Dear Mr. Walser:
A letter of request for Authorization to Construct: was rer..Pived Tune 9,
1.988, by the Division and final plans and specifications for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of additional wastewater treatment facilities
consisting of a. 4,000 gallon recirculating dosing tank with 2.-88.5 gpm pumps -
r.elocated from existing wet well, an existing septic tank modified into a grease
trap, a 40x50 recirculating sand filter, a 4-tube Sanuril. Model 1.000 chlorinator
with flow measuring, and all related valves, piping and appurtenances.
This Authorization to Construct is issued in accordance with Part III,
paragraph B of NPDES Permit No. NCO040045 issued Sept-ember 29, 1.988, and shall
be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified in
Permit No. NC0040045.
The Permi.ttee must employ a. certified wastewater operator in accordance
with Part III paragraph C of the referenced permit. ,
The sludge generated from these treatment facilities mast: be disposed of in
accordance with General Statute 143-215. 1 and in a manner- epp.r.ovable by the
North Carolina. Division of. Environmental Management.
The Winston Salem Regional Office, phone no. 919/761-2351. shall be
notified at least twenty-four (24) hours in advance of of the installed so
that an in-place inspection can be made. Such notification to the regional
supervisor shall be made during the normal. office hours :from 8:00 a.m. until.
5:00 p.m. on Monday through Friday, excluding State. llol:i_days.

In the event the facilities fail to perform satisfactorily in meeting its
NPDES permit effluent limits, the Permi.ttee shall take such immediate. corrective
action as may be required by this Division, including the r.0nsi:rllctiort of
additional wastewater treatment and disposal facilities.
The sand media. of the sludge drying bed must comply with t:lie Division's
sand specifications. The engineer's certification will. be evidence that this
certification has been met.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer
certifying that the permitted facility has been installed i.n Iccordance with the
approved plans end specifications. Mail. the certification to H-ie Permits and
Engineering Unit, P.O. Box 27687, Raleigh, N.C. 2761.1.-7687.
A copy of the approved plans and specifications shall he maintained on file
by the permittee for a minimum of five years from the date of issuance.
