Permit Modification Application for:
City of Burlington, Residuals Land Application Program
Permit No. WQ0000520
On June 21 and 22,2018, WSRO staff met with Mr. Shane Fletcher, ORC (City of Burlington)to
review proposed modifications to existing land application fields included in the application for
permit modification dated April 24, 2018. This inspection report confines comments to the review
of the proposed modifications only.
Based upon WSRO's review of the subject modifications to the existing fields, the items
described below need to be addressed through an additional information request prior to the
permit issuance.
Site AM-16
Field #1: There are rock outcrops present in the field which are not shown on the new
modified map. There is also a rock outcrop present in the new area to be added to the field
where the property line buffer was removed. Please add the previously shown rock
outcrops and the new rock outcrop with the associated buffers to the site maps.
Field #3: There are rock outcrops present in the field which are not shown on the new
modified map. Please add the previously shown rock outcrops with the associated
buffers to the site maps.
Field #5: There are rock outcrops present in the field which are not shown on the new
modified map. Please add the previously shown rock outcrops with the associated
buffers to the site maps.
Field #6: There is a new area shown on the modified site map below the pond in the
northern section of the field. This proposed new area needs to be removed from the map
due to unsuitable wetness. Please remove this proposed new area from the site map.
Field#6: There are rock outcrops present in the north and south sections of the field which
are not shown on the new modified map. Please add the previously shown rock outcrops
with the associated buffers to the site maps.

Site AM-116
Field #12: The new area located in the southeast area of the southern section of the field
has some slopes greater than 10%. Please revise the site maps to show the area with
slopes greater than 10% as buffered out of the land application area.
Site AM-118
Field #2: There are rock outcrops present in the field which are not shown on the new

buffers to the site maps.

Site AM-119
Field#1: There are rock outcrops present in the new area of the field which are not shown
on the maps. Please add the rock outcrops with the associated buffers to the site maps.
Site CS-36
Field #9: There is a well present near the western border of Field #9 which is not shown
on the new modified map. Please add this well with the associated buffers to the site
maps.
Field#10: There is a drainage feature which is not shown on the new modified map. Please
add this drainage feature as previously shown with the associated buffers to the site
maps.
Site CH-98
Field #5: There is a proposed new area of the field located on the north end of the field.
This new area is overgrown with trees&shrubs,and cannot be land applied to in the current
condition. Mr. Fletcher indicated during the site visit that they will remove this section on
a revised map. Please remove this new area from the application area on the site maps.
Field #6: There is a proposed new area located on the east end of the field. This new area
has an unsuitable depth to seasonal high water table (SHWT). Please remove this
unsuitable area from the application area on the site maps.
Field #9: There is a drainage feature present on the north side of the field which is not
shown on the site maps.Please add this drain and the associated buffer to the site maps.
Field#10: There are rock outcrops present near the center of the field which are not shown
on the maps. Please add the rock outcrops with the associated buffers to the site maps.

Site OR-64
Field#5: There is a proposed new area located in the southern corner of the field. This new
area is to be removed. Please remove this unsuitable area from the application area on
the site maps.
Site OR-79
Field#2: There is a proposed new area located on the east side of the field,next to the flood
zone. This new area is to be removed. Please remove this unsuitable area from the
application area on the site maps.
Additional notes and observations from the site reviews are listed below.
• All existing permitted sites that were not specifically mentioned in the application are to
remain in the Attachment B with no changes. Per Mr. Fletcher.
• Some sites had no modifications shown to the maps other than being redrawn using GIS.
The acreages changed on many of these sites due to more accurate mapping versus the old
hand drawn maps.
• Soil hand auger borings were placed in some of the proposed new areas of fields. See field
notes for additional details and field maps for the locations of borings.
• The following sites/fields which had proposed modifications were not reviewed onsite due
to access restrictions, crops present in the field, tall vegetation, and/or time constraints:
AM-18 Field #2, AM-116 Field #3/#4, AM-118 Field #3, AM-126 Field #9, and
CS-113 Field #4. Requested the Permittee (Mr. Fletcher) contact WSRO to allow site
review prior to the next application event to these sites.
The above described revisions and concerns were verbally discussed with the Permittee following
the site visits. Please let me know if you need additional information or have any questions.
