Failure to submit all required items will lead to your application being returned as incomplete or additional processing and review time. This form may be photocopied for use as an original. ® I. Application Form - Submit one original and one copy of the completed and appropriately executed application form. Any changes to this form will result in the application being returned. The Division of Water Quality (Division) will only accept application packages that have been fully completed with all applicable items addressed. You do not need to submit detailed plans and specifications unless you respond NO to Item B(12). ® Il. Application Fee - Submit a check in the amount of $400 made payable to: North Carolina Department of Environment and Natural Resources (NCDENR). ❑ III. Certificates of Public Convenience and Necessity — If the application is being submitted in the name of a privately -owned public utility, submit two copies of the Certificate of Public Convenience and Necessity (CPCIJ) which demonstrates that the public utility is authorized to hold the utility franchise for the area to be served by the sewer extension. If a CPCN has not been issued, provide two copies of a letter from the North Carolina Utilities Commission's Public Staff that states that an application for a franchise has been received, that the service area is contiguous to an existing franchised area, and/or that franchise approval is expected. The project name in the CPCN or letter must match that provided in Item A(2a) of this application. ❑ IV. Operational Agreements — Submit one original and two copies of a properly executed operational agreement if the sewer extension is permitted by a private applicant and will be serving residential or commercial lots (e.y., houses, condominiums, townhomes, outparcels, etc.) that will be sold to another entity. If the applicant is a homeowners' association, use Form HOA 02/03. If the applicant is a developer, use Form DEV 02/03. EVEN IF THE PROJECT WILL BE TURNED OVER TO A MUNICIPALITY, FORM DEV 02/03 IS REQUIRED. ® V. Flow Acceptance Letters — If the owner of the downstream sewers and/or WWTF is different from the applicant, submit two copies of a flow acceptance letter from the owner of the downstream sewers and WWTF, if different. Flow acceptance letters must contain the following minimum information: applicant and project name, amount of flow accepted, and name and permit number of the receiving sewers/WWTF. The flow acceptance must not expire prior to permit issuance and must be dated less than a year prior to the application date. Intergovernmental agreements or other contracts will not be accepted in lieu of a project -specific flow acceptance letter. ® VI. Map — Submit an 8.5-inch by 11-inch COLOR copy of the portion of a 7.5-minute USGS Topographic Map along with this form. The map should identify the entire project area location as well as the closest downslope surface waters as clearly as possible. Each map portion must be labeled with the map name and number, the identified location of the sewer line and pump stations, and be of clear and reproducible quality. For instance, if the project involves 4 miles of interceptor sewer that traverses over or near several different waterbodies (or counties, basins, etc.), the map should have location ID's shown for each different waterbody (where the sewer line is within 100 feet of such waterbody - see Instruction VII). This location ID is self chosen and used to cross reference the location in Section C of the permit application. ® VII. Stream Classifications — Section C • If any portion of the project is within 100 feet of any down slope surface water, Section C must be completed for the pertinent sections. • If the entire project area is a minimum of 100 feet away from any down slope surface waters BUT there is a pump station involved where a history of power outage is to be used to provide adequate design storage instead of dual feed or permanent or portable generator, Section C must be completed to demonstrate that the closest down slope surface water is Class C (see 15A NCAC 2H .0219(h)(3)(D)). • Use the guidance document entitled, "Determining Watershed Classifications for Form FTA 02/03 (Fast -Track Sewer Systems)" available from http://h2o.enr.state.nc.us/ndpu or by contacting the appropriate regional office. Applications can not be processed until a Finding of No Significant Impact (FONSI) or Environmental Impact Statement (EIS) has been issued. A copy is to be submitted with that permit application. IX. Certifications — Section D The application must be certified by both the applicant and a North Carolina Registered Professional Engineer (PE). The applicant signature must match the signing official listed in Item A(1 b). The PE should NOT certify the application if he/she is unfamiliar with 15A NCAC 2H .0200, the Gravity Sewer Minimum Design Criteria (mcst recent version) and the Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force Mains (most recent version), as applicable to the project. Certification by a PE who is unfamiliar with these documents is subject to NC Board referral. McNeill, Jr. Robert Conoly February 6, 2006 Re: Hoke Count 's Industrial Site & Sewer Allocation From City Dear Mr. Wood: The City of Raeford has adequate capacity at the Wastewater Treatment Facility to treat the wastewater from the Pate Road industrial site as long as the County does not exceed the .5 mgd limit including all other sources of wastewater from Hoke County. The wastewater shall also be in compliance with our pretreatment standards. Once an industry locates at the site, their process will have to be evaluated to determine the wastewater parameters for that particular industry's pretreatment system. Design Criteria Sewer system size determinations are normally determined by evaluating drainage basin acreage and development factors. The above described sewer system sizing is based on the estimated values contained in the Site Certification developed by The Wooten Company and Hoke County. However, the size of sewer system actually installed will be