I. David T. Upchurch as a duly registered Professional Engineer in the
State of North Carolina, having been authorized to observe (periodically/weekly/full time) the
construction of the modifications and improvements to the Town of Magnolia WWTP, located
on West Faison Street, Magnolia in Duplin County for the Town of Magnolia, hereby state that,
to the best of my abilities, due care and diligence was used in the observation of the following
construction:
Wastewater Treatment Plant Upgrade-- Complete and in use
• demolition of existing 0.08 MGD wastewater treatment plant and existing effluent outfall
structure at Millers Creek, existing sludge drying beds and sand filters, and various yard
piping.
• microscreen rated at 0.626 MGD installed into the existing headworks structure.
• influent hump station consisting of three (3) 220 gpm pumps,
• dual extended aeration activated sludge treatment reactors, consisting of dual 22,900
gallon pre-reaction chambers, and dual 100,700 gallon aeration basins, with fine bubble
diffusers, and dual 397 scfm, 15 Hp air blowers.
• Dual 20-foot diameter, 120foot side water depth center-fed secondary clarifiers,
• dual mircroscreen drum filters,
• a UV disinfection unit consisting of two (2) channel banks in series,
• 900 gallon post aeration chamber using diffusers,
• an effluent pump station consisting of three (3) 220 gpm, 10 Ilp vertical turbine pumps,
and effluent flowmeter.
dual 50 gpm pumps for non-potable water in-plant use,
• one WAS/RAS pumping station with two 350gpm submersible pumps with two 4"
dia. telescoping valves,
• a 500 gpm, 8 Hp sludge transfer pump next to existing sludge digester,
• a 89,900 gallon sludge digester with fine bubble diffusers, with dual 224 scfm, 10 Hp air
blowers,
• a�sludge loading station (for trucks) with 500 gpm sludge transfer pump, and
• a 972 square-foot operations/lab building.
• Associated electrical work, including one 150 kW and one 40 kW emergency generator
with automatic transfer switch, and instrumentation.
Effluent Force Main and Outfall--Complete and in use
Construction of a 6.1 mile effluent force main and outfall at Stewarts Creek, to serve the Town
of Magnolia, to consist of.
• approximately 32,057 linear feet of 8-inch effluent force main, five (5) air release valves,
2 manholes, a concrete discharge outfall structure, and associated appurtenances,

and approved by the Department of Environment and Natural Resources.

Dear Linda:
We have recently completed modifications to the RAS/WAS system at the Magnolia
WWTP.'The new system is comprised of 2-350gpt-n pumps, 2- 4" telescoping valves,
mist valves and associated piping. The pump discharge is located in the influent splitter
box, which aiiows the flow to be directed to either side of the plant at the operators will.
The system has been started and is operating properly
At Gerald Horton's request we are forwarding a copy of the information that was sent to
CG&L that includes, Record Drawings of the modifications and an updated Engineers
CertificatiOn.
We anticipate that the Town of Magnolia will be requestinv, that thu N'10ratOriUrn be lifted

Dear Gerald:
Enclosed you will find a revised Engineer's Certification for the Magnolia WWTP.
The revision is submitted due to a change to the plant's RAS/WAS system.
• ', he original certificate certified two airlift RAS/WAS pumps.
• This modification certifies one RAS/WAS pump station with t.vo 350gpm
submersible pumps, two 4"dia. telescoping valves and associated valves and
piping.
The requirement for this change is due to a lack of adequate performance of the original
airlift pu;�-.p system.
The newly installed submersible pumps and equipment have been started and are
operating correctly and efficiently. Record Drawings are included with this submittal.

Dear Mayor Quinn:
A letter of request for Authorization to Construct was received April 26, 2004, by the
Division, and final plans and specifications for the subject project have been reviewed and found
to be satisfactory. Authorization is hereby granted for the construction of modifications to the
existing 0.09 MGD Town of Magnolia Wastewater Treatment Plant, with existing discharge of
treated wastewater into Millers Creek in the Cape Fear River Basin.
This authorization results in an increase in design and permitted capacity to 0.25 MGD, a
new 6.1 mile effluent force main and new discharge outfall at Stewarts Creek and abandonment
of the existing oiEtfall at Millers Creek, and is awarded for the construction of the following
specific modifications:
Wastewater Treatment Plant Upgrade
• demolition of existing 0.09 MGD wastewater treatment plant and existing effluent outfall
structure at Millers Creek, existing sludge drying beds and sand filters, and various yard
piping,
• microscreen rated at 0.626 MGD installed into the existing headworks structure,
• influent pump station consisting of three (3) 220 gpm pumps,
• dual extended aeration activated sludge treatment reactors, consisting of dual 22,900 gallon

Effluent Force Main and Outfall
Construction of a 6.1 mile effluent force main and outfall at Stewarts Creek, to serve the Town of
Magnolia, to consists of:
• approximately 32,057 linear feet of 8-inch effluent force main, five (5) air release valves, 2
manholes, a concrete discharge outfall structure, and associated appurtenances,
pursuant to the application received on April 26, 2004, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of
Environment and Natural Resources.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NI'DES Permit No. NC0020346 issued August 15, 2005, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0020346.
This Authorization To Construct is issued pursuant to T I 5A:2 H.02 I 9(p) for the
construction of a reclaimed water utilization facility without flow equalization at the site of the
Town of Magnolia WWTP. In the event that non-compliance with any condition or limitation
contained in Permit No. WQ0028752 results from the absence of flow equalization, the Town's
NPDES permit may be administratively reopened, and a requirement for the construction of a
flow equalization basin may be incorporated into the permit.
The sludge generated from these treatment facilities must be disposed of in accordance
with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Water Quality.
in the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

The Wilmington Regional Office, telephone number (910) 796-7215, shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the penmitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to Cecil G. Madden, Jr., P.E.,
Construction Grants & Loans, DWQ / DENR, 1633 Mail Service Center, Raleigh, NC 27699-
1633.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at
least equivalent to or greater than the classification assigned to the wastewater treatment
facilities by the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions oFTl5A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class 11, 111 and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions ofTI 5A:8G.0202.
A copy ol'the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Pcrmit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.
