At Water Quality Labs request the sample was analyzed out of holding time. The sample was collected May 15, 2015 and received by Environmental Testing Solutions on June 29, 2015. Chloride has a maximum holding time of 28 days, which was exceeded. Additionally, the chain of custody was not fully filled out, which could compromise the legal defensibility of the data. Please be advised that under provisions of North Carolina Regulation, 15A NCAC .0805 (a) (7) (N), any time a laboratory receives samples that do not meet sample collection, holding time, or preservation requirements, the laboratory must notify the sample collector or client and secure another sample if possible. If another sample cannot be secured, the original sample may be analyzed but the results reported (on both the client report and the Discharge Monitoring Report or other regulatory compliance report submitted to the state of North Carolina) must be qualified with the nature of the infraction(s), and the laboratory must notify the State Laboratory about the infraction(s). The notification must include a statement indicating corrective actions taken to prevent the problem for future samples. Failure to comply with these requirements may result in enforcement action. I realize you purchased that Hach Pocket Colorimeter II in good faith thinking it would meet the requirements. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of. facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit Your permit became effective April 1, 2011 and expires on March 31, 2016. Records & Reports Mr. Howell maintains a daily log that appears to be in good order. The log shows visitation times, process controls (settleability, alkalinity, residual chlorine before dechlorination), and documents plant observations. Laboratory results and chain -of -custody forms are kept on file. Facility Site Review The aeration basin had a good gentle rolling action, approximately 5% of the surface was covered with tan foam. There was no odor. The # 1 motor (5 horsepower) was not working. A new motor is ordered. The clarifier effluent was clear. The clarifier was covered with floating solids. The tablet chlorinator used four (4) tubes with Norweco tablets, and appeared to be working properly. The March 2014 DMR shows effluent pH ranged from 7.44 to 8.83 for the first twelve days of the month of March. Since, there is no I &I problems at this plant, it appears the plant received a slug load containing high pH chemicals. As discussed during the inspection, a partial list of the following chemical are used in Meth Labs: Acetone, Rubbing Alcohol, Toluene (brake cleaner), Anhydrous Ammonia, (farm fertilizer), Lye, Red Phosphorus (matches/road flares), Trichloroethane (gun scrubber). Flow Measurement Instantaneous flow is measured with a 22%2° V-notch weir. The height of the water in the weir is measured in inches, and it is converted to flow (MGD) by reading an Isco flow table. An ultrasonic device was installed that gives a direct flow reading. The flow was calibrated and documented on May 29, 2014 by Butch Howell. The instantaneous flow at the time of the inspection measured with an Isco 4210 and ultrasonic sensor was 1.6 gpm (0.0023 MGD). Laboratory Field certification for pH, total residual chlorine, temperature was issued Mr. Tom Kilpatrick for Buffalo Meadows (lab #5037). Mr. Howell performs pH, total residual chlorine, temperature; and these tests are recorded on the daily monitoring report. The total residual chlorine is performed with a Hach DR2400 spectrophotometer using the DPD colorimetric method with a flow cell. The annual 5-point calibration was performed on February 25, 2014. A Hach pH meter is used. A 4 and 10 buffer calibration is performed and a check at 7.0 SU. The calibration records are well documented. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters At the time of the inspection the discharge to Buffalo Creek, Class C-trout waters were clear with no foam or solids. The receiving waters did not appear to be impacted. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Tim Church, Grade IV. Mr. Howell indicated that the influent tank acts as an oil & grease trap and collects solids, which is pumped approximately once every six months. Compliance Evaluation Inspection June 2, 2014 Page 3 of 3 Mr. Howell indicated that the plant is not experiencing inflow & infiltration (I & I) problems. Mr. Howell uses Settleability (-50%) and the observed thickness of the Return Activated Sludge (RAS) as process control to operate the plant. This plant appears to be very labor intensive to operate, in order to maintain compliance. Self -Monitoring Program A review of the monthly self -monitoring reports from July 2013 through March 2014 did reveal the following limit violation: Parameter Date Type Flow July 2013 Monthly Average Sludge Handling Disposal Permit Limit DMR 0.010 MGD 0.0156 MGD Sludge is removed from the plant by contracting Myers Septic Service. Conclusion During the inspection last year this office has some concern that the plant is very labor intensive to operate at optimum efficiency. