On November 18, 2009, the Division of Water Quality (DWQ) received your application dated November
6, 2009 for the above referenced project. The DWQ has determined that your application was incomplete
and/or provided inaccurate information as discussed below. Please provide the following so that we may
continue to review your project.
Additional Information Requested:
1. Water Quality Certification GC3699 (NW 12) states, "In accordance with North Carolina General
Stature Section 143-215.3D(e), any requirement for written approval for a 401 Water Quality
Certification must include the appropriate fee." Please rernit $240 made payable to Division of Water
Quality (DWQ).
Please submit this information or fee within 30 calendar days of the date of this letter. If we do not receive
this requested information within 30 calendar days of the date of this letter, your project will be withdrawn
and you will need to reapply with a new application and a new fee.
