1.2. RELEASE INFORMATION
Date Discovered: Elevated chloride concentrations were first discovered and identified in
groundwater beneath the Valhalla Water Treatment Plant (also referred to as the "site," Figure
1) in 2014 by the North Carolina Department of Environmental Quality (NCDEQ), Division of
Water Resources (DWR) when one groundwater sample was collected from a temporary
monitoring well installed by DWR staff to confirm chloride concentrations above the 15A NCAC
2L Groundwater Standards. On November 18, 2014, DWR issued a Notice of Regulatory
Requirements (NORR) directing assessment of the groundwater quality at the compliance
boundary for the water treatment plant (WTP). Analytical results from monitoring wells MW1
though MW3 installed by Chowan County on May 11, 2015 led DWR to issue a Notice of
Violation (NOV) on October 6, 2015 for the Valhalla WTP. The NOV required a site assessment
and preparation of a Corrective Action Plan.
On November 11, 2015, GMA collected groundwater samples from the monitoring -well network.
Analytical results were consistent with previous data from the May 26, 2015, and July 10, 2015
monitoring events. Well MW1 contained the highest concentrations of chloride, iron, and
manganese, and all three concentrations exceeded the 2L groundwater standards.
Concentrations of iron and chloride exceeded the 2L groundwater standards in all three
monitoring well samples. These results were submitted in an Initial Assessment Report (IAS)
dated December 18, 2015 (GMA, 2015) to DWR. Upon review of the ISA prepared by GMA,
DWR issued an ISA Report Review dated January 11, 2016, and directed completion of a
Comprehensive Site Assessment (CSA).

Cause of Release: The release appears to have been caused by groundwater infiltration from
the unlined former borrow pit that is the discharge point for Valhalla WTP effluent.
Source of Release: The source of the release appears to be effluent from the water treatment
processes (including removal of hardness, iron, and manganese from groundwater).
Estimated Quantity of Release: The quantity of the release is unknown.

2.1 WATER -SUPPLY WELLS
GMA completed a survey within a radius of 2000 feet around the site to identify property owners
and water -supply wells. GMA conducted interviews, searched for water meters and well houses
on foot and by vehicle, reviewed the locations of Chowan County's production wells, and
gathered information from the County. All developed properties are connected to public water.
Six water --supply wells were identified within 2000 feet of the source area (Figure 2). Two
municipal production wells owned and operated by Chowan County (WSW1 and WSW2, Table 1)
(Figure 2) are located 335 feet northeast and 1600 feet northeast, respectively, of the source
area. Well WSW1 is currently active, 319 feet deep, and is screened from 304 feet to 314 below
land surface (BLS). Well WSW2 is currently offline, is reportedly 124 feet deep, and is screened
from 84 feet to 124 feet BLS. Two inactive non -potable wells are located 600 feet northwest-__
(WSW4) and 1200 feet southwest (WSW6) of the source area, respectively. One inactive non -
potable well, reportedly 12 feet deep (WSW3), was used to dewater during installation of an in-

ground pool located 1200 feet to the northeast, and one active irrigation well (WSW5) located-
650 feet southwest of the source area is reportedly 40 feet deep and screened from 14.to 40
feet BLS (Figure 2, Table 1).
2.2 PUBLIC WATER SUPPLIES
Potable water is provided throughout the survey area by Chowan County.