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of. facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit Your permit became effective April 1, 2011 and expires on March 31, 2016. Records & Reports Mr. Howell maintains a daily log that appears to be in good order. The log shows visitation times, process controls (settleability, alkalinity, residual chlorine before dechlorination), and documents plant observations. Laboratory results and chain -of -custody forms are kept on file. Facility Site Review The aeration basin had a good gentle rolling action. There was no odor. There were two new 7 % horsepower pumps and blowers installed. The clarifier effluent was clear with no solids over the weir. The tablet chlorinator used four (4) tubes with Norweco tablets, and appeared to be working properly. The tablet de -chlorinator used four (4) tubes with Norweco (85% NaSOA and appeared to be working properly. Flow Measurement Instantaneous flow is measured with a 22% V-notch weir. The height of the water in the weir is measured in inches, and it is converted to flow (MGD) by reading an Isco flow table. An ultrasonic device was installed that gives a direct flow reading. Per the permit only instantaneous flow is required. Flow was calibrated by Horizon Engineering on March 5, 2013. Laboratory Field certification for pH, total residual chlorine, temperature was issued Mr. Tom Kilpatrick for Buffalo Meadows (lab #5037). Mr. Howell performs pH, total residual chlorine, temperature; and these tests are recorded on the daily monitoring report. The total residual chlorine is performed with a Hach DR2400 spectrophotometer using the DPD colorimetric method with a flow cell. The annual 5-point calibration was performed. The temperature is recorded against a MST traceable thermometer. Effluent/Receiving Waters At the time of the inspection the discharge to Buffalo Creek, Class C-trout waters was clear with no foam or solids. The receiving waters did not appear to be impacted. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Tim Church. Mr. Howell indicated that the influent tank acts as an oil & grease trap and collects solids, which is pumped approximately once every six months. Mr. Howell indicated that the plant is not experiencing and inflow & infiltration (I & I) problems. The aeration basin had a good gentle rolling action. The diffusers were replaced, which allowed for a higher mixed liquor, thereby, giving better treatment. This also reduces the amount of time needed to operate and maintain the plant in order to meet the permit limits. Mr. Howell uses Settleability (-40%) and the observed thickness of the Return Activated Sludge (RAS) as process control to operate the plant. V 2013 of 3 Self -Monitoring Program A review of the monthly self -monitoring reports from April 2012 through January 2013 did reveal the following limit violations: Fecal April 3, 2012 Daily Maximum 900 #/100mL Fecal January 8, 2013 Daily Maximum > 600 #/100mL Sludge Handling Disposal Sludge is removed from the plant by contracting Myers Septic Service. 11 To 1 IT i1 During the inspection last year this office has some concern that the plant is very labor intensive to operate at optimum efficiency. The addition of new blowers and repaired diffusers appears to have made a remarkable difference. Mr. Howell continues to operate this plant and provide excellent technical knowledge. You are compliant with this reporting requirement. Facility Site Review Upon arrival the blower motor tripped off and there was no aeration or activated return operating. The aeration basin was slightly dark brown and had a slight septic odor. The other blower motor was being repaired. The blower motor was reset and the aeration basins operated with a good rolling aeration on the surface. The clarifier had a slightly dark color and slight septic odor. The plant recovered well in about 30 minutes and the blower did not trip off again. The tablet chlorinator used four (4) tubes with Norweco tablets, and appeared to be working properly. Conclusion During the inspection last year this office has some concern about the high clarifier blanket. Any solids discharge from the clarifier will be collected in the chlorine contact basin. This demonstrates that the plant is very labor intensive to operate at optimum efficiency. A review of past data reveals that limit violations over the last year can be traced to plant operations or possibly a plant in need of repair. This office will consider present and future violations for enforcement actions. Mr. Howell continues to operate this labor intensive plant and provide excellent technical knowledge. The compliance evaluation inspection is satisfactory, but it is of the opinion of this office that the plant requires higher than normal operation & maintenance activities. Issues of concern are: -general housekeeping. Naturally, that falls directly to me and will be corrected. The very junk of your concern is now a concern of NCDENR and will be corrected. -George Smith, NCDENR, talked quite sternly to me regarding our violations of in the past year. BOD/TSS, and Fecal Coliform. He pressed very hard for quantifiable changes to rectify these violations and stated that future violations would result in him recommending $$$ associated with each. Of particular note was our repeated reporting of high NH- 3 numbers. While not in violation of permit limitations (we have none) it was an area of his concern and in that it demonstrates a problem with either nitrification in the aeration basin or a denitrification problem in the downstream tanks (old concrete tank and the c12 chamber or both). I'm certainly inclined to agree. To correct this problem, my recommendation In the short term is to pump the downstream tanks whenever we note an increase in BOD/TSS, or NH-3 concentrations. (allowing of course some latitude in NH-3 increases due to colder weather or possible over WAS events). This, of course, is going to cost more in pumping and disposal costs. Long term, well..... almost endless possibilities. None of which are attractive from a cost perspective. I have considered transitioning the concrete tank where the old building stood into another c12 chamber whereby during times of cleaning, supernatant could be withdrawn and only thickened solids removed by the pumper truck. Just and idea...... Clearly, any decision will involve an expenditure, and will be dependent upon your wishes. Just let me know what, if any, corrective action course you wish that I take. I am still fighting motor/blower issues. As fate would have it, the 5hp motor/blower was inop when George Smith inspected. The motor was tripped at the control panel, and from the slight septic odor, had probably been so since late the evening prior. While making my checks the day before the inspection, I