Box 266 Lansing, NC 28643 SUBJECT: Compliance Evaluation Inspection (CEI) Town of Lansing WWTP NPDES Permit # NCO066028 Ashe County Dear Mr. Church: On October 9, 2015 an inspection was performed by George Smith. Mr. Tim Church, ORC, was present. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit Your NPDES permit became effective August 1, 2011 and expires March 31, 2016. As discussed during the previous inspection, the Town may request a permit flow of 30,000 gpd, thereby, changing the sampling requirements to a grab sample. This can be done during your permit renewal application. It is recommended that the Town register for eDMR. It will be a permit requirement in your next permit cycle. Please remember to submit your NPDES permit renewal application EPA Form A. Send to: Division of Water Resources WQ Permitting Section - NPDES 1617 Mail Service Center Raleigh, NC 27699-1617 Records & Reports An activity log was also kept in the building, which logs operator visitation and plant operations. All certificates of analytical analysis were kept on file along with chain of custody forms. Calibration records for pH, total residual chlorine, and temperature were documented. In summary, the records and reports documentation by Mr. Church is considered to be excellent. Both blower pumps were operational, and delivering a gentle rolling action in the aeration basin. The aeration basin had little foam and a good color with an earthy odor. There was no evidence of straggler flock or solids from the up -flow clarifier cone unit. The discharge was clear. The backwash system is functioning properly. The Sanuril tablet chlorination unit was installed in the final effluent contact tank. One of the four tubes is used for disinfection. The unit is satisfactory. An in -ground tablet dechlorinator (Sanuril model) was installed. Sodium bisulfite tablets are used in one tube. Flow Measurement The recording flow chart is operating, and measures the height of water measured in inches passing through a 60' V-notch weir. The flow calibration log and charts were kept on -site for at least 3-years, and were well documented. The flow meter was calibrated and documented on August 5, 2015. Laboratory Total Residual Chlorine (TRC), Temperature, and pH are performed and recorded on the Daily Monitoring Report (DMR). The field parameters (TRC, Temp, pH) are currently performed under field parameter certification #5419. The pH is performed with a Hach Sension 1 meter using a two -point calibration (pH 4 & 10) and a mid -range check (pH 7), which is considered satisfactory. Mr. Church performs a temperature calibration against a NIST traceable thermometer. Total Residual Chlorine (TRC) is measured with a Hach DR2500, which is capable of detection to permit limit of 28 ug/L. The annual 5-point calibration is scheduled to be performed during October 2015. Effluent/Receiving Waters The effluent was clear with no color or solids present at the time of inspection. Big Horse Creek, Class C-trout, HQW waters, appeared to be clean with no visible impacts. Operations & Maintenance Mr. Tim Church is a grade IV operator and is the current ORC. Mr. Fred Walters is the Grade II back-up operator. PV The facility and grounds are maintained in excellent condition. Plant -Pro (Chlorine 68%) is used for disinfecting, and Plant -Pro (Sodium bisulfite) is used for dechlorination. They appear to work well. Settleability test is periodically performed as process control to judge the operational performance of this facility. For the most part, operation & maintenance is judged by observing plant performance. Self -Monitoring Program A review of the monthly self -monitoring reports from January 2015 through July 2015 showed no monitoring or limit violations. The effluent composite sample is collected with an Isco Model 3710 FR using the flow proportion method at 200 mL/ 1000 gal. The composite is stored in ice. Sludge Disposal The sludge removal is contracted through Doug Cox septic service and hauled to the Town of Jefferson WWTP. The compliance evaluation inspection is considered satisfactory. Sullins, Director Division of Water Quality On November 20, 2008 a CEI was performed by George Smith. Mr. Tim Church, ORC, was present for the inspection. Mr. Marc Stokes, Technical Assistance was present during the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. The attached inspection form evaluates nine (9) areas as noted in Section C of the attached EPA inspection report, and the observations and comments during the inspection are addressed below: Permit Your NPDES permit became effective April 1, 2006 and expires March 31, 2011. Records & Reports An activity log was also kept in the building, which logs operator visitation and plant operations. All certificates of analytical analysis were kept on file along with chain of custody forms. Calibration records for pH, total residual chlorine, and temperature were documented. In summary, the records and reports documentation by Mr. Church is considered to be excellent. Facility Site Review The influent barscreen is maintained. The two influent pumps work on alternating cycles and appeared to operating properly, and there was no evidence of overflows. The aeration basin was a brown color