Inspection Summary:
On February 26, 2020 WSRO staff made a site visit to the subject facility for review of the replacement monitoring well
installation (MW-3). Below is a summary of notes from the site visit.
• Proposed placement of MW-3 was discussed with Permittee, consultants & drillers, and subsequently approved by WSRO
staff.
• Monitoring wells MW-1 and MW-2 had previously been requested for abandonment. WSRO staff checked the water level in
both wells and found them to be dry. Both wells were subsequently approved for abandonment. Drillers started abandonment
procedures while onsite.
• The replacement well MW-3 was drilled to a depth of 65 ft., with no water being present at the time of the drilling.
• Reminded Permittee and representatives that a request for permit modification that includes: MW-3 construction records,
updated site map, abandonment records for MW-1 & MW-2 need to be submitted to CO.
• Reminded Permittee and representatives that MW-3 needs to be checked for water and an initial sample pulled if
applicable.
• On March 2, 2020 the Permittee notified WSRO that water is present in MW-3 and they will purge the well to flush drilling
materials prior to sampling. This sampling event will cover the initial sampling requirement as well as the required March
sample event.
