On November 24, 2009 the Division of Water Quality(DWQ)received your application sets dated
November 23,2009 and check#1170 for the above referenced project. The DWQ has determined that
your application was incomplete and/or provided inaccurate information as discussed below.
Additional Information Requested:
® Incorrect and/or missing wetland, intermittent-perennial stream and/or buffer impacts.
In reviewing page 5 of 5 for wetland and stream impacts, Section 2f shows 0.48 acres;
however section 2g shows 0.019 acres. Section 3g under stream impacts reflects 30 If of
impact but 3h has been left blank. Please supply one(1)corrected original and four(4)
corrected copies of page 7.
Please submit this information within 30 calendar days of the date of this letter. . If we do not receive this
requested information within 30 calendar days of the date of this letter,your project will be withdrawn and
you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,waters
or protected buffers. Please be aware that any impacts requested within your application are not