with white colored foam covering less than 5% of the surface of the basin. There was no evidence of straggler flock or solids from the up -flow clarifier cone unit. The discharge was clear. The backwash system was functioning properly. The Sanuril tablet chlorination unit was installed in the final effluent contact tank. One of the four tubes is used for disinfection. The unit is satisfactory. A sodium bisulfate tablet dechlorination tube was placed after the effluent weir in order to meet the total residual chlorine limit of 28 ug/L. This treatment may be functioning properly because the facility only receives 20% of the permitted flow. In future inspections, especially if the daily flow increases the Division may decide to perform a field test in order to check the performance of this unit. Flow Measurement The recording flow chart is operating, and measures the height of water measured in inches passing through a 60' V-notch weir. The flow calibration log and charts were kept on -site for at least 3-years, and were well documented. The flow meter was calibrated by BW Allen and documented on November 20, 2008. Laboratory Total Residual Chlorine (TRC), Temperature, and pH are performed and recorded on the Daily Monitoring Report (DMR). The field parameters (TRC, Temp, pH) are currently performed under field parameter certification #5419. The pH is performed with a Hach Sension 1 meter using a two -point calibration (pH 4 & 10) and a mid- range check (pH 7), which is considered satisfactory. Mr. Church performs a temperature calibration against a NIST traceable thermometer. Total Residual Chlorine (TRC) is measured with a Hach DR2500, which is capable of detection to permit limit of 28 ug/L. The annual 5-point calibration was performed on April 24, 2008. Effluent/Receiviniz Waters The effluent was clear with no color or solids present at the time of inspection. Big Horse Creek, Class C- trout, HQW waters, appeared to be clean with no visible impacts. Mr. Church December 10, 2008 Page 3 of 3 Operations & Maintenance Mr. Tim Church is a grade IV operator and is the current ORC. Mr. Fred Walters is the Grade II back-up operator. The facility and grounds are maintained in excellent condition. Plant -Pro (Chlorine 68%) is used for disinfecting, and Plant -Pro (Sodium bisulfate) is used for dechlorination. They appear to work well. Settleability test is periodically performed as process control to judge the operational performance ofthis facility. For the most part, operation & maintenance is judged by observing plant performance. Self -Monitoring Program A review of the monthly self -monitoring reports from January 2008 through August 2008 showed no monitoring or limit violations. The influent and effluent samples are collected with an Isco Model 2910. The new flow meter will be used for flow proportion sampling. All composite and grab samples are collected after the effluent weir, which is after all treatment units. Sludge Disposal The aerated digester was approximately 50% full at the time of the inspection. The sludge removal is contracted through Doug Cox septic service and hauled to the Town of Jefferson WWTP. Our goal is to continuously manage all of our water quality facilities in a manner that not only exceeds all state and federal standards, but also protects and enhances the surface waters of the New River Basin. This report covers the calendar year 2007. Collection System Performance is critical to our river basin. The town maintains over a mile of gravity sanitary sewers and a pump station at the wastewater treatment facility. In 2007 the North Carolina Rural Water Association conducted a smoke testing study of the sanitary sewer system aimed at locating sources of inflow and infiltration of groundwater and surface water into the collection system. Several citizens of the town also assisted in this endeavor. The study was successful in locating a large facility with its roof drains connected to the sewer. Some points of infiltration were also located within the collection system itself. The town is working to correct these sources. It is also important for customers to realize the potential problems presented to the collection system by the introduction of FOG (Fats, Oil, and Grease). These materials tend to solidify in the sewer lines and can create blockages that lead to the overflows of raw sewage into nearby streams. Homeowners should take care to avoid pouring any cooking oils or grease into their drains. Placing the grease into a container and allowing it to solidify and then placing the material into your garbage will avoid such problems. Restaurant owners and commercial users should install grease traps on their service line prior to its connection to the sewer main. No sanitary sewer overflows occurred during 2007. Wastewater Plant Performance for 2007 was again excellent. Your wastewater facility has a design capacity of 50,000 gallons per day and is currently treating about 15,000 gallons per day. The plant's NPDES discharge permit requires the weekly analysis of the following parameters. Conventional Pollutant AyL. The facility appears to be doing a good job of accurately transcribing data. V. Sullins, Director Division of Water Quality On March 4, 2008 a CEI was