That last paragraph should say:
Samir and Thom — WaRO needs to help us figure out the status of this operation. Based on the owed fees, this
permittee shouldn't have an active permit. Before we inactivate this one based on no response, can WaRO determine
if they are still in business and need a permit? If so, they can come into compliance under this COC before we eliminate
it; otherwise, once it's gone, they'd have to apply for a new permit.

Suzanne, this one is weird. The fees in BIMS were waived several years ago (2000-2005), and there are several
correspondence letters with this permittee about owing them. The 2006 letter implies persistent compliance
issues. I'm not sure this permit COC should even be active — it probably got swept up in the batch of automatic renewal
cycles ever since because with "no fee"/waived fee status, it would not have come up in our "fees owed" data pull and
gotten held back from a renewal. That was our oversight.

I'm guessing it should have been inactivated at the time those never -paid fees were waived because of no response
from the permittee (obviously they never responded to the letters), but that event was never entered. The 2006 is the
most recent correspondence in the file on this one.
Dan and Brian — WiRO needs to help us figure out the status of this operation. Based on the owed fees, this permittee
shouldn't have an active permit. Before we inactivate this one based on no response, can WiRO determine if they are

No they have not been corrected. I do not have authority to edit in the billing tab. This is intentional, otherwise all
billing tasks would have been transferred to me.
I am suspicious that these are all "renewals" and if I am doing something wrong when entering them in BIMS, if there is
a step I must/must not do that has caused these to become "NO FEE"

Have these been corrected in BIMS so they are invoicing a fee now? NCG080455 should still have no fee. I haven't

All, just below is what Tom sent. There is one permit here that still should not receive invoices: NCG080455. It was
made NO FEE intentionally [more discussion on that below the table], and it's expired. The rest are active permits that
probably ought to be receiving invoices.
These are the NO FEE ones ... there were others that had the wrong fee that are not in this table (but identified earlier I
think).

Here is what I find for current active/expired NPDES SW permits and most of these have a history of payments in the
past. I have included the total count of annual invoices as well as the last Permit Annual Fee event for each of the
permits. It looks like a lot of these have all their past invoices current through last year except for the 4 indicated in
YELLOW below.

For some reason, these all had a fee category and invoices at one time but were given a No Fee within recent year or
so. No new invoices will be generated once that fee category has been changed to NO FEE.
We need to be ensuring that renewals and new NOls are not getting set up without a fee. I did this accidentally for a
mod once, so it wasn't a step I'd ever been familiar with either! I'm not sure how it the process works for a brand new
application. When a mod is issued, it becomes a new version of that permit, so the mod needs the fee associated when
it is first set up in BIMS.
Okay, for NCG080455: Sue White and Brandon Wise at WSRO had worked on this one, and ultimately we advised the
permittee that they did not belong under NCG08. I think there was some follow up that was supposed to happen from
the region, but Sue left to come here around that time, and now Brandon isn't there any longer. See the email below
that is in Laserfiche. We intentionally did not renew this COC, and I had Deborah stop fees for this one so they couldn't
argue that they maintained coverage. This was important because normally invoices keep going out for expired
permits. There's been no further action (i.e., rescission request), and I'm thinking we should make this permit inactive
based on no response from the permittee, but I don't know if that's acceptable. It is the only sure-fire way this
"exception" wouldn't start getting billed in the future when I'm not around to explain what happened if the question
ever comes up again.

These non-stormwater discharges are not authorited under General Permit NCGO80000. That NPDES general permit
only covers stormwater discharges —not any other sources. even if it is potable water. These discharges should be
routed to the sanitary sewer, or the facility needs to obtain an alternative Wastewater permit. Please see the excerpt
from the front section of our general permit below_ Please contact the D1NR Wastewater Program for further directic
if the site will pursue a wastewater discharge permit. I've copied John Hennessy on this email, who can provide
further guidance.
I realize the situation at hand has been thoroughly confusing. However, the fundamental issue here is that the
discharges from this O/W separator are not entirely from stormwater, and therefore they must be permitted and/or
disposed of differently. The NPD S stormwater permit COC this site currently holds does not apply to the discharges p
describe.
