Dear Ms. Georgoulias,
The purpose of this letter is to submit a Stormwater Permit Rescission Request for Mannington
Wood Floors (Mannington). Phoenix Environmental, Inc. (Phoenix) completed the
accompanying Rescission Request Form for existing stormwater permit (NCGNE0791). It is
noted Mannington operated under NCG210014 in the past but that permit was rescinded when
the Certification of No Exposure was established.
Mannington is located at 1327 Lincoln Drive, High Point North Carolina 27260. The facility will
cease manufacturing on April 24, 2020. This permit rescission request is being made for April
24, 2020 to coincide with the shutdown date.
