Dear Mr. Hatchett,
A review of the self -monitoring data for the subject facility has been completed. This included
review of the Non -Discharge Monitoring Reports (NDMR) and Non -Discharge Application
Reports (NDAR) for the period of June 2019 through January 2020. The violation indicated below
were revealed during this review:

As required by Permit Condition IV.4, an effluent sample is required to be sampled three times a
year (January, May, and September). No effluent was collected and none of the required
parameters were analyzed in January 2020. Remedial actions, if not already implemented, should
be taken to correct this problem. The Division of Water Resources may pursue enforcement
actions for this and any additional violations. Please provide a response to this letter within 60
days of receipt by your office indicating the corrective actions taken to address this violation.
As a reminder, your permit requires that you notify the Winston-Salem Regional Office of any
non-compliance as soon as possible, but in no case more than 24 hours from first knowledge of
occurrence.