performed by George Smith. Mr. Tim Church, ORC, was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of. facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. The attached inspection form evaluates nine (9) areas as noted in Section C of the attached EPA inspection report, and the observations and comments during the inspection are addressed below: Permit Your NPDES permit became effective April 1, 2006 and expires March 31, 2011. Records & Reports An activity log was also kept in the building, which logs operator visitation and plant operations. All certificates of analytical analysis were kept on file along with chain of custody forms. Calibration records for pH, total residual chlorine, and temperature were documented. In summary, the records and reports documentation by Mr. Church is considered to be excellent. A Wastewater System Performance Annual Report is required per General Statute § 143-215.1 C (a). The report shall be prepared on either a calendar or fiscal year basis and shall be provided no later than 60 days after the end of the calendar or fiscal year. Facility Site Review There appears to be an increased amount of stormwater run-off adjacent to the effluent box. The increased stormwater flow caused the soil above the effluent pipe to become exposed. The two influent pumps work on alternating cycles and appeared to operating properly, and there was no evidence of overflows. Both blower pumps were operational, and were delivering a gentle rolling action in the aeration basin. The aeration basin was a brown color with white colored foam covering approximately 5% of the surface of the basin. There was no evidence of straggler flock or solids from the up -flow clarifier cone unit. The discharge was clear. The backwash system was functioning properly. The Sanuril tablet chlorination unit was installed in the final effluent contact tank. One of the four tubes is used for disinfection. The unit is satisfactory. A sodium bisulfate tablet dechlorination tube was placed after the effluent weir in order to meet the total residual chlorine limit of 28 ug/L. This treatment may be functioning properly because the facility only receives 20% of the permitted flow. In future inspections, especially if the daily flow increases the Division may decide to perform a field test in order to check the performance of this unit. Flow Measurement The recording flow chart is operating, and measures the height of water measured in inches passing through a 60' V-notch weir. The flow device was not working. The paperwork for a new flow measurement device was initiated. It is a permit requirement to have a calibrated flow measurement device. The flow calibration log and charts were kept on -site for at least 3-years, and were well documented. The flow meter was calibrated by Mr. Church and documented on February 20, 2007. During the previous inspection it was documented that the facility is experiencing the onset of inflow & infiltration problems. This office received your reply on January 16, 2007 concerning inflow and infiltration (I&I), which also addressed the installation of a new well. The I&I results and the date the repairs were completed must be submitted to this office. Laboratory Total Residual Chlorine (TRC), Temperature, and pH are performed and recorded on the Daily Monitoring Report (DMR). The field parameters (TRC, Temp, pH) are currently performed under field parameter certification #5419. Mr. Church March 7, 2008 Page 3 of 4 The pH is performed with a Hach Sension 1 meter using a two -point calibration (pH 4 & 10) and a mid- range check (pH 7), which is considered satisfactory. Mr. Church performs a temperature calibration against a NIST traceable thermometer. Total Residual Chlorine (TRC) is measured with a Hach DR2500, which is capable of detection to permit limit of 28 ug/L. The annual 5-point calibration is required annually. Effluent/Receiving Waters The effluent was clear with no color or solids present at the time of inspection. Big Horse Creek, Class C- trout, HQW waters, appeared to be clean with no visible impacts. Operations & Maintenance Mr. Tim Church is a grade IV operator and is the current ORC. Mr. Fred Walters is the Grade II back-up operator. The facility and grounds are maintained in excellent condition. Plant -Pro (Chlorine 68%) is used for disinfecting, and Plant -Pro (Sodium bisulfite) is used for dechlorination. They appear to work well. Settleability test is periodically performed as process control to judge the operational performance of this facility. For the most part, operation & maintenance is judged by observing plant performance. The clarifier cones were backwashed. This type of system appears to be inefficient for use in a wastewater treatment plant. In general, WWTP clarifiers should not be backwashed, but allowed to settle solids without being disturbed. As the facility approaches the permitted flow of 0.050 MGD this design may not be able to produce an effluent that meets the NPDES permit requirements. Self -Monitoring Program A review of the monthly self -monitoring reports from January 2007 through December 2007 showed no monitoring or limit violations. The effluent sample was collected with an Isco Model 2910 using a time composite at 