NCDEQ collected groundwater samples and analyses confirmed chloride concentrations above the 15A NCAC 2L Groundwater Standards (2LGWOS). On November 18, 2014, DWR issued a Notice of Regulatory Requirements (NORR) directing assessment of the groundwater quality at and beyond the compliance boundary assigned as part of the National Pollutant Discharge Elimination System (NPDES) permit for the water treatment plant (WTP). Analytical results from monitoring wells MW1 though MW3 installed by Chowan County on May 11, 2015, led DWR to issue a Notice of Violation (NOV) on October 6, 2015, for the Valhalla WTP. The NOV required a site assessment and preparation of a Corrective Action Plan. Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 2 of 1 I Cause of Release: The release appears to have been caused by groundwater infiltration from the unlined former borrow pit that is the discharge point for Valhalla WTP effluent. Source of Release: The source of the release appears to be effluent from the water treatment processes (including removal of hardness, iron, and manganese from groundwater). Estimated Quantity of Release: The quantity of the release is unknown. 1.3 REPORT CERTIFICATION I, Katie T. Hackney, a Licensed Geologist for Groundwater Management Associates, Inc. The WTP treats raw water to reduce levels of iron, manganese, and hardness in order to meet state and federal drinking water standards; thereby generating waste water that contains increased levels of iron, manganese and chloride. The WTP discharges the waste water under National Pollutant Discharge Elimination System (NPDES) permit number NC0032719. The current discharge location is to the former borrow pond (unlined sand quarry) located on the WTP property (Figure 4). The actual discharge point required in the permit has ranged over the years from the former ditch located along Highway 32, Rocky Hock Creek, and an unnamed tributary of Pollock Swamp. 2.2 PURPOSE AND SCOPE The purpose of this report is to provide current groundwater and surface water quality data, and a general site status for the Valhalla Water Treatment Plant. GMA sampled 18 monitoring wells, two water supply wells, and nine surface water locations. Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 3 of 1 1 2.3 SOURCE CHARACTERIZATION The source of elevated chloride, iron, and manganese concentrations in groundwater immediately below and downgradient of the WTP appears to be from the current and historical discharge of WTP effluent to the unlined former borrow pond on the WTP property. 2.4 ACTIONS PERFORMED TO DATE Chowan County has investigated multiple options over the years for an alternate discharge point, but no permanent agreement has been made with NCDEQ. Currently, the County and NCDEQ have entered into a Special Order of Consent (SOC) signed September 6, 2016 by Chowan County to work towards alternate temporary and permanent solutions for discharge of the WTP effluent. 3.0 SENSITIVE RECEPTORS AND MIGRATION PATHWAYS 3.1 WATER -SUPPLY WELLS Water -supply wells located within 2,000 feet of the contamination source area are shown in Figure 3. GMA completed a survey within a radius of 2,000 feet around the site to identify property owners and water -supply wells. GMA conducted interviews, searched for water meters and well houses, reviewed the locations of Chowan County's production wells, and gathered information from the County. All developed properties are connected to public water. GMA identified six water -supply wells within 2,000 feet of the source area (Figure 3). Two municipal production wells owned and operated by Chowan County, (WSW1 and WSW2; Figure 3, Table 1) are located 335 feet northeast and 1600 feet northeast, respectively, of the source area. Well WSW1 is currently active, 319 feet deep, and is screened from 304 feet to 314 below land surface (BLS) in the Beaufort Aquifer. Although the cased portion of well WSW1 is located within the contaminant plume (Figures 10 and 13), the well is screened below the plume and is currently not impacted. Well WSW2 is currently offline, is reportedly 124 feet deep, and is screened in the Yorktown Aquifer from 84 feet to 124 feet BLS. Two inactive private non -potable wells are located 600 feet northwest (WSW4) and 1,200 feet southwest (WSW6) of the source area, respectively. One inactive non -potable well, reportedly 12 feet deep (WSW3), was used to dewater during installation of an in -ground pool located 1,200 feet to the northeast, and one now inactive irrigation well (WSW5) located 650 feet southwest of the source area is 30 feet deep and reportedly is screened from 14 to 30 feet BLS (Figure 3, Table 1). 