Inspection Summary:
Meet with Pat Shillington the engineer that developed the storm water plan and Zack Edwards the mine manager. I went
through all the conditions required by the permit and found that most of the conditions have not been complied with.The
Facility lacked narative description of practices, a BMP summary, a spill prevention and response plan,a preventive
maintenance and good house keeping plan, documented employee training,a list of responsible parties,a storm water
facility inspection program and copies of the permit and certificate of coverage] will recommend that we issue a Notice of
Difficiency to insure that the operator is fully aware of the compliance issues
