UTILITY NOTES
1. OCCUPATIONAL SAFETY & HEALTH ADMINISTRATION REGULATIONS (OSHA): CONTRACTOR SHALL ENSURE ALL
CONSTRUCTION AND EXCAVATION ACTIVITIES PROCEED ACCORDING TO THE LATEST OSHA GUIDELINES.
2. UTILITY LOCATIONS:
2.A. NOT ALL EXISTING UTILITIES ARE SHOWN ON THE PLAN. THE CONTRACTOR IS RESPONSIBLE FOR
LOCATION OF ALL EXISTING UNDERGROUND UTILITIES. CALL NORTH CAROLINA 811 (PHONE
1-800-632-4949) THREE WORKING DAYS PRIOR TO EXCAVATION.
2.B. COORDINATE WITH UTILITY OWNERS FOR LOCATION OF EXISTING UTILITIES AND WORK WITH THEM TO
REMOVE CONFLICTS.
2.C. VERIFY LOCATION AND ELEVATION OF EXISTING UTILITIES BEFORE CONSTRUCTION BEGINS AND
COORDINATE WITH THE ENGINEER FOR CHANGES TO THE SEWER LINE LOCATION AND, OR UTILITIES. IF
CHANGES ARE NEEDED, PLANS MUST BE APPROVED BY THE APPROPRIATE AGENCY.
2.D. LOCATION AND ELEVATION OF EXISTING UTILITIES AS SHOWN ON THE PLANS ARE APPROXIMATE.
3. ALL MATERIALS FURNISHED AND WORK PERFORMED SHALL BE IN CONFORMANCE WITH HOKE COUNTY PUBLIC
WORKS.
4. NOT ALL FITTINGS AND VALVES ARE SHOWN ON THE PLANS. CONTRACTOR SHALL INSTALL ALL FITTINGS AND
AND VALVES AS REQUIRED TO MEET HOKE COUNTY PUBLIC WORKS STANDARDS.
5. 42" MINIMUM COVER SHALL BE PROVIDED OVER ALL WATER MAINS UNLESS SHOWN OTHERWISE.
6. THE CONTRACTOR SHALL PROVIDE THRUST BLOCKING AT BENDS AND TEES PER HOKE COUNTY PUBLIC
WORKS STANDARDS. THE CONTRACTOR SHALL BE RESPONSIBLE FOR RELOCATION OF EXISTING UTILITIES IF
REQUIRED DURING INSTALLATION OF NEW WORK.

1. THE CONTRACTOR SHALL HAVE A COMPLETE SET OF CONTRACT DOCUMENTS AS WELL AS ALL APPROVALS ON THE JOB SITE AT ALL TIMES.
2. CONSTRUCTION AND MATERIAL SPECIFICATIONS SHALL CONFORM TO HOKE COUNTY PUBLIC WORKS AND NCDOT STANDARDS AND SPECIFICATIONS.
3. IT IS THE CONTRACTOR'S RESPONSIBILITY TO ACCURATELY LOCATE BOTH HORIZONTALLY AND VERTICALLY ALL EXISTING UTILITIES PRIOR TO THE START OF
CONSTRUCTION (NC ONE CALL CENTER 1-800-632-4949). COST TO REPAIR DAMAGED UTILITIES SHALL BE BORNE BY THE CONTRACTOR. ANY LOCATION OF
UNDERGROUND UTILITIES AS SHOWN ARE BASED ON VISIBLE EVIDENCE AND DRAWINGS IF PROVIDED TO THE SURVEYOR. LOCATION OF UNDERGROUND UTILITIES
AND STRUCTURES MAY VARY FROM LOCATIONS SHOWN HEREON, AND ADDITIONAL BURIED UTILITIES MAY EXIST. CONTACT THE APPROPRIATE UTILITY COMPANIES
FOR INFORMATION REGARDING BURIED UTILITIES.
4. CONTRACTOR SHALL BE RESPONSIBLE FOR THE RECONNECTION OF DISTURBED UTILITY SERVICES ENCOUNTERED WITHIN 8 HOURS OF INTERRUPTED SERVICE.
CONTRACTOR SHALL NOTIFY USERS 24 HOURS PRIOR TO POSSIBLE INTERRUPTIONS OF SERVICE.
5. EXCEPT AS SPECIFICALLY STATED OR SHOWN, THE BASE SURVEY DOES NOT REPORT ANY OF THE FOLLOWING: EASEMENTS, OTHER THAN THOSE VISIBLE DURING
FIELD EXAMINATION OR AS IN DOCUMENTATION PROVIDED BY THE CLIENT OR ATTORNEY, BUILDING SETBACKS, RESTRICTIVE COVENANTS, ZONING OR LAND USE

14. CONTRACTOR SHALL MAKE EVERY EFFORT TO SAVE PROPERTY IRONS, MONUMENTS, OTHER PERMANENT POINTS AND LINES OF REFERENCE AND CONSTRUCTION
STAKES. PROPERTY IRONS, MONUMENTS, AND OTHER PERMANENT POINTS OF REFERENCE DESTROYED BY THE CONTRACTOR SHALL BE REPLACED BY A
PROFESSIONAL LAND SURVEYOR AT THE CONTRACTOR'S EXPENSE.
15. CONTRACTOR SHALL INSTALL AND MAINTAIN MEASURES AS NECESSARY DURING CONSTRUCTION FOR EROSION AND SEDIMENTATION CONTROL AND SHALL AT ALL
TIMES ADHERE TO THE EROSION AND SEDIMENTATION LAWS AS PRESCRIBED BY THE NC DEPARTMENT OF ENVIRONMENT AND NATURAL RESOURCES.
16. ALL CONSTRUCTION SHALL BE IN ACCORDANCE WITH ALL APPLICABLE HOKE COUNTY, HOKE COUNTY PUBLIC WORKS AND NCDOT STANDARDS AND
SPECIFICATIONS.
17. OCCUPATIONAL SAFETY & HEALTH ADMINISTRATION REGULATIONS (OSHA): CONTRACTOR SHALL ENSURE ALL CONSTRUCTION ACTIVITIES PROCEED ACCORDING TO

18. AMERICAN WITH DISABILITIES ACT (ADA): CONTRACTOR SHALL ENSURE THAT ACCESSIBLE PARKING SPACES, ACCESS AISLES, RAMPS, AND ACCESSIBLE ROUTES TO
ALL ACCESSIBLE BUILDINGS (AS DETERMINED BY THE OWNER) MEET THE CURRENT MINIMUM REGULATIONS AS OUTLINED IN THE ADA STANDARDS FOR ACCESSIBLE
DESIGN. IF CONTRACTOR IS UNABLE TO MEET THE CURRENT MINIMUM REGULATIONS AS OUTLINED IN THE ADA STANDARDS FOR ACCESSIBLE DESIGN, FOR ANY
REASON, THE CONTRACTOR SHALL NOTIFY LARRY KING AND ASSOCIATES, RLS, PA, AND THE OWNER/DEVELOPER, IN WRITING, PRIOR TO COMMENCING ANY

19. IT IS RECOMMENDED THE OWNER/DEVELOPER CONSULT WITH A GEOTECHNICAL ENGINEER OR OTHER PROFESSIONAL FOR ALL BUILDING FOOTINGS/FOUNDATIONS.