3.2 SURFACE WATER Multiple ponds are located near the source of the release (additional former sand quarries, Figures 3 and 4) on the subject parcel. A pond owned by Mr. Rodney Harrell is located Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 4 of I I approximately 550 feet west of the source area. A pond owned by Mr. C.T. Dixon is located approximately 800 feet west of the source area. A tributary of Pollock Swamp is located approximately 1300 feet east of the source area (Figures 1 and 3). Bennett Millpond is located approximately 1300 feet west of the source area (Figures 1 and 3). 3.3 WELLHEAD PROTECTION AREAS There are no designated wellhead protection areas in Chowan County. 3.4 DEEP AQUIFERS IN THE COASTAL PLAIN PHYSIOGRAPHIC REGION The site (Figure 1) is located in the Coastal Plain Physiographic Province (NCGS, 1985), and is underlain by Tertiary -age Yorktown, Pungo River, Castle Hayne, and Beaufort Formations. Cretaceous -age Black Creek and Cape Fear Formations underlie these Tertiary -age formations. The primary aquifers underlying the source area are the shallow post -Miocene Surficial and Yorktown Aquifers, the middle -Miocene Pungo River Aquifer, the Oligocene to Eocene Castle Hayne Aquifer, and the Paleocene Beaufort Aquifer, that are underlain by the Cretaceous Aquifer System (CAS). The CAS is comprised of the Black Creek, Upper Cape Fear, and Lower Cape Fear Aquifers (Winner and Coble, 1989). 3.5 SUBSURFACE STRUCTURES There are subsurface raw water, potable water, electric, telephone, fiber optic, and cable television lines throughout the survey area. GMA did not locate any basements at the site or in the surrounding area. 3.6 LAND USE Properties within the surrounding area are primarily used for residential and agricultural purposes or are undeveloped (Figure 3). A few small businesses are located along Virginia Road (NC Highway 32). The current property uses appear unlikely to change in the near future. The closest place of public assembly is the Faith Pentecostal Holiness Church located 300 feet north of the source area (Figure 3, Table 1). The residence closest to the source area is located 325 feet west and is owned by Robert and Janie Harrell (Figure 3, Table 1). 3.7 PROPERTY OWNERS AND OCCUPANTS Property owners located within the 2,000 foot survey radius (Figure 3) are identified in Table 1. GMA negotiated and obtained property access agreements from multiple property owners concerning ongoing groundwater and surface water investigations of the site. 4.0 SITE GEOLOGY AND HYDROGEOLOGY 4.1 SITE STRATIGRAPHY Shallow stratigraphy was determined during the installation of three, 12-foot deep groundwater monitoring wells (MW1 through MW3) and field activities including Geoprobe° borings, and Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 5 of 1 1 installation of multiple monitoring wells ranging from 47 to 221 fee BLS (MW4 — MW17; Figure 5). Table 2 includes detailed well construction information. The generalized stratigraphy encountered is a brown to tan fine sand from the land surface to 14 feet BLS, an orange to mottled tan medium to fine sand with some coarse sand and/or pebbles from 14 to 16 feet BLS, a dark grey very fine sandy silt interbedded with clay lenses, and coarse sand lenses from 16 to 25 feet BLS, a dark grey silty sand from 25 to 44 feet BLS, dark grey fine silty sand with shell beds, some interbedded wood debris, and gravel beds from 44 to 155 feet BLS, a grey clayey very fine sandy silt from 155 to 180 feet BLS, a grey sandy clayey silt with lots of medium to large shells from 180 to 193 feet BLS, a greenish grey clayey silty with phosphate bits from 193 to 197 feet BLS, a greenish grey clay hard clay lens at the top then back to a phosphate and shelly, silty sand from 197 to 214 feet BLS, and a hard phosphate/sandstone cap with quartz and phosphate sand/gravel that transitions with depth back to a silty, quartz and phosphate sand interbedded with shell fragments from 214 to at least 221 feet BLS. The total thickness of the Yorktown Aquifer is approximately 180 feet. 4.2 SITE GEOLOGY The site (Figures 1 and 2) occurs in a low -relief portion of the Atlantic Coastal Plain Physiographic Province at an elevation of approximately 40 feet above mean sea level (AMS). Surface to near surface sediments beneath the site are Quaternary in age, and deeper sediments are Tertiary to Cretaceous in age and consist of sand, clayey sand, and glauconitic, locally-fossiliferous and calcareous clay (North Carolina Geological Survey, 1985). Patches of sandy molluscan —mold limestone occur locally. 4.3 SITE HYDROGEOLOGY Due to the topography of the source area and its proximity to Bennett Millpond (approximately 1,450 feet west), groundwater in the Surficial Aquifer migrates westward and toward Bennett Millpond (Figure 1). Regular effluent discharge to the unlined ponds at the WTP is likely causing mounding of water table and is affecting the direction of groundwater flow within the Surficial Aquifer. The site is located along a sand ridge known as the Suffolk Scarp (NCGS, 1985) therefore groundwater in the Surficial Aquifer is affected locally and likely flows roughly to the east and west with the Scarp divide (Figures 1 and 2). Bennett Millpond flows into Rocky Hock Creek about 3000 feet south of the site and ultimately discharges to the Chowan River approximately 2.3 miles downstream of Bennett Millpond. 