based on the water requirements of the industries that develop in the Park. The system as described in the Site Certification is based on a total of 1 million gallons per day (mgd) being pumped to the City of Raeford sewer system. B. These lines are called interceptor lines and are located to provide service along the low areas of the project. The depth of these lines will control the depth of the pumping stations. Other lines are located within road right- of-way, and will provide sewer service to industry that locates away from the interceptor lines. Using this approach sewer service should be available to any areas of the site that could be developed. Additional lines will have to be designed if smaller industries develop in the Park at locations not close to these designed lines. Also line sizes will have to be reevaluated based on the actual industries that locate in the Park. B. Manhole Design Manholes will be 4-foot diameter standard manholes, and will be located approximately 400 feet apart with a maximum separation of 425 feet. The manholes will have a standard fall across the invert of 0.08 feet, and invert in and invert out will be noted in the profile. Manholes where the deflection angle is more than 80 degrees will have a fall across the invert of 0.20 feet. Drop manholes with outside drops will be used where the difference in invert elevations are 2.5 feet or more. C. Crossings Culvert crossings will be limited as much as possible. Generally, the wastewater line will pass under the culvert with 12 inches clear. Stream crossings should be made at an angle close to 90 degrees to the centerline of the stream. The line will be designed to pass 1 foot below the bottom of the streams with 1 foot minimum cover, and ductile iron pipe (DIP) should be used at these crossings. At water line crossings, the wastewater line will pass under the water line with at least 18 inches clear between them. If the sewer line is less than 18 inches below the water line or the sewer line must be located above the water line, both the water line and the sewer line will be DIP. Where water lines are planned, the sewer lines will be designed to provide proper clearance. All road crossings will be installed using a bore and jacking method. To bore and jack a line first a steel casing pipe is installed in a bored hole. A carrier pipe (sewer line) is installed within the casing pipe. The steel casing should be sized 4 inches larger than the carrier pipe bell outside diameter (OD). The carrier pipe will be either DIP or Polyvinyl Chloride (PVC). Railroad Crossings will be made by the bore and jacking method as well. The following are the anticipated steel casing sizes for the listed carrier lines: Carrier line Steel Casing Size in. Size h 8" 16" 10" 18" 12" 20" Power line crossings will be made at 60 to 120 degrees from the right-of-way. Where the crossing is parallel to the railroad right-of-way, the angle of the crossing will be the same as exists. D. Limits of Ductile Iron Pipe (DIP) DIP will be used at the following locations: 1. Creek crossings (creeks defined as blue line on the USGS topographical map). 2. Where less than 3 feet of cover over pipe. 3. Where the line is located closer than 100 feet to a water supply well. 4. Where the sewer line is less that 18 inches below a water line or must cross above the water line. If after the DIP is laid out, it reaches to within 100 feet of the manhole, DIP shall be extended to the manhole. DIP will be shown in 20 feet lengths E. Services No service lines will be required for this project. Services will be installed as industries locate in the Park. F. Pumping Station Design Two pumping stations will be required to provide sewer service to the Industrial Park. These pumping stations were initially sized based on an assumed total flow in the Park of 1 mgd. If this were a residential area we could assume a flow based on a development density and multiply it times a set flow per house. Because this is an Industrial Park, the flows could vary greatly, and the only purpose for estimating the flow here is to provide a reasonably sized design for the system. Estimating the flows based on a typical residential design, the total flow from the Industrial Park would be as follows: 1,400 acres total - 210 acres (15%) not developable (wetlands, etc.) = 1,190 acres 1,190 acres x 1.5 houses per acre x 300 gpd/house = 535,500 gpd (average) Since flows will occur at certain times of the day and not consistently throughout the day, we multiply the average flow times a peaking factor. An average peaking factor is 2.5, which will give a total peak flow from the Industrial Park of 930 gpm or 1.3 mgd. PS 1 is the smaller station and serves only the portion of the Industrial park that fronts Golf Course Road (SR 1432), approximately 300 acres. Using the same flow design criteria as shown above, we come up with an average flow of 114,750 gpd or a peak pumping station design flow of 200 gpm. This will provide a velocity of 2.3 fps in a 6- inch force main. To comply with North Carolina DENR requirements the minimum velocity for a force main is 2 fps, so the force main for PS 2 will be 6 inches. PS 2 will collect wastewater from the whole site, including the flow from PS 1, and pump it to the City of Raeford wastewater system. A design flow of 930 gpm will flow at 2.6 fps in a 12-inch force main. However, the flow of 930 gpm is an assumed build out flow. The initial flows from the Park will be less and the velocity in the force main would drop below the minimum 2 fps. Because of the nature of this project, we recommend initially sizing the pumping station and force main for approximately half of the anticipated flow. The minimum flow for the 8-inch force main is a reasonable maximum flow is 475 gpm. Therefore, Pumping Station No. 2 will be initially designed for 475 gpm. Once that capacity is used up the pumps would be replaced and the force main would need to be paralleled. If the Industrial Park develops initially with a large wastewater producer, the station would need to be redesigned to support that industry.
