4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The project is located along Alamance Church Road. Adjacent land use includes commercial on the western half of the project area and low density residential along with institutional
buildings on the eastern half of the project area. Suburban land uses are found in the vicinity of the project area.

4h. Explain the purpose of the proposed project:*
The purpose of the project is to provide improvements to Alamance Church Road as identified in previous public meetings including pedestrian and bike access and safety. Alamance
Church Road is identified on the City of Greensboro's transportation improvement plan and funds for improvements were approved in a 2008 City bond referendum. The 2015 Biped
Plan Update also identified the road as candidate for bike lanes.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The project consists of enhancements and improvements to Alamance Church Road (SR 1005), including roadway widening with curb and gutter, sidewalks, grass utility strip, storm
drainage improvements, and bicycle lanes from the intersection with J.M. Hunt Jr. Expressway, M.L. King J. Drive, and Patton Avenue to the Greensboro city limit.
Typical roadway construction equipment Will be used including backhoes, bulldozers, pans, pavers, rollers, sheepsfoot, and dumptrucks. All appropriate sediment and erosion control
measures will be implemented and monitored throughout construction.

6i. Comments:
At Sites 1, 2 and 3 buffer consists of the streambank which is a narrow strip of mowed grass. Site 1 and 2 impacts are temporary due to construction
of a retaining wall. Site 3 is permanent due to installation of a new stormwater pipe. Site 4 pond is a permanent impact to zone 2 from the installation of
a wet detention pond. The pond and outfall channel are located outside of the zone 2 buffer. The area will be seeded With herbaceous seed. Site 3
culvert impacts are due to the road Widening and associated culvert extension.

1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
Retaining walls have been added along Alamance Church Road to avoid additional impacts to SB. SB flows parallel to the road as a result of historic
realignments. Due to the proximity of the stream to the road, stormwater pipes discharge directly to the stream however impacts were minimized by
eliminating rip rap.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Typical sediment and erosion control measures will be taken on site. Silt fences and temporary sediment basins Will be utilized to prevent sediment
from entering streams.

Comments:
Pre/post analysis shows no significant increase or minor increase in discharge or velocities for most outfalls. A wet pond is proposed for the outfall where there is a greater increase of
the peak Q2 and Q10.

NHP Natural Heritage Data E)plorer, IPaC, and site evaluation. The majority of the area consists of maintained lawn. Limited areas epst that contain
suitable habitat for Schweinitz's sunflower as the species grows along roads and utility corridors. No sunflowers were located during site evaluation.
Habitat for small whorled pogonia was not observed. No occurrences are listed within a mile of the site.