5.0 SOIL CONTAMINATION INVESTIGATION The WTP treats raw water to reduce levels of iron, manganese, and hardness, therefore, the wastewater generated contains increased levels of iron, manganese, and chloride. The WTP discharges the waste water directly into the former borrow pond at the site where it infiltrates the groundwater system. Depth to groundwater near the borrow pond has been shown to be approximately six feet BLS (Table 2). Soil contamination has not been identified or suspected Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 6 of 1 I because it appears that the borrow pond intersects the surficial water table. No impacts to unsaturated soils are expected. Sediments/sludge have accumulated in the former sand quarry over time and will need to be collected and disposed of once discharge of the WTP effluent is discontinued. 6.0 GROUNDWATER CONTAMINATION INVESTIGATION 6.1 MONITORING WELL NETWORK The current monitoring well network (Figure 5) consists of four surficial aquifer (MW1-MW3, and MW17), 11 shallow Yorktown aquifer (MW4 — MW14), and five deep Yorktown aquifer (MW2D, MW9D, MW13D, MW15, and MW16) monitoring wells. Table 2 summarizes well depths and screen intervals. The well locations and depths were chosen based on the temporary Geoprobe° groundwater investigation completed by GMA in May of 2016 (GMA, 2016; Table 3). GMA supervised the installation of monitoring wells MW4 through MW13, MW21), MW91), and MW13D between November 29, 2016 and December 2, 2016. Monitoring wells MW14 and MW9DD were installed on April 11 and 12, 2017, to further delineate the horizontal and vertical extent of the plume (GMA, 2017a). On February 5, 2019, GMA authorized the abandonment of monitoring well MW9DD due to suspected failure of the bentonite seal, which was most likely caused by high chloride levels in the groundwater. The well abandonment record is provided in Appendix I. On February 9 — 10, 2019, GMA supervised the installation of monitoring wells MW15 — MW17. GMA installed the wells to monitor water table fluctuations and further delineate the horizontal and vertical extent of the plume, and determine groundwater interactions with the Bennett Millpond. Copies of the well construction permit and well construction records are provided in Appendices II and III, respectively. 6.2 WATER -LEVEL MEASUREMENTS AND GROUNDWATER MIGRATION Figures 6 and 7 illustrate historical equipotential maps utilizing data from the July 2018 monitoring event. On January 9, 2019, GMA measured and recorded water levels in the following 18 monitoring wells: MW1 — MW14, MW2D, MW9D, MW13D and MW9DD. The average depth to groundwater in the shallow Yorktown Aquifer was 16.09 feet BLS (Table 2). The resulting equipotential map for January 9, 2019, indicated that groundwater in the shallow Yorktown Aquifer was flowing to the west and east along an average hydraulic gradient of approximately 0.002 feet/foot (Figure 8). The average depth to groundwater in the Surficial Aquifer was 5.19 feet BLS (Table 2). GMA prepared an equipotential map for the Surficial Aquifer that indicated groundwater was flowing north along an average hydraulic gradient of approximately 0.002 feet/foot (Figure 9). GMA recognizes there were only three monitoring wells screened across the water table and that the mounding effect of the ponds is not shown in Figure 9. Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 7 of I I In the deeper Yorktown Aquifer, the average depth to groundwater was approximately 21.3 feet BLS, and the groundwater was flowing toward the west along an average hydraulic gradient of approximately 0.002 feet/foot (Figure 9). These gradients correspond to the topography of the area surrounding the monitoring wells. On January 9, 2019, GMA calculated an average downward -directed, vertical hydraulic gradient of approximately 0.3 feet/foot at the source area by comparing water -level data for Surficial Aquifer and deep Yorktown Aquifer well pair MW2/MW2D (Figure 5, Table 2). An average downward -directed, vertical hydraulic gradient of approximately 0.03 feet/foot was identified by comparing water -level data for shallow and deep Yorktown Aquifer well pairs MW9/MW9D and MW13/MW13D. An average downward -directed, vertical hydraulic gradient of approximately 0.008 feet/foot was identified by comparing water -level data from the deep Yorktown Aquifer and the underlying Castle Hayne Aquifer well pair MW9D/MW9DD (Figure 5, Table 2). These vertical hydraulic gradients indicate the preference for groundwater and contaminants to move downward more rapidly than horizontally (i.e., the site is in a recharge area). Contaminants detected in monitoring wells tapping both the Surficial Aquifer and the deeper Yorktown Aquifer confirm downward migration of contaminated groundwater. 