2.3 SURFACE WATER
Multiple ponds are located near the source of the release (additional former borrow pit ponds,
Figure 2). A pond owned by Mr. Rodney Harrell is located approximately 550 feet west of the
source area. A pond owned by Mr. C.T. Dixon is located approximately 800 feet west of the
source area. A tributary of Pollock Swamp is located approximately 1300 feet east of the source
area (Figures 1 and 2). Bennett Millpond is located approximately 1450 feet west of the source
area (Figures 1 and 2).
2.4 WELLHEAD PROTECTION AREAS
There are no designated wellhead protection areas in Chowan County.
2.5 DEEP AQUIFERS IN THE COASTAL PLAIN PHYSIOGRAPHIC REGION
The site (Figure 1) is located in the Coastal Plain Physiographic Province (NCGS, 1985), and is
underlain by Tertiary -age Yorktown, Pungo River, Castle Hayne, and Beaufort Formations.
These Tertiary -age formations are underlain by the Cretaceous -age Black Creek and Cape Fear
Formations. The primary aquifers underlying the source area are the shallow post -Miocene
Yorktown/Surficial aquifer, the middle -Miocene Pungo River aquifer, the Oligocene to Eocene
Castle Hayne aquifer, and the Paleocene Beaufort aquifer. These are underlain by the
Cretaceous Aquifer System (CAS), which is comprised of the Black Creek, Upper Cape Fear, and
Lower Cape Fear aquifers (Winner and Coble, 1989).
2.6 SUBSURFACE STRUCTURES
There are subsurface raw water, potable water, electric, telephone, fiber optic, and cable
television lines throughout the survey area. Basements were not identified at the site or in the
surrounding area.
2.7 LAND USE
Properties within the surrounding area are primarily used for residential and agricultural
purposes or are undeveloped (Figure 2). A few small businesses are located along Virginia Road
(NC Highway 32). The current property uses appear unlikely to change in the near future. The
closest place of public assembly is the Faith Pentecostal Holiness Church located 300 feet north
of the source area (Figure 2, Table 1). The residence closest to the source area is located 325
feet west and is owned by Robert and Janie Harrell (Figure 2, Table 1).
2.8 PROPERTY OWNERS AND OCCUPANTS
Property owners located within the 2000 foot survey radius (Figure 2) are identified in Table 1.
Property access agreements were negotiated and obtained from multiple property owners
concerning ongoing groundwater and surface water investigations of the site.
3.0 SITE GEOLOGY AND HYDROGEOLOGY
The site (Figure 1) is located in the Coastal Plain Physiographic Province according to the
Geologic Map of North Carolina (NCGS, 1985). Shallow stratigraphy was determined during the

installation of three, 12-foot deep groundwater monitoring wells (MW1 through MW3) and recent
field activities including Geoprobe° borings. Magette Well and Pump Company, Inc. installed the
monitoring wells for compliance boundary monitoring purposes, as requested by NCDEQ, DWR.
Table 2 includes detailed well construction information. The generalized stratigraphy
encountered is a brown to tan fine sand from the land surface to 14 feet BLS, an orange to
mottled tan medium to fine sand from 14 to 16 feet BLS, and a dark grey very fine sandy silt
interbedded with clay lenses, some shell fragments, and coarse sand lenses from 16 to at least
32 feet BLS. The total thickness of the surficial aquifer is not presently known, but it is_
anticipated to be no more than 50 feet.
On November 11, 2015, GMA measured the average depth to groundwater in the monitoring
wells at approximately 5.1 feet BLS (Table 2). The resulting equipotential map for November 11,
2015, indicated that groundwater was flowing predominantly to the northwest along a hydraulic
gradient of 0.003 feet per foot (GMA, 2015). This gradient appears to generally correspond to
the topography of the area surrounding the monitoring wells. Groundwater in the_ urficial
aquifer generally migrates westward toward Bennett Millpond (Figure 1), which is approximately
24 feet lower in elevation than the site. A Steeper hydraulic gradient probably exists west of
Virginia Road.
4.0 SAMPLING RESULTS
4.1 HISTORICAL GROUNDWATER SAMPLING
On November 12, 2015, groundwater samples were collected from the monitoring wells MW1
though MW3 and submitted to Prism Laboratories, Inc. (Prism) for analysis of chloride, total
manganese, and total iron. Analytical results (Table 3) indicated that chloride andiron
concentrations in all samples exceeded the 15A NCAC 2L groundwater standards (2L Standards).
The manganese concentrations in the monitoring well MW1 sample also exceeded the 2L
Standard (GMA, 2015)
4.2 SAMPLING METHODS
On May 25-26, 2016, 15 temporary Geoprobe° groundwater borings were advanced (TW1
through TW15) to depths ranging from 20 to 32 feet below land surface (BLS). Multiple
groundwater samples were collected at different depths from each boring (Figure 4, Table 4).
The borings were located on multiple parcels surrounding the Valhalla WTP (Figures 2 and 4),
and property access agreements were obtained from area property owners prior to performing
the field work.
A four -foot, 0.006-inch, continuous -slot screen was advanced to two or three different sampling
intervals (ranging from 3-7 feet to 28-32 feet BLS) at each Geoprobe° boring location. Table 4
shows the total boring depths and the screen interval for the sample collection. The data
reported for each boring location is for the screen interval with the highest chloride
concentration. A peristaltic pump was used to purge the temporary wells and to collect the