had found the motor off in similar fashion and reset it. As of this morning, the motor had not tripped again. I am at a loss as to why we continue to have electric motor issues of the magnitude we do. I have been exploring replacement options on the 7.5hp motor that failed last month. If you recall, the 7.5 hp motor AND the spare would not run. Both of these motors were sent to the motor rewind shop for examination/repair. Both had winding shorts, indicating that either one or both must be rewound (not recommended, but an option) or replaced. The findings on this are listed below and again will depend on how much money we want to throw at it. I recommend the slightly more expensive option of changing this 7.5 hp motor to an identical configuration as the 5 hp motor for ease 1 of interchangeability and lower motor replacement cost later. The 7.5 hp motor/blower configuration, in my opinion, is overkill. 7.5 hp motor options/costs Rewind present motor(s) - $550.00 ea. China motor replacement - $552.90 + $100.00 freight = $600.00 each Baldor brand replacement motor - — $925.00 5 hp motor options/costs to replace the above 7.5 hp configuration Conversion base (7.5 hp motor frame to 5 hp motor frame direct fit) _ $70.00 China motor = $405.00 + $100.00 freight = $505.00 Baldor brand motor =—$800.00 New heaters for starter—$30.00 Estimated total for 7.5 hp to 5 hp conversion =—$605.00 This is assuming no sheaves or belts will be needed for either application. Dear Tom: Tom, sorry I have not responded earlier, but I've been sidetracked with the Carolina Water rate case, the CWS Systems rate case filing, and the Aqua rate case filing. My concern/objection was and remains giving A&D a check to pay for repairs/improvements based on estimated costs. To do so will jeopardize the utility by having additional repairs/improvements arise after said money is paid and the seller has left the scene. The method you propose in your e-mail and attached letter is agreeable to the Public Staff with the provisos that (1) all repairs/improvements are identified by and agreed to by the appropriate DENR Regional office (Asheville, Mooresville, Winston-Salem?) prior to the work, (2) that DENR issues any necessary plan approval letters/permits for the work, (3) that DENR approve all abandonments of wells and other infrastructure (we assume these costs will be included in the payments), and (4) that DENR inspect and "approve" the work when completed. The Public Staff assumes such work, while performed by your firm, will be for the CPCN holder for each system. If DENR signs off on the above, then the Public Staff will do what it can to assist in an orderly and efficient transfer of the Culligan systems to A&D. All parties must understand that the Public Staff will be required to notify customers of the transfers, that hearings may be required depending on customer response, and that any increase in rates will, of necessity, require more time for review and recommendation. The Public Staff will be recommending a bond be posted in the amount of between $70,000 and $100,000. We strongly encourage you to negotiate for that bond to be included in the amount to be paid by CulliganNiola. An additional concern is how A&D will operate/oversee/manage the systems in the counties removed from its current systems in Henderson, Buncombe, and Transylvania counties and again assume that will be settled at the time of the transfer application. While the NCUC does not regulate your firm, I do request that you keep me informed as the repair/improvement projects proceed. Of course, this causes a backup into the plant. The outfall was still under water this morning at 0630 hours. I will continue to monitor the situation and will keep you posted. -hutch George Smith PROPOSED MAINTENANCE, REPAIR & IMPROVEMENT ITEMS BUFFALO MEADOWS SUBDIVISION PSID # NC 01-05-102 WATER SYSTEM — including well, well house, ground storage tank, distribution system and associated grounds: ENERAL CONDITIONS The system is an older system using one well. There is no backup well. The mains are deep in some areas and the exact location is unknown. No plans and/or profiles are available. A hand drawn sketch is available. The system is unmetered. Some services have cutoffs, some don't and some are buried. Some homes have internal cutoffs and some don't. There is no way to uniformly cut service for non-payment. Outstanding arrears for the past 10 years exceed $50,000. Several main line valves were installed in 2010 to assist in finding leaks. Some homes have been detected for lead & copper. Recommendations: 1. Well house needs siding, insulation, roofing, door, interior paint, lighting, and minor electrical repair; 2. Needs and auto -dialer to report low pressure, loss of power, etc.; 3. The lead and copper corrosion control system (approved and ATC now in hand) needs to be installed; 4. Water meters required and to be installed (approx. 61 required); 5. The water tank needs external and internal inspection, sanded, painted and repaired as required. The tank overflow, access ladder and perimeter fencing needs repair. NPDES # NC0030325 WASTEWATER SYSTEM — including WWTP, collection system and associated grounds: GENERAL CONDITIONS The WWTP is an old, in -ground concrete 10,000 gpd system. The collection system is 8-inch PVC and has had few problems. Recommendations: 1. WWTP and equipment needs painting; 2. Unit needs a flow meter; 3. One tank needs to be removed and re -piped; 4. Safety grating needs replace me nt/repa i red/pa i nted as needed; 5. Diffusers need to be replaced (fine bubble type is recommended); 6. Replace approximately 300 If of 1-inch service line under the paved road and install new frost proof hydrant; 7. Install a metal type storage building & slab for chemical storage; 8. Repair existing fence as needed; 9. Construct aluminum safety stairs & handrails to access top of digester; construct safety handrails around top of digester area; 10.