6.3 SAMPLE COLLECTION AND ANALYTICAL METHODS On January 9 — 10, 2019, GMA collected groundwater samples from 18 monitoring wells (MW1- MW14, MW2D, MW91), MW13D, and MW9DD), nine surface water locations (SW1-SW9), and two water supply wells (WSW1 and WSWS). Appendix IV describes GMA's standard sampling procedures. The samples were transported to a North Carolina certified laboratory, Prism Laboratories, Inc. (Prism), by their courier service in ice -filled coolers under chain -of -custody protocol. Prism analyzed the samples for chloride, TDS, total and dissolved iron, and total and dissolved manganese. GMA also performed field monitoring for dissolved oxygen (DO), conductivity, temperature, pH, oxidation-reduction potential (ORP), specific conductivity, and turbidity during sampling. Copies of the laboratory analytical reports and chain -of -custody forms for the samples are provided in Appendix V. 6.4 GROUNDWATER SAMPLING ANALYTICAL RESULTS Table 4 summarizes the analytical results for groundwater samples collected in January of 2019, along with historical analytical results. Analytical results from the 18 samples collected on January 9 — 10, 2019, indicate chloride, TDS, total iron, and total manganese were detected (Table 4) in the majority of wells at concentrations greater than the applicable 15A NCAC 2L groundwater standard. Source area wells MW1, MW2D, MW12, and downgradient wells MW7, MW9D, and MW9 contained the highest concentrations of chloride (Table 4, Figure 10). Representative concentration maps show the estimated horizontal extents of chloride, total iron, Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 22, 2019 Page 8 of 1 1 and total manganese in January 2019 (Figures 10 through 12, respectively). Data summarized on Figure 10 includes temporary well chloride data collected on May 25 — 26, 2016 to help define the extent of the plume. Historical plume maps (July 2018) for chloride, total iron, and total manganese are shown in Figures 13 through 15, respectively. 6.5 SURFACE WATER SAMPLING ANALYTICAL RESULTS GMA collected nine surface water samples (SW1-SW9) during the January 2019 sampling event (Figure 5, Table 5). Chloride was detected in surface water samples SW3-SW7 at concentrations greater than the applicable 15A NCAC 2B surface water standard (213 Standard). The highest concentration of chloride was detected in sample SW6, with additional 2B Standard violations in downgradient samples SW3, SW4, SW5, and SW9 (Figure 5, Table 5). Compared to the July 2018 data, chloride concentrations in sample SW9 decreased significantly, with slight decreases in samples SW4, SW5, and SW6 (Figures 10 and 13, Table 5). Elevated total iron and total manganese concentrations were detected in some of the surface water samples collected, however the DWR and EPA removed the surface water standards for both constituents due to multiple studies verifying naturally high levels across the State (Table 5). This does not rule out that the adjoining surface water features have been impacted by the effluent from the WTP as the highest iron and manganese levels are located downgradient of the WTP effluent infiltration point (Table 5, Figures 11 and 12, respectively). Analytical results of surface water samples from January of 2019 indicate reductions in chloride, iron, and manganese concentrations in most of the samples collected compared to results from samples collected in July of 2018. These changes are likely due to seasonal fluctuations in precipitation and its effects on surface water runoff and groundwater infiltration. Appendix IV describes the technical methods and standard procedures utilized for sample collection. Appendix V provides a copy of the laboratory report and chain -of -custody forms. 6.6 WATER -SUPPLY WELL SAMPLING RESULTS GMA collected a sample from WSW1 on January 10, 2019 and analytical results indicate stable chloride concentrations through time (Figures 10 and 13, Table 4). GMA also collected a sample from WSW5 on January 10, 2019. Chloride, TDS and total iron and total manganese concentrations increased slightly since the July 2018 sampling event (Table 4). The owner discontinued use of the well, which will hopefully prevent additional movement of the chloride plume in the direction of well WSW5. Appendix IV describes the technical methods and standard procedures utilized for sample collection. Appendix V provides a copy of the laboratory report and chain -of -custody forms. Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 19, 2019 Page 9 of 1 1 6.7 FIELD MEASURED WATER QUALITY PARAMETERS On January 9 —10, 2019, all surface water and groundwater samples were tested in the field for DO, specific conductance, conductivity, temperature, pH, ORP, and turbidity to evaluate the water quality conditions (Tables 4 and 5). The high specific conductivity and conductivity measurements are consistent with elevated chloride concentrations in the monitoring wells, water -supply wells, and surface water samples. 7.0 CONCLUSIONS • On January 9, 2019, the average depth to the water table in the Surficial Aquifer was approximately 5.19 feet BLS, and groundwater was flowing toward the north along a hydraulic gradient of approximately 0.002 feet/foot. • On January 9, 2019, the average depth to the water table in the shallow Yorktown Aquifer was approximately 16.09 feet BLS, and groundwater was flowing to the west and east along an average hydraulic gradient of approximately 0.002 feet/foot. • On January 9, 2019, the average depth to the water table in the deeper Yorktown Aquifer was approximately 21.3 feet BLS, and groundwater was flowing toward the west along an average hydraulic gradient of approximately 0.002 feet/foot. • GMA identified an average downward -directed, vertical hydraulic gradient of approximately 0.3 feet/foot at the source area by comparing water -level data for Surficial Aquifer well and deep Yorktown well pair MW2/MW2D. • GMA identified an average downward -directed, vertical hydraulic gradient of approximately 0.03 feet/foot by comparing water -level data for shallow and deep Yorktown Aquifer well pairs MW9/MW9D and MW13/MW13D. • GMA identified an average downward -directed, vertical hydraulic gradient of approximately 0.008 feet/foot by comparing water -level data for the deep Yorktown Aquifer and the underlying Castle Hayne using well pair MW9D/MW9DD. • The vertical hydraulic gradients indicate a preference for groundwater and contaminants to move downward more rapidly than horizontally. • Contaminants detected in monitoring wells tapping both the Surficial Aquifer and the deeper Yorktown Aquifer confirm downward migration of contaminated groundwater. Data from the deepest monitoring well MW9DD indicate a significant increase in chloride and manganese detections and actual detection of iron below the Yorktown Aquifer, which is likely due to the failure of the bentonite seal in the presence of high chloride concentrations. • Analytical results from the 18 groundwater samples collected on January 9 — 10, 2019, indicate chloride, total iron, and total manganese were detected in the majority of wells at concentrations greater than the applicable 15A NCAC 2L groundwater standard. Source area wells MW1, MW2D, MW12 and downgradient wells MW7, MW9 and MW9D contained the highest concentrations of chloride. • Iron and manganese concentrations occur above the 15A NCAC 2L groundwater standard in most samples collected from the monitoring wells, with the highest concentrations occurring in source area and downgradient wells. Groundwater Monitoring Report Valhalla WTP (NPDES Permit# NC0032719) February 19, 2019 Page 10 of 1 I • Samples collected from water -supply well WSW5 continue to contain chloride concentrations even though it is no longer used for irrigation purposes. • Elevated chloride, iron, and manganese concentrations continue to impact the surface water samples collected from within and downgradient of the source area. 8.0 RECOMMENDATIONS • Survey top of casing elevations of newly installed monitoring wells MW15, MW16, and MW17. • Establish surface water level gauging survey points. • Conduct a comprehensive groundwater level gauging event utilizing all monitoring wells. • Collect groundwater samples from newly installed wells MW15, MW16, and MW17 for analysis of chloride, TDS, and total and dissolved iron and manganese. • Conduct aquifer testing using new wells MW15 and MW16. • Review new well data and make decisions about possible new monitoring well/temporary well data needs and replacement options for a deep monitoring well. • Begin work on the Site Conceptual Model that will support construction of a groundwater flow model. We will calibrate the groundwater flow model using site data, and we will perform solute transport simulations. GMA will provide results of the modeling in a report. • Continue semi-annual sampling to include 20 monitoring wells, 9 surface water samples, and sampling of water -supply wells WSW1 and WSW5. If possible GMA will coordinate the sampling event to coincide with the WTP's quarterly sampling requirements under their NPDES permit in July of 2019. • Continue to collected data on geochemical parameters of dissolved oxygen (DO), oxygen reduction potential (ORP), pH, temperature, specific conductivity, and turbidity for all samples. • GMA will continue to monitoring the iron and manganese levels and utilize low flow sampling methods during sampling events to help reduce turbidity. GMA will establish background levels for iron, manganese, and chloride. • Utilize new data to aid in planning and research of active remediation options. 