samples. The Geoprobe° rods and screen were decontaminated after each sample and each
boring, and fresh vinyl pump tubing was used to collect each sample.
Multiple surface water (SW1 through SW5, and SW7) and water -supply well samples (WSW4 and
WSW5) were collected on May 25-26, 2016. Grab samples were collected from either the surface
water body or discharge water from the well.
Water quality analysis was performed in the field on all surface water and groundwater samples.
GMA's field water -quality analyses included temperature, dissolved oxygen (DO), specific
conductivity, conductivity, pH, oxidation-reduction potential (ORP), and chloride. The field
measurements of DO, ORP, pH, specific conductivity, conductivity, and temperature were
monitored using an YSI Professional Plus meter. GMA performed field chloride analyses using
Hach QuanTab° titration test strips.
Select samples were submitted to Prism, for analysis of chloride, total manganese, dissolved
manganese, total iron, and dissolved iron. A description of GMA's technical methods and
standard procedures used are summarized in Appendix II. A copy of the laboratory report and
chain -of -custody forms for the surface and groundwater samples is included in Appendix III.
4.3 CURRENT GROUNDWATER RESULTS
Eleven of the 15 boring locations contained detectable concentrations of chloride by field and
laboratory analyses. Nine of the 15 samples shown in Table 3 are reported above the chloride 2L
Standard of 250 milligrams per liter (mg/Q. Samples from TW5 and TW12 contained detectable
iron and manganese.
The greatest concentrations of chloride (TW3, TW4, and TW5) are located to the west of the
WTP and generally toward Bennett Millpond, toward the southwest (TW8 and WSW5) and
perhaps toward well WSW5 due to active irrigation use, and north (TW10) of the WTP. Total
iron, total manganese, dissolved iron, and dissolved manganese were all detected above the 2L
standards for samples TW5 and TW12.
4.4 CURRENT SURFACE WATER RESULTS
Six surface water samples were collected on May 25-26, 2016, from surface water features
located near the WTP (SW1 though SW5 and SW7; no SW6 was collected at this time). The
former borrow ponds are located adjacent to the source of the release (SW7). A pond owned by
Mr. Rodney Harrell is located approximately 550 feet west of the source area (SW3). An
intermittent tributary of Bennett Millpond is located approximately 750 feet west and southwest
of the source area (SW2 and SW4). A pond owned by Mr. C.T. Dixon is located approximately
800 feet west of the source area (SW5). A tributary of Pollock Swamp is located approximately
1300 feet east of the source area (SW1). Bennett Millpond is located approximately 1450 feet
west of the source area (Figures 1 through 4).

