If yes, please explain: The Oaks farm is a laroe 700+ acre farm that is fairty remote. with the nearest
residence (1) bein_ approx. 400' from field LL-6 along; Dies Rd iSR 1733) and a second residence approx.
800' from field LL-6. The remaining fields used for land application of residuals are over 2000'+ from the
two residences. No complaints have been received in the Fayetteville Regional Office during the last three
rears and the last permit renewal cycle concerning the Oaks Farm. Residuals land application has not
taken place for the past three or four years. Currently. the residuals that use, to be land applied at the farm
are being hauled to the McAlpine WWTP vrimarily and other CMUD wastewater treatment 121ants in the
Charlotte/Mecklenburgcounty area. Future land application of residuals at the Oaks Farm/Liquid Waste
will be dependent on volumes beiny- pumned for clients and access to the CMUD wastewater plants.

Willcox of Willcox & Mabe.

If yes, please explain: The "Fields" tab in BIMS has been updated I during the permit renewal review in
2018 with information provided in the re port by Wilcox & Mabe on the LASC 06-16 form to accurately
identify the new field configuration (i.e. new field numbering), acreage and predominant soil tvves. The
new buffer map represented by Fig. No. 2 is accurate and representative of the fields at the Oaks
Farm/Liquid Waste. Field acreages have been updated to reflect combing, fields due to current cropping
ractices row crops verses pasture/ha • fields and to better reflect field mana-ement and to remove LW-
14 from the Attachment B. Lat./Lonr, coordinates provided by Wilcox & Mabe are accurate and have been
plotted on Goa=le Earth and shown on the attached ma .

V. ADDITIONAL REGIONAL STAFF REVIEW ITEMS
The Oaks Farm/Liquid Waste Incor lorated has not land applied residuals for several years(including 2019 and has
converted several fields from pasture/h4y fields to row crop co soybean, wheat rotation to accommodate the
farmer leasing -the Oaks Farm fields. The farm and land application fields are well managed by Ronnie Oaks and his
sons.
The row crop farmer has leased the Oaks Farm fields for the past two growing seasons, planting soybeans. In the
coming ears L & L hol2es to have residuals to land apply so the farmer can rotate to corn and wheat.
Field LW-14 was removed during the 2018 permit renewal review. This was done to allow Ronnie Oaks/Liquid
Waste the flexibility to utilize this field for se toe land application- if se to a amounts increase in the future. Li uid
Waste may incorporate the acrea a from former field LW-14 into the DWM Se to a land a lication permit in the
future.
The current ORC and Back -Up ORC are correctly identified in Section III item 1 of this staff report. Rob Willcox
was informed that BIMS does not identif either operator and the current ORC identified has an invalid certification.
Rob indicated he would download the Operator Designation Form WPCSOCC and provide to Ronnie Oaks so Cla
Oaks and GaU Garrison can sign and send to Beth Buffin on to update BIMS moving forward.

The North Carolina Division of Water Resources conducted an inspection of the L & L Environmental
Services RLAP on 7/31/2019. This inspection was conducted to verify that the facility is operating in
compliance with the conditions and limitations specified in Non -discharge Permit No. WQ0014843. The
findings and comments noted during this inspection are provided in the enclosed copy of the inspection
report entitled "Compliance Inspection Report".
