REQUEST FOR MORE INFORMATION-EXPRESS REVIEW PROGRAM
Dear Mr. Bayer:
On March 17, 2006, the Division of Water Quality (DWQ) received your application to impact
approximately 221 linear feet of streams at the above referenced site. The DWQ has determined that the
application will require additional information in order to review and provide an approval to impact these
resources. Failure to provide this information may result in denial of your application as required by 15A
NCAC 2H .0506.
Please provide the following information:
1. Your project is not part of a phased development. Please provide three (3) sets of a complete
stormwater management plan including construction plans and specifications, stormwater structure
worksheet(s), and supporting documentation calculations, and a signed and notarized operations
and maintenance agreement for the stormwater structures.
2. Please provide one original and two copies of an Operation & Maintenance agreement for all
structural stormwater BMPs on this project.
Please respond within three (3) days of the date of this letter. The Express Review Program is a process
that requires all parties to participate in a timely manner. This letter only addresses the application review
and does not authorize any impacts to wetlands, waters or protected buffers. Please contact Cynthia Van
