Dear Mr. Schertz:
On December 28, 2009 the Division of Water Quality (DWQ) received the application sets dated
December 4, 2009 for the above referenced project. The DWQ has determined that your application was
incomplete and/or provided inaccurate information as discussed below. Please provide the following so
that we may review and approve your project.
Additional Information Requested:
1. The application indicates Riparian Buffer Authorization is requested however, the PCN failed to
reflect any recorded buffer impacts on pages 5 of 10; 6f and/or 6g. Please supply one (1) original
and four (4) copies of just the impacts page to continue the review and approval process.
2. The signature (William Vogg) on page 10 of 10 is not reflected on the PCN as agent. The
signature of William Vogg would only be valid with a signed agent authorization from the
applicant.
Until the information requested in this letter is provided, I will request (by copy of this letter) that the
Corps of Engineers place this project on hold. Also, this project will be placed on hold for our processing
due to incomplete information (15A NCAC 2H .0507(a)).