120 mL/hr. The influent sample was collected with an Isco Model 2910 using a time composite at 120 mL/hr. The sludge removal is contracted through Doug Cox septic service and hauled to the Town of Jefferson WWTP. Action Items • Please send the results of the inflow and infiltration studies, and the date these repairs were implemented. • Please send the information and date the new flow measurement device is installed. • Please send the date and sampling method the flow proportion method will be implemented. • Submit the annual performance report for 2007. In that letter you will note the use of the word "will" in regards to issuing Notices of Violation (NOVs) for Sanitary Sewer Overflows (SSOs). The intent of that statement was to make sanitary sewer system owners and operators aware that each SSO will be evaluated on a case -by -case basis to determine if a NOV is warranted. The statement was not intended to indicate that in all cases an NOV will be issued without a review of case specific factors. You may have also noted a second statement using the word "will' regarding the issuance of civil penalties. Civil penalties are only issued based on case -specific factors that include, but are not limited to the following: volume, volume reaching surface waters, duration, gravity, impacts to public health, fish kills or recreational area closures. Again, the statement is not intended to indicate that in all cases a civil penalty assessment will be issued without review of case specific factors. These sections of sewer include aerial lines, lines contacting surface waters, sub -waterway crossings, siphons, and sections of sewer parallel to stream banks which are subject to erosion and the potential for failure of the sewer line. The sewer line right-of-way must be maintained to allow access to the sewer lines for inspection and repair. A map with dates noted for the work performed or a log could be used for documentation. A general observation of the entire system must be conducted every year and a log (or map) should be maintained to document these observations. Where there are areas that are visible to the general public, right of way maintenance has been conducted or where sewer cleaning has occurred, the 'observation" requirement has essentially been met for that section of line. The lines that are not normally seen by the public or maintenance crews are of main concern for this "observation " requirement. A Grease Control Program must be in place and include the distribution of grease education materials (example attached) to the users of the system twice every year. It must also include the legal means to require grease interceptors for new construction and retrofit of interceptors where necessary and the authority to inspect and enforce for violations. If sewer overflows are caused by grease, education materials shall be distributed more often. An operation and maintenance plan must be developed and implemented which lists the frequency of pump station inspections (see below), preventative maintenance schedule, spare parts inventory, and overflow response plan. Testing frequency for any emergency equipment should also be included in this plan. Pump stations that have operational telemetry must be inspected weekly while those without telemetry must be inspected daily (365 days/ year). The preventative maintenance plan for pump stations should list the frequency to exercise valves, lubricate pumps and mechanical equipment, remove any debris from the wetwell, check operation of alarms, telemetry and any generators. A map of the collection system must be prepared and maintained with any additions to the system. A press release to all electronic and print news media outlets that provide general coverage in the county where the discharge occurred is required within 48-hours of first knowledge of the spill for all discharges of at least 1000 gallons to surface waters. If there is a spill of at least 15,000 gallons to surface waters a public notice must also be issued in a newspaper having general circulation in the County where the spill occurred and in the County immediately downstream. Contact this office if you need any forms or further information on this topic. Records of the spills that are less than 1000 gallons and do not reach surface waters should also be maintained as this information can be utilized to identify obstructions to the lines or excessive infiltration / inflow (1/1). Overflows should be estimated as accurately as possible and attached is one suggested way to accomplish such calculations. If you have developed other ways to estimate the overflow and you feel that they are reliable, please do not change the process that you use, but we would like for you to share your process with us. When a spill is reported, the beginning time for the spill should be the time that you first had knowledge of the spill (not arrival time for your crew). In some few cases there will be evidence to show that the spill began even before your first knowledge and that information can be used to determine the best estimate of the starting time for the spill (dead fish in creek at the site or downstream and even the size of the pooled sewage in the area around the sewer). Please recognize