Samples SW1, SW2, SW4, SW5, and SW7 were collected as grab samples from the body of
water. Sample SW3 was collected from the underground french drain pipe that discharges into
the pond. All samples were submitted to Prism for analysis of chloride, total manganese,
dissolved manganese, total iron, and dissolved iron. The chloride concentrations detected in
samples SW3, SW4, SW5, and SW7 exceeded the 2L Standard (Table 3). Total iron and total
manganese concentrations detected in samples SW1 through SW5 were reported above the
respective 2L Standards. The dissolved manganese concentrations were also above the 2L
Standards in samples SW3 and SW4 (Table 3).
Technical methods and standard procedures utilized for sample collection are described in
Appendix II. A copy of the laboratory report and chain -of -custody forms is included in Appendix
III.
4.5 WATER -SUPPLY WELL SAMPLING
Two water -supply wells were sampled on May 25-26, 2016 (WSW4 and WSW5). Samples were
collected and submitted to Prism for analysis of chloride, total manganese, dissolved manganese,
total iron, and dissolved iron. Both samples contained detectable concentrations of chloride, total
iron, total manganese, and dissolved manganese (Table 3). The sample collected from well
WSW4 contained concentrations of total iron, total manganese, and dissolved manganese above
the 2L Standards. The sample collected from well WSW5 contained concentrations of chloride,
total manganese, and dissolved manganese above the 2L Standards.
Active use of well WSW5 for yard irrigation is likely increasing chloride migration toward the well
and the surrounding area. The use of the well should be limited or stopped to prevent additional
movement of the chloride plume in the direction of well WSW5 and to prevent accumulation of
chloride in the soils at this property over time. Technical methods and standard procedures
utilized for sample collection are described in Appendix II. A copy of the laboratory report and
chain -of -custody forms is included in Appendix III.
4.6 FIELD MEASURED WATER QUALITY PARAMETERS
On May 25-26, 2016, all surface water and groundwater samples were tested in the field for DO,
specific conductivity, conductivity, temperature, pH, ORP, and chloride to evaluate the surficial
aquifer conditions (Table 4). The high specific conductivity and conductivity measurements are
consistent with elevated chloride concentrations in the monitoring wells.
5.0 CONCLUSIONS
Based on the ISA Report, additional delineation of the chloride plume was required by DWR
(Appendix I). GMA obtained property access from multiple parcel owners for ongoing surface
water and groundwater investigations. On May 25-26, 2016, GMA completed field investigations
that included surface water, water -supply well, and groundwater sampling and testing.
Groundwater samples were collected from temporary Geoprobe° groundwater borings at varying
depths. GMA utilized field water quality analysis to aid in assessment horizontally and vertically

of the chloride plume and laboratory data for confirmation of select data. This investigation is
the subject of this Phase II Site Assessment Report.
The predominant movement of the chloride plume appears to be toward the west and Bennett
Millpond, and a tributary of Bennett Millpond, and toward the southwest and active irrigation
well WSW5. Additional groundwater data are needed for areas north of sample location TW10,
and additional surface water data is recommended for Bennett Millpond northwest of sample
location SW5. Existing data for iron and manganese are inadequate to determine the extent of
those metals, but additional sampling will be included as part of the Comprehensive Site
Assessment recommended below.
6.0 RECOMMENDATIONS AND CSA WORK PLAN
GMA recommends completion of a Comprehensive Site Assessment for the Valhalla Water
Treatment Plant which will include the following tasks:
• Installation of 13 permanent groundwater monitoring wells for horizontal and vertical
delineation of the chloride, iron, and manganese plumes. Ten wells are proposed to
depths up to 50 feet BLS, and three deeper wells are proposed to depths of 90 feet BLS
(Figure 8).
• Coordination with property owners to select locations for permanent monitoring wells.
• Obtain necessary well construction permits for installation of permanent monitoring wells.
• Monitoring well data from an upgradient, offsite location will be used to establish natural
background concentrations for chloride, iron, and manganese (proposed wells MW13 and
MW13D).
• Conduct a comprehensive groundwater monitoring event that includes all monitoring
wells, area water -supply wells, and surface water samples, including the nearest point of
Bennett Millpond.
• Conduct aquifer testing using select monitoring wells.
• Complete a Comprehensive Site Assessment (CSA) Report and required Public
Notification of the CSA.

