Permit Modification Application for:
City of Burlington, Residuals Land Application Program
Permit No. WQ0000520
On September 20, 2017, WSRO staff met with Mr. Shane Fletcher, ORC (City of Burlington) to
review the proposed land application fields included in the application for permit modification
dated August 21, 2017 and the additional information received on September 11, 2017. This
inspection report confines comments to the review of the proposed land application acreage only.
Based upon WSRO's review of the subject proposed fields, the items described below need to
be addressed through an additional information request prior to the permit issuance.
1. All site maps, except for Site OR-64, need to be revised to address the following items
which are required in the map guidance document:
a) There were no public road names shown on the site maps. Add road names to maps.
b) There were no legends for the site maps. Add legends to maps.
c) There were many streams and drainage features on the sites and adjacent to the sites
which were not shown on the site maps. Show all onsite & adjacent streams, and
drainage features on the site maps.
d) The map symbols used are not consistent from map to map(e.g. some site maps showed
streams as solid lines and drains as dashed lines,while other site maps showed streams
as dashed lines).
If each map contained an individual legend this would not be an issue. It was indicated
that the generic map legend sheet submitted as additional information with the last
permitting application is applicable for these maps also. However,the subject maps do
not follow that generic map legend sheet. This can be addressed through submittal
of individual map legends or through revision of the maps to use consistent
symbols and colors for required mapping features.
/2. All of the soil profile descriptions for Site AM-09 Fields #1, #2, and #3 were missing in
the application package.

the site visit. Therefore, only a reconnaissance review (i.e. no soils eval. & no detailed
onsite review) was feasible_for these areas and fields. WSRO recommended that the
Permittee contact WSRO for a more detailed review of these fields prior to the initial land
application event.
Site AM-14
The site map needs to be revised to address the following items:
va) There is a stream which flows adjacent to Field#6 which is not shown on the site
jmap. The adjacent stream and any buffer impacts should be shown on the map.
) According to GIS, the 100-year floodplain associated with the stream described
above in item a. may extend into the edge of Field#6 along the northwest border.
The current site map does not show the 100-year floodplain boundary.
Revised applicable forms with the correct acreage should also be submitted.
Site AM-35
The site map needs to be revised to address the following items:
La) There is a stream adjacent to Field#6 which flows just to the north of the field.This
stream is not shown on the site map. The adjacent stream and the buffer impacts
should be shown on the map.
713) According to GIS, the 100-year floodplain associated with the stream described
above extends into the edge of Field#6 along the northwest border.The current site
map does not show the 100-year floodplain boundary.
/c) There is a stream which flows just to the east of Field#6 that is not shown on the
site map. This stream along with the buffers need to be added to the map.
/d) There is a well located in Field #6 which is not shown on the site map. The well
along with the required buffer need to be added to the map.
Revised applicable forms with the correct acreage should also be submitted.
Site AM-100
The site map needs to be revised to address the following items:
/a) There is a stream adjacent to Field#2 which flows just to the north of the field.This
stream is not shown on the site map. The adjacent stream and any buffer impacts
should be shown on the map.
Revised applicable forms with the correct acreage should also be submitted.
Site CH-01
The site map needs to be revised to address the following items:
Ja) There are a series of ditches that run through the northern section Field #2 which
are not shown on the site map with the required buffers.