that in accordance with the recent determination by the Director of the Division of Water Quality, enforcement for SSOs will become a high priority for the Division. The decision to enforce will be based on factors including but not limited to, volume released, volume reaching surface waters, duration, gravity, impacts to public health, fish kills and money saved by non- compliance. This office will be increasing the level of oversight, compliance and enforcement relating to collections systems and thus we wanted to be sure you are aware of the requirements for these systems. Testing for water Inflow and Infiltration (I/I) is offered by the NCRWA. The NCRWA conducts these tests using liquid smoke, and the only charge is the cost of the liquid smoke used during the testing. However, the NCRWA requires that each Town have in place an OSHA approved confined space entry program before they can loan the portable electronic equipment necessary for locating major I/I sources in the Town's sewerage system. sew.5ar, CouId USe st 40 Additionally, the NCRWA provides electronic flow meters for the Town's staff to install, and the NCRWA staff collects the water flow data periodically - especially after rain events. Seven additional 1/I sites remain and the property owners are cooperating with the Board of Aldermen to make necessary repairs. This work is expected to be completed by this Fall. Installation of New Flow Measurement Device The Town has selected B.W. Allen Company to install a Stevens AxSys CCR Wastewater Flowmeter and 6 inch circular chart recorder. It will be equipped with 4-20 mA output to accommodate flow proportional sampling. The cost of this project is $3,930.00. Due to budget restraints this work is scheduled to be completed immediately after the start of the coming fiscal year, beginning July 1, 2008. The two influent pumps work on alternating cycles and appeared to operating properly, and there was no evidence of overflows. Both blower pumps were operational, and delivering a gentle rolling action in the aeration basin. The aeration appeared to have old brown foam with approximately 50% coverage on the basin. Mr. Church feels they may have been hit with a slug of grease. The old sludge appearance needs some attention. There was no evidence of straggler flock or solids from the up -flow clarifier cone unit. The discharge was clear. The backwash system is functioning properly. The Sanuril tablet chlorination unit was installed in the final effluent contact tank. One of the four tubes is used for disinfection. The unit is satisfactory. An in -ground tablet dechlorinator (Sanuril model) was installed. Sodium bisulfite tablets are used in one tube. Flow Measurement The recording flow chart is operating, and measures the height of water measured in inches passing through a 60' V-notch weir. The flow calibration log and charts were kept on -site for at least 3-years, and were well documented. The flow meter was calibrated and documented on October 15, 2013. Laboratory Total Residual Chlorine (TRC), Temperature, and pH are performed and recorded on the Daily Monitoring Report (DMR). The field parameters (TRC, Temp, pH) are currently performed under field parameter certification #5419. The pH is performed with a Hach Sension 1 meter using a two -point calibration (pH 4 & 10) and a mid -range check (pH 7), which is considered satisfactory. Mr. Church performs a temperature calibration against a NIST traceable thermometer. Total Residual Chlorine (TRC) is measured with a Hach DR2500, which is capable of detection to permit limit of 28 ug/L. The annual 5-point calibration is scheduled to be performed during October 2013. Big Horse Creek, Class C-trout, HQW waters, appeared to be clean with no visible impacts. Operations & Maintenance Mr. Tim Church is a grade IV operator and is the current ORC. Mr. Fred Walters is the Grade II back-up operator. The facility and grounds are maintained in excellent condition. Plant -Pro (Chlorine 68%) is used for disinfecting, and Plant -Pro (Sodium bisulfite) is used for dechlorination. They appear to work well. Settleability test is periodically performed as process control to judge the operational performance of this facility. For the most part, operation & maintenance is judged by observing plant performance. Self -Monitoring Program A review of the monthly self -monitoring reports from January 2012 through August 2012 showed no monitoring or limit violations. The effluent composite sample is collected with an Isco Model 2910 using the flow proportion method at 200 mU 1000 gal. Sludge Disposal The sludge removal is contracted through Doug Cox septic service and hauled to the Town of Jefferson WWTP. The compliance evaluation inspection is considered satisfactory. I agree this will be a more effective means of doing the job. The unit cost is minimal. However installation will require a moderate amount of labor. My assistant and I will probably do this ourselves and charge the town for the labor. It will require a small excavator to expose the effluent line and we will probably install a small vault in which to enclose the tablet dispenser. I have not shared this with the aldermen, but I do not anticipate any reluctance on their behalf. I will probably inform them in the March meeting. Mr. Tim Church, ORC, was present. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit Your NPDES permit became effective August 1, 2011 and expires March 31, 2016. As discussed during the previous inspection, the Town may request a permit flow of 30,000 gpd, thereby, changing the sampling requirements to a grab sample. This would have been something to consider while the permit was in the renewal process. Records & Reports An activity log was also kept in the building, which logs operator visitation and plant operations. All certificates of analytical analysis were kept on file along with chain of custody forms. Calibration records for pH, total residual chlorine, and temperature were documented. In summary, the records and reports documentation by Mr. Church is considered to be excellent. Facility Site Review The influent manual barscreen is maintained. The two influent pumps work on alternating cycles and appeared to operating properly, and there was no evidence of overflows. There was no evidence of straggler flock or solids from the up -flow clarifier cone unit. The discharge was clear. The backwash system was functioning properly. The Sanuril tablet chlorination unit was installed in the final effluent contact tank. One of the four tubes is used for disinfection. The unit is satisfactory. A sodium bisulfite tablet dechlorination tube was placed after the effluent weir in order to meet the total residual chlorine limit of 28 ug/L. A tablet dechlorinator will need to be installed after the effluent weir box. (Such as a Sanuril model) Flow Measurement The recording flow chart is operating, and measures the height of water measured in inches passing through a 60' V- notch weir. The flow calibration log and charts were kept on -site for at least 3-years, and were well documented. The flow meter was calibrated documented on September 17, 2011. Laboratory Total Residual Chlorine (TRC), Temperature, and pH are performed and recorded on the Daily Monitoring Report (DMR). The field parameters (TRC, Temp, pH) are currently performed under field parameter certification #5419. The pH is performed with a Hach Sension 1 meter using a two -point calibration (pH 4 & 10) and a mid -range check (pH 7), which is considered satisfactory. Mr. Church performs a temperature calibration against a NIST traceable thermometer. Total Residual Chlorine (TRC) is measured with a Hach DR2500, which is capable of detection to permit limit of 28 ug/L. The annual 5-point calibration was performed on December 16, 2011. The current Lansing WWTP facility began operation in 1989 and discharges treated 100% domestic wastewater into Big Horse Creek. In December 2010 this segment of Big Horse Creek was reclassified as C, HQW water in the New River Basin. Big Horse Creek was classified as C, Trout, HQW water prior to this reclassification. Untreated domestic wastewater enters the plant headworks where it flows across a manual bar screen and collects in a sump. A composite sampler collects influent samples from the well. Pumps then transfer the wastewater to the aeration basin where blowers provide the aeration. The wastewater then continues to a clarifier module. The liquid continues from the clarifier, through a tablet chlorinator, through a chlorine contact chamber, and through a tablet dechlorinator. The treated wastewater flows through a flow weir with records effluent flow volumes and is then discharged into Big Horse Creek. An effluent composite sampler collects samples after the meter. When needed the clarifier cones are backwashed and diverted to the aerobic sludge digester. As needed the sludge in the digester is dewatered and hauled by a contractor to Jefferson WWTP for disposal. The Region has noted that this type clarifier system may not be as efficient at maximum permitted flow conditions. There have been some historical problems with undersized and/or poor performing blowers for the aeration basin but they were resolved. Fact Sheet NPDES NCO066028 Page 1 of 3 HISTORY Level B models were conducted for this permitted discharge based on a stream class C and supported secondary treatment standards (designated by the model as water quality limiting) for BOD, TSS, and pH with no additional limits on ammonia or dissolved oxygen. The secondary standard to meet 85% removal of TSS and BOD was also implemented at some point, at or before 2001. Instream and effluent monitoring for fecal coliform was required in 1996. Fecal limits were added in 2001 based on the impact to the receiving stream, compliance was deferred for one year. The instream fecal monitoring requirement was removed in 2006 as it did not apply. The TRC limit was also added in the 2001 permit and compliance was deferred for one year. The installation date of the tablet dechlorination system is unknown. The ammonia toxicity was evaluated in 1988 and the Division determined no ammonia limits were required but ammonia monitoring was required. No WET testing was required. COMPLIANCE Since 2006 several incidences of unreported and/or missing DMR data did occur but were reconciled with the Regional Office. The fecal coliform limits were exceeded on several occasions in late 2009 and early 2010. No fines were accessed. PERMITTING STRATEGY Data Review DMRs were reviewed for the period of January 2009 to December 2010. Receiving Stream Reclassification The reclassification of the receiving stream removed the Trout designation. Based on WQS the dissolved oxygen minimum standard is reduced to 5.0 mg/1 as compared to the Trout classification of 6.0 mg/l. This does not affect any of the facility existing permit requirements. HQW Rules This facility meets existing permitting criteria. However, if this facility ever expands or replaces this treatment works, the HQW rules for new/expanding facilities will apply. Mr. Tim Church, ORC, was present. