Please check the Permit Contacts Report for this permit (also attached).
1) Please notify Deborah Reese of any changes to Billing Contact. She is the only one who can change that. (I see
there are overdue fees for this permit - so that's important to resolve with her.)
2) Please fill out the Owner Affiliation Change form on-line if Andrew Sauber has changed
(https://edocs.deg.nc.gov/Forms/SW-Owner-Affiliation).
3) Please e-mail Suzanne McCoy (copied here - she is our Stormwater Admin) about changes to the other contacts
listed on the bottom of the report. She can change the ones aside from billing contact.
4) Please ensure that you have a copy of the current COC on hand. I have attached a copy that was obtained on
6/22/2018, as well as the request form, just for your records.

Good morning Ms. Georgoulias,
My name is Deborah Byrd. Over the last year and half, there have been a lot of changes here at Cooper Standard
Automotive Goldsboro. In that time, our plant manager left, our HSE manager left (he was responsible for renewing this
permit) and our HR manager left. None were replaced due to this facility is closing and will be closed by Nov. of this
year. We need to change the names of the people that will be ensuring compliance and their emails. Can you tell me
how do I go about doing that? I am currently responsible for HR and HSE and our Engineering manager is acting as the
plant manager.
