreage of Field NC- AM-016-01 and submit a corrected Land Application Site Certification(LASC)Form. /12. According to the Alamance County Tax Office, Field NC-AM-016-04 combines two different pieces of property belonging to Peggy J. Hadley Revocable Trust and Russell B. Hadley Revocable Trust. These fields are NC PIN numbers 8796365375 and 8796253666. The property line is not shown on Map NC-AM-16; Fields 1-2,4 that you submitted nor is the required 50' setback as required by 15A NCAC 02T.1108(b)(2). Please show the property line and the required 50' setback on Map NC-AM- 16; Fields 1-2,4 and recalculate acreage of Field NC-AM-016-04 and submit a corrected Land Application Site Certification(LASC)Form. / ✓ 13. Field NC-AM-17-03 has a drain present that is not shown on the site map. Please show the drain, the required 25' setback as required by 15A NCAC 02T.1108(b)(2),and recalculate acreage of Field NC- AM-17-03 and submit a corrected Land Application Site Certification(LASC)Form. Field NC-AM-73-01 contains a stream and buffer. The buffer needs to be 100' on the map. Recalculate acreage of Field NC-AM-73-01 and submit a corrected Land Application Site Certification (LASC) Form. 16. Field NC-AM-73-02 has increased in acreage due the signature on setback waiver forms. However, there is no change in acreage reported. Recalculate acreage of Field NC-AM-73-02 and submit a corrected Land Application Site Certification(LASC)Form. /17. Field NC-AM-73-03 has a drain passing through it that is not shown on the map(though it was shown on earlier buffer map). Please show the drain and buffer on the map,recalculate acreage of Field NC- AM-73-03 and submit a corrected Land Application Site Certification(LASC)Form. ✓18. Field NC-AM-110-02 has its northern neck being buffered, yet there was no reduction in acreage reported. Recalculate acreage of Field NC-AM-110-02 and submit a corrected Land Application Site Certification(LASC)Form. ✓ 19. Fields NC-AM-116-07 and NC-AM-116-08 have property lines that are not shown on the maps. Please provide revised buffer maps with the property lines. /20. According to the Alamance County Tax Office, Field NC-AM-118-09 lies on three different parcels belonging to two different owners. The NC PIN numbers of the parcels are 8766756487 belonging to Simon Ross Stephens;and 8776052793 and 8776046694 belonging to Sylvia Stephens Rogers. While there is a property line buffer between parcels 8776052793 and 8776052793,there is not one between 8776052793 and 8776046694, which there should be according to NCAC 02T .1108 (b)(2). Please show the property line and the required 50' setback on Map NC-AM-118;Fields 3,8-9;and recalculate acreage of Field NC-AM-118-09 and submit a corrected Land Application Site Certification (LASC) Form. ✓21. On Map NC-AM-119, it is not possible to tell where a property line is. According to the Alamance County Tax Office,Field NC-AM-119-01 lies on a parcel with NC PIN 8766828913 belonging to Nelda Lee Stephens; but also either very close to or including a portion of parcel with NC PIN 8766828913 belonging to Nelda Lee Stephens Kiser. Please show this property line and any setbacks required according to 15A NCAC 02T .1108 (b)(2) on Map NC-AM-119. If an acreage revision is required, please show this on the LASC form. /2. Fields NC-AM-125-01 and NC-AM-125-04 according to the Alamance County Tax Office lie on a parcel with NC PIN 9727142308. Fields NC-AM-125-02 and NC-AM-125-03 according to the Alamance County Tax Office lie on a parcel with NC PIN 9727253080. What is not shown on Map NC-AM-125 is the property line between fields with NC PIN 9727142308 and 9727253080 and the buffering required according to 15A NCAC 02T .1108 (bX2). Please show this property line and any setbacks required. If an acreage revision is required,please show this on the LASC form. ✓23. Field NC-AM-126-01 has a stream on the northwest border that is not shown on the buffer map(though was on a previous map). Please show the stream and buffer on the map, recalculate acreage of Field NC-AM-126-01 and submit a corrected Land Application Site Certification(LASC)Form. ✓4. According to the Alamance County Tax Office, Field NC-AM-126-08 is located on parcels with NC PIN numbers 8757572609 and 8757477619 belonging to Travis Chad Fogleman. Map NC-AM-126; Fields 5-8 does not show a property line or the 50' setback required by 15A NCAC 02T .1108 (b)(2). Because of many site changes to Field NC-AM-128-01 to-08,the Permittee has indicated verbally that ir' 1 the fields should be removed from the permit. Please show this on a revised LASC form. 26. According to the Chatham County Tax Office,Fields NC-CH-039-02 has an AKPAR number of 85316 and is adjacent to Field CH040-01 with an AKPAR number of 91430, i.e., two separate parcels. However,no property line nor setbacks are shown on Map NC-CH039-(1-4)nor NC-CH040-01. Please provide a revision of both maps with the property lines and 50' setbacks as required by 15A NCAC 02T .1108(b)(2). If an acreage revision is required,please show this on the LASC form. V 27. Please show the maps and the buffers,recalculate the acreage and submit a revised LASC form. /30.According to the Chatham County Tax Office,Fields NC-CH-039-04 has an AKPAR number of 100 1 and is adjacent to Field CH040-01 with an AKPAR number of 91430, i.e., two separate parcels. However,no property line nor setbacks are shown on Map NC-CH039-(1-4)nor NC-CH040-01. Please provide a revision of both maps with the property lines and 50' setbacks as required by 15A NCAC 02T .1108(b)(2). If an acreage revision is required,please show this on the LASC form. 141. Field NC-CH-098-05 contains unsuitable depth to bedrock and rock outcropping areas and buffering that are not shown or buffered on the map. Please show the unsuitable areas on the map with buffering, recalculate areas, and provide an updated LASC form. /32. Field NC-CH-098-06 was determined to be unsuitable for residuals application because of unsuitable depth to the seasonal high water table and depth to bedrock. Please provide a revised map eliminating this field and a revised LASC form. V33. Field NC-CS-036-12 has a new residence constructed across the road that is not shown on the site map. Please add the residence and buffers to the site map. If this affects acreage, please recalculate and revise the LASC as necessary. OP it �34. The soils series for Fields NC-AM-06-08 and NC-AM-06-09 is DdB — Durham and not Appling as contained on the LASC form. Please correct the LASC form. /36. The predominant soils series for Fields NC-AM-13-01, -02 and-03 is the Tarrus soil series rather than the Georgeville soils series as contained on the LASC form. Please correct the LASC form. The soils for Fields NC-CS-036-06 to-12 are Frogsboro series. Please correct the LASC form. ✓ 40. The soils for NC-CS-113-05 is the Vance soils series and not Pacolet. Please correct the LASC form. V 41. Field NC-OR-079-06 has no soils map. Please provide a soils map. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations.
