Dear Mr. Smith,
A review of the self -monitoring data for the subject facility has been completed. This included
review of the Non -Discharge Monitoring Reports (NDMR), Non -Discharge Application Reports
(NDAR), and Groundwater Monitoring Reports (GW-59) for the period of January 2019 through
December 2019. The violations indicated below were noted during this review:

As required by Permit Condition IVA, an effluent sample is required to be sampled three times a
year (March, July, and November). No effluent was collected and none of the required parameters
were analyzed in July 2019. Remedial actions, if not already implemented, should be taken to
correct any noted problems. The Division of Water Resources may pursue enforcement actions for
this and any additional violations.
As a reminder, your permit requires that you notify the Winston-Salem Regional Office of any non-
compliance as soon as possible, but in no case more than 24 hours from first knowledge of
occurrence. Please provide a response to this letter within 60 days of receipt by your office
indicating the corrective actions taken to address this violation.
