On February 24, 2017 a site inspection was conducted for the Mathews Pit #4 facility located at 5121
Devils Racetrack Road, Johnston Co. County, North Carolina. A copy of the Compliance Inspection
Report is enclosed for your review. The plant manager, Tony Gandy, was also present during the
inspection and his time and assistance is greatly appreciated. The site visit and file review revealed that
the subject facility is covered by NPIDES Stormwater General Perm it-NCG020875, Certificate of
Coverage-NCG020000. Permit coverage authorizes the discharge of stormwater from the facility to
receiving waters designated as Hannah Creek, a Class C waterbody in the Neuse River Basin.
As a result of the inspection, the facility was found to be out of compliance with the conditions of the
NCG020875 permit. Please refer to the enclosed Compliance Inspection Report for additional comments
and observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff has any

and Land Resources, conducted a site inspection for the Matthews Pit #4 facility located at 5121 Devils
Racetrack Road, Johnston County, North Carolina. A copy of the Compliance Inspection Report is
enclosed for your review. Tony Gandy was also present during the inspection and his time and assistance
is greatly appreciated. Permit coverage authorizes the discharge of stormwater from the facility to
receiving waters designated as Hannah Creek, a Class C waterbody in the Neuse River Basin. The site
visit and file review revealed that the subject facility is covered by NPIDES Stormwater General Permit-
NCG020000, Certificate of Coverage-NCG020875.
As a result of the site inspection, the following permit conditions violation(s) are noted
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has not been developed, recorded, or properly
implemented.
2) Qualitative Monitoring
Qualitative monitoring has not been conducted and recorded in accordance with permit requirements.
3) Analytical Monitoring
Analytical monitoring has not been conducted and recorded in accordance with permit requirements.
Other Observations:
Sufficient mechanical measures to insure polluted runoff doesn't leave the site where not installed and
maintained and the site was not inspected for problems after rain events.
Requested Response:
You are asked to respond to this office, in writing, within 30 calendar days from receipt of this notice.
Your written response should include a reasonable explanation as to why the aforementioned violations
have occurred as well as a Plan of Action to prevent these violations from recurring.
