Scott Ross, PE PO Box 1620 Dublin, VA 24084 Subject : Compliance Inspection Report: Permit No. NCG 020155 Southside Materials—Shelton Quarry Caswell County Dear Mr. Raza: On February 4, 2020, Brandon Wise and Zac Lentz, inspectors with the North Carolina Department of Environmental Quality(NCDEQ)—Division of Energy, Minerals, and Land Resources, inspected the facility located at 1524 Rock Quarry Road in Pelham. The purpose of this visit was to conduct a compliance inspection as part of NCDEQ's goal to inspect all permitted facilities. The inspection consisted of review of the Stormwater Pollution Prevention Plan (SWPPP)and each of its components,review of the facility outfall, as well as the review of the overall site conditions. Permit: This facility has General Stormwater Permit NCG020155 to discharge stormwater from industrial activity associated with mining and quarrying of non-metallic minerals (except fuel),mine excavation, processing, and vehicle maintenance; along with authorized wastewater point source discharges from mining operations under the National Pollutant Discharge Elimination System(NPDES). This version of the permit was issued on October 1,2015 and will expire on September 30, 2020. The facility has been permitted since September 30, 1993. Records/Reports: This facility is required to develop and maintain a SWPPP in accordance with Part III of the permit. All qualitative and analytical monitoring records are required to be maintained with the SWPPP for a minimum of five (5) years. Under this permit,analytical monitoring is required under current operating procedures. The facility records show sampling is being completed and is kept onsite in the office. Annual updates to the SWPPP are required in accordance with Part 111, Section 8 of the permit. While some records were shown of annual updates with no changes to the permit these should be printed out, signed, and kept with the SWPPP. Training records are required in accordance with Part III, Section 6 of the permit. There were no training records kept with the permit. It is required that some form of document showing who received training and when be kept with the SWPPP. Overall the SWPPP was well put together just lacked documentation of some parts of the permit. A Spill Prevention, Control, and Countermeasure(SPCC) was included as part of the SWPPP due to the trigger of having over 1,320 gallons of above ground oil storage. This plan was also well put together and included all required aspects of the plan. Facility Site Review: This facility operates as a large stone quarry with a large pit that is over 50% of the current footprint of the facility. There are multiple ponds on site that are used as a closed loop system that allow for a majority of the stormwater that falls on site to be used in on site processes. These ponds have a pump system so that if they are needed to be pumped down the water is pumped back into the pond located in the pit. A single outfall is located in the northern section of the facility that discharges offsite. The majority of the stone crushing is done to the east of the main pit near the railroad tracks. This area had very good housekeeping and showed signs of being maintained very well. Just west of the office was the maintenance area where any oil containers were located and kept within secondary containment. The largest tank onsite was a 10,000-gallon double walled diesel tank. The maintenance and storage sheds were kept up very well and did not have any signs of staining from leaking oil. Overall site conditions were very good for a mining operation. Effluent Receiving Waters: Effluent from this facility drains into an unnamed tributary of Tardy Branch, class"C"waters of the Roanoke River Basin. Please note that currently this class of waters does not have any extra protections but over time classifications can change. Self-Monitoring Program: This facility is required to provide qualitative and analytical monitoring twice annually. The outfall discharge from the facility is expected to be properly maintained and monitored. At the time of the inspection monitoring was being completed and records showed regular inspections were being completed as well. Monitoring records showed limits were being met during the sampling. There are no additional comments or changes to the currently operating permit. Please continue to keep up the facility and continue to monitor the stormwater outfalls and discharges. Plan needs to be updated annually or at least documented that it was reviewed. Employee training needs to be documented and kept with the plan as well. Facility plan had all required aspects including a SPCC plan due to 10,000 gallon tank of diesel stored on site.
