Dear Ms. Allman:
A letter of request for an Authorization to Construct was received May 30,
1989 , by the Division and final plans and specifications for the subject project.
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of improvements to the existing wastewater treatment
facilities, these improvements consisting of.. four (4) additional 1500 gallon septic_
tanks, a 4000 gallon dosing tank with dual. 1.30 GPM pumps and high water alarms and
the replacement of the existing subsurface sand filters with dual 4. 706 sq. ft.
surface sand filters .
This Authorization to Construct is issued in accordance with Part Ii.I paragraph
B of the NPDES Permit No. NCO055212 issued May 31., 1988, and shall be subject to
revocation unless the wastewater treatment fac.i.l.i.Lies are constructed in accordance
with the conditions and l.-im:Ltati.ons specified in Permit No. N00055212.
Upon 50% completion of the subject facilities the Permittee must employ a
certified wastewater operator in accordance with Part III. paragraph C of the
referenced permit .
The sludge generated from these treatment: facilities must be disposed of ill
accordance with G. S. 1.43-21.5 . 1 and in a manner npprovable by the North Carolina
Division of Environmental ManngemonL.
The Permi.Ltee must employ a certified wastaoater operator in accordance with
Part. JJI paragraph C of the referenced permit.
The Winston-Salem Regional Office, telephone number 919-761 -2351 shall he
notified at least twenty-four (24) hours in advance of operation of these
facilities . Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a .m. until. 5: 00 p.m. nn Monday through Friday,
excluding State Holidays,

In event the facilities fail to Perform saLi.sfncCori_l.y in meeting its NPDES
permit effluent .limits , the permitter shall take snch i.mmedi.ate. corrective action as
may he required by this Division, including the construction of additional
wastewater treatment and disposal. fnc_i.]it.ies.
The sand media of the sludge drying bed must comply with the Division' s sand
specifications. The engineer's certification will, he evidence that this
certification has been met.
Upon completion of construction and prior to operation of this permitted
facility. a certi.fi.cation ,must be received from n professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications .
Permits for sewer lines will require retention for five (5) years after
construction and all. other projects will require retention for the life of the
project.
