Inspection Summary:
Staff of the Mooresville Regional Office (MRO) conducted a compliance inspection on January 27th, 2020. The MRO would
like to thank Mrs. Bass and Mr. Sid Gaskins (system contractor) for their time during the inspection. Based on the site visit
and the January 29th, 2020 email communication received from Mr. Gaskins, necessary repairs and routine maintenance
items were completed.
The following items of interest are noted:
• Per our phone conversation today, your home is sold and you are moving; but, your current contact information (email and
phone) is valid for the remainder of this year. A hardcopy of this report will also be mailed to the current resident.
The Change of Ownership, Renewal Application and Operations and Maintenance Agreement Forms are attached to this
email for permit transition to the new owners.
For future reference the forms may also be located at the NCDEQ website below.
https://deq.nc.gov/about/divisions/water-resources/water-resources-permits/wastewater-branch/non-discharge-permitting-unit
/application#application-forms. Scroll the page to the "Single -Family Residence Wastewater Irrigation Systems" section.
• This system has historically shown significant organic material at the filter and growth of biomass throughout the system
(vs. occupancy of two adults for the past several years). Mr. Gaskins cleaned all filters during the site visit. The main filter
may require more frequent cleaning than is currently scheduled with the system contractor. Your contractor can advise or
train you on filter cleaning between visits.
Please contact Maria Schutte by email maria.schutte@ncdenr.gov or phone (704-235-2184) should you have questions.

*** The septic tank and filters should be checked annually and pumped/cleaned as needed.
Is all wastewater from the home connected to the septic tank? ❑ ❑ ❑
# Does the permittee/resident know where the septic tank is located? ❑ ❑ ❑
Has the septic tank been pumped in the last 5 years? ❑ ❑ ❑
If YES, describe if known and proof (include date pumped):
Current owners have only been in the home for a couple of years. Contractor measured levels at 2 inches for
bottom solids and surface scum.
# Does the septic tank have an EFFLUENT FILTER or SANITARY T? 0 ❑ ❑ ❑
If FILTER, when was the filter cleaned and by who? Today by
contractor.

# Does the permittee know where the sandfilter is located? 0 ❑ ❑ ❑
Does the sandfilter require maintenance? ❑ ❑ ❑

# Who completes the weekly check for the UV? ( Non -Discharge)
Reason for Visit: Routine
Owner
Comment: The only safe way to check UV on this system design, is to feel the control box for
warmth/vibration, per discussion with the contractor. Mr. Bass travels often, but attempts to
view the system weekly and maintains a notepad. Per our 2-19-2020 phone conversation, his
notepad is packed as the home sold and they are moving.
Pump Tank Yes No NA NE
*** All pump and alarm sytems shall be inspected monthly. (Non -Discharge)
Is the pump working? 0 ❑ ❑ ❑
Is the audible and visual high water alarm operational? 0 ❑ ❑ ❑
# Does the permittee know how to check the pump & high water alarm? ❑ ❑ ❑
# Last functional test: During site visit.
Comment: The float is on delay to contact Mr. Gaskins, so it will not trigger the alarm system (in a timely
manner) while on -site. The contractor triggered the alarm at the panel. Both audible and visual
were operational at the time of inspection. The visual is rather dim and angled in a manner that
seems it would be difficult to see at the home. But, that is how the system was
designed/installed.
Drip or Irrigation Yes No NA NE
*** Irrigation sysetm shall be inspected monthly to ensure system is free of leaks and equipment is operating
as designed. ***
# Type of system (DRIP or IRRIGATION): Drip
# If IRRIGATION, number of sprinkler heads: NA
Are buffers and setbacks adequate? ❑ 0 ❑ ❑
Is the site free of ponding and runoff? ❑0 ❑ ❑
Does the application equipment appear to be working properly? ❑ ❑ ❑
Is there a minimum two wire fence surrounding the entire irrigation area? 0 ❑ ❑ ❑
Comment: There is a waiver with this system, because the drip lines encroach on the fifty -foot setback from
the access road leading to a neighbor's property. The waiver was signed and filed with Union
County Register of Deeds when the current owners acquired this permit.
It was raining during this site visit. There was minimal ponding at a small portion of the drip field
near a pate. The drip field (and a sloped piece of land in the back corner) is the driest area
throughout this property. The surrounding grounds were saturated.
General Yes No NA NE
# Are the treatment units locked and/or secured? 0 ❑ ❑ ❑
# Has resident had any sewage problems? ❑ 0 ❑ ❑
If YES, explain:
The resident has not contacted their contractor with any sewage concerns.
Does the system match the permit description? ❑ ❑ ❑
If NO, explain:
Is the system compliant? ❑ ❑ ❑