9.0 REFERRENCES CITED Groundwater Management Associates, Inc., December 18, 2015, Initial Site Assessment, Valhalla Water Treatment Plant, Edenton, Chowan County, 5 pages plus attachments. MONITORING WELL INSTALLATION The construction of all monitoring wells conforms to North Carolina well construction standards (15A NCAC 2C). Well materials used include: flush -threaded PVC casing and slot 0.01-inch PVC screen; medium -grained (#2) quartz sand; bentonite pellets; and a water -tight, lockable well plug. Mud -rotary drilling was used to install all monitoring wells. A licensed well construction contractor supervised the drilling and well construction. The wells were developed by pumping. II. SURVEYING The top of casing elevations for monitoring wells MW1 through MW13, MW2D, MW9D, and MW13D were measured by Josiah A. Webb, II P.L.S. on January 14, 2017 to be accurate within 0.05 feet. The elevations of monitoring wells MW14 and MW9D casing tops were measured and tied into the existing monitoring well network by GMA staff by using an automatic level, and are accurate to within 0.01 feet. Elevations were determined relative to the datum referenced by Mr. Webb. Locations and distances were measured using a measuring tape and/or a surveyor's wheel. III. WATER -LEVEL MEASUREMENTS Depth to water within the monitoring wells was measured with an electronic water level meter that is accurate to within 0.01 feet. Standard practice is open all monitoring wells by removing the well plug and allow adequate time for the water levels to equilibrate. The water levels are then measured and recorded. IV. WELL PURGING AND COLLECTION OF GROUNDWATER SAMPLES The monitoring wells were purged of three well volumes prior to groundwater sampling using a decontaminated purge pump with new disposable polyethylene tubing was used to purge the well. Fresh, disposable vinyl gloves were worn during purging and sampling of the well. In order to determine when a well has been adequately purged and to field test water quality, GMA monitored the geochemical parameters of dissolved oxygen (DO), oxidation-reduction potential (ORP), pH, conductivity, specific conductivity, temperature, and turbidity of the groundwater removed. All field groundwater -quality monitoring was performed using a flow -through cell. Prior to purging each monitoring well the volume of water standing in the water column was determined. With respect to volume, an adequate purge is normally achieved when three well volumes have been removed. Groundwater samples were transferred directly into the laboratory supplied sampling containers (1 Liter plastic and 500 milliliter plastic) from the purge pump tubing. The samples were submitted to Prism for analysis of chloride, total manganese, dissolved manganese, total iron, and dissolved iron. The sample containers were stored in plastic Ziploc® bags and immediately placed on ice for transport to the laboratory under chain -of -custody protocol. V. SURFACE WATER SAMPLING Samples SW1, SW2, SW4, SW5, SW6, and SW7 were collected as grab samples from the body of water. Sample SW3 was collected from the underground french drain pipe that discharges into the pond. Samples SW8 and SW9 were collected utilizing a peristaltic pump and new tubing at a depth of approximately three feet below the surface of Bennett Millpond. The samples were placed in containers provided by Prism Laboratories and were submitted to Prism for analysis of chloride, total manganese, dissolved manganese, total iron, and dissolved iron. The sample containers were stored in plastic Ziploc® bags and immediately placed on ice for transport to the laboratory under chain -of -custody protocol. VI. WATER -SUPPLY WELL SAMPLING The water -supply well was purged for a minimum of five minutes prior to sample collection. Any hose was disconnected prior to sample collection in the laboratory supplied container. New disposable nitrile gloves were worn during the purging and sampling of each well. For wells without an active pump, either a peristaltic pump with new disposable polyethylene tubing, or a decontaminated purge pump with new disposable polyethylene tubing was used to purge the well. New disposable nitrile gloves were worn during the purging and sampling of each well. Groundwater samples were transferred to laboratory supplied containers for analysis. Standard procedure for chloride, total and dissolved manganese, and total and dissolved iron analysis is to collect one groundwater sample for all three analysis from the well. All sample containers were stored in plastic Ziploc® bags and were immediately placed in a cooler with ice. Samples were submitted to a Prism for analysis under chain -of -custody protocol, by courier service APPENDIX V COPY OF LABORATORY REPORT AND CHAIN -OF -CUSTODY FORM FOR GROUNDWATER AND SURFACE WATER SAMPLES Case Narrative R I S M - LA 30RATORIESS INC.