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit Your NPDES permit became effective April 1, 2006 and expires March 31, 2011. As discussed during the inspection, the Town may request a permit flow of 29,000 gpd, thereby, changing the sampling requirements to a grab sample. This should be done before the new permit is issued. Records & Reports An activity log was also kept in the building, which logs operator visitation and plant operations. All certificates of analytical analysis were kept on file along with chain of custody forms. Calibration records for pH, total residual chlorine, and temperature were documented. In summary, the records and reports documentation by Mr. Church is considered to be excellent. Facility Site Review The influent barscreen is maintained. The two influent pumps work on alternating cycles and appeared to operating properly, and there was no evidence of overflows. The aeration basin was a brown color with white colored foam covering less than 20% of the surface of the basin. 11 There was no evidence of straggler flock or solids from the up -flow clarifier cone unit. The discharge was clear. The backwash system was functioning properly. The Sanuril tablet chlorination unit was installed in the final effluent contact tank. One of the four tubes is used for disinfection. The unit is satisfactory. A sodium bisulfite tablet dechlorination tube was placed after the effluent weir in order to meet the total residual chlorine limit of 28 ug/L. This treatment may be functioning properly because the facility only receives 20% of the permitted flow. In the future, the facility may need a dechlorination box after the effluent weir. This will be evaluated in future inspections. Flow Measurement The recording flow chart is operating, and measures the height of water measured in inches passing through a 60' V- notch weir. The flow calibration log and charts were kept on -site for at least 3-years, and were well documented. The flow meter was calibrated by BW Allen and documented on November 11, 2010. Laboratory Total Residual Chlorine (TRC), Temperature, and pH are perfon-ned and recorded on the Daily Monitoring Report (DMR). The field parameters (TRC, Temp, pH) are currently performed under field parameter certification #5419. The pH is performed with a Hach Sension 1 meter using a two -point calibration (pH 4 & 10) and a mid -range check (pH 7), which is considered satisfactory. Mr. Church performs a temperature calibration against a NIST traceable thermometer. Total Residual Chlorine (TRC) is measured with a Hach DR2500, which is capable of detection to permit limit of 28 ug/L. The annual 5-point calibration was performed. Effluent/Receiving Waters The effluent was clear with no color or solids present at the time of inspection. Big Horse Creek, Class C-trout, HQW waters, appeared to be clean with no visible impacts. FE I February 24, 2011 Page 3 of 3 Operations & Maintenance Mr. Tim Church is a grade IV operator and is the current ORC. Mr. Fred Walters is the Grade II back-up operator. The facility and grounds are maintained in excellent condition. Plant -Pro (Chlorine 68%) is used for disinfecting, and Plant -Pro (Sodium bisulfite) is used for dechlorination. They appear to work well. Settleability test is periodically performed as process control to judge the operational performance of this facility. For the most part, operation & maintenance is judged by observing plant performance. Self -Monitoring Program A review of the monthly self -monitoring reports from January 2010 through December 2010 showed a weekly average limit violation for Fecal Coliform during November 2010. The effluent composite sample is collected with an Isco Model 2910 using the flow proportion method at 200 mL/ 1000 gal Sludge Disposal The aerated digester was approximately 50% full at the time of the inspection. The sludge removal is contracted through Doug Cox septic service and hauled to the Town of Jefferson WWTP. The compliance evaluation inspection is considered satisfactory. This is in violation of Part II, condition D(2) of the NPDES permit, as well as 15A NCAC 213 .506(a), which requires the submittal of Discharge Monitoring Reports no later than the last calendar day following the reporting period. Failure to submit reports as required will subject the violator to the assessment of a civil penalty of up to $25,000 per violation. You will be considered noncompliant with the self -monitoring requirements of your NPDES permit until the report has been submitted. To prevent further action, please submit said report within fifteen (15) days of receipt of this notice.