Dear Mr. Howard:
On December 22, 2015, the Washington Regional Office received a report for the above
referenced facility that presented the results of the Initial Site Assessment performed in response
to the Notice Of Violation (NOV) issued by the Division of Water Resources — Water Quality
Regional Operations Section on October 6, 2015, for exceedance of the North Carolina
Groundwater Quality Standards (15A NCAC 2L). Based on our review, this office concurs with
the conclusions and recommendations in the report and requests that Chowan County proceed with
the performance of a Comprehensive Site Assessment (CSA), as required by 15A NCAC 2L
.0106(g). The CSA should include, but is not limited to the following:
The source and cause of contamination;
2. Any imminent hazards to public health and safety and actions taken to mitigate them
in accordance with 15A NCAC 2L .0106(fl;
All receptors and significant exposure pathways;
4. The horizontal and vertical extent of soil and groundwater contamination and all
significant factors affecting contaminant transport; and
5. Geological and hydrogeological features influencing the movement, chemical, and
physical character of the contaminants.

The CSA must be conducted under the supervision of a Professional Engineer (P.E.) or
Licensed Geologist (L.G.). The subsequent report must be signed and sealed by a P.E. or L.G.
For information on available consultants and engineers, you may contact the Consulting Engineers
Council of North Carolina at (919) 781-7934, or the North Carolina Board for Licensing of
Geologist at (919) 850-9669, or the North Carolina Petroleum Marketers Association at (919) 782-
4411.
The items listed above must be addressed in a CSA report within ninety (90) days of

TECHNICAL METHODS AND STANDARD PROCEDURES
I. GEOPROBE® GROUNDWATER SAMPLING
A pick-up truck mounted Geoprobe°, decontaminated 4-foot Geoprobe° rods, and a
decontaminated 4-foot, 0.006-inch continuous -slot screen was used to collect the Geoprobe°
groundwater samples (TW1 through TW15). The screen segment was advanced to two or
three select sampling intervals at each boring. A peristaltic pump was used to purge the
temporary wells and to collect the samples. The Geoprobe° rods and screen were
decontaminated after each sample and boring, and fresh vinyl tubing was used for the purging
and sampling of each temporary well. Fresh, disposable vinyl gloves were worn during the
installation, purging, and sampling of each temporary well. Groundwater collected for field
analysis was collected in decontaminated stainless steel beakers. All laboratory sample
containers were stored in plastic Ziploc® bags and were immediately placed in a cooler with ice
following sample collection. Select groundwater samples collected from the temporary
Geoprobe° wells were submitted to a North Carolina certified laboratory, for analysis of
chloride, total iron, dissolved iron, total manganese, and dissolved manganese. Samples were
delivered to the North Carolina certified laboratory, Prism Laboratories, Inc., for analysis under
chain -of -custody protocol by the lab's courier service. All borings were abandoned using
bentonite clay and capped with native soil at land surface.
II. COLLECTION OF SURFACE WATER SAMPLES
Surface water samples were collected using a stainless -steel beaker and new disposable nitrite
gloves. The stainless -steel beaker was decontaminated between each sample location. Prior to
sample collection the stainless -steel pitcher was rinsed with surface water by submerging it into
standing water within the surface water feature, then removing it from the water and dumping
its contents on the bank of the surface water feature. The stainless -steel pitcher was then used
to collect standing water within the surface water feature and transferred to sample containers
provided by the North Carolina certified laboratory. All sample containers were stored in plastic
Ziploc® bags and were immediately placed in a cooler with ice. Samples were delivered by a
courier to a North Carolina certified laboratory, Prism Laboratories, Inc., for analysis under
chain -of -custody protocol.
III. COLLECTION OF WATER -SUPPLY WELL SAMPLES
The water -supply well was purged for a minimum of five minutes prior to sample collection. Any
hose was disconnected prior to sample collection in the laboratory supplied container. New
disposable nitrile gloves were worn during the purging and sampling of each well.
For wells without an active pump, either a peristaltic pump with new disposable polyethylene
tubing, or a decontaminated purge pump with new disposable polyethylene tubing was used to
purge the well. New disposable nitrile gloves were worn during the purging and sampling of
each well.
Groundwater samples were transferred to laboratory supplied containers for analysis. Standard
procedure for chloride, manganese, and total iron analysis is to collect one groundwater sample
for all three analysis from the well. All sample containers were stored in plastic Ziploc® bags
and were immediately placed in a cooler with ice. Samples were submitted to a North Carolina
certified Laboratory for analysis under chain -of -custody protocol, by courier service.
