Dear Mr. Wright:
Staff of the Mooresville Regional Office (MRO) viewed an active land application event on
January 22nd, 2020. The MRO would like to thank Alex Fox and Mike Cox (Synagro) for their
time during the site inspection. Mr. Fox also drove to additional fields to review buffers for
proposed new and existing acreage noted in the current permit renewal/modification.
MRO staff then drove to the plant to view the lagoon storage, but did not see on -site staff as it
was after 4:OOPM and the office was locked.

Lagoon storage at the WWTP and field conditions at the land application site appeared to meet
program requirements at the time of inspection. The following items of interest are noted:
• Please address renewal specific questions to the DWR assigned permit writer,
ranveer.katyal(a)ncdenr.gov.
• Our data base shows outdated information for the land application operators. An
updated "Operator Certification Form" (attached to email) should be completed and
resubmitted perform instructions.

Comment: Current records were reviewed in the renewal application. MRO staff scheduled this inspection
spontaneously with Synagro staff upon notification of a land application event. MRO staff drove
past the WWTP to view lagoon storage late in the day, after all acreage was viewed, but
plant
staff was not contacted or present while on -site. Additional records will be reviewed in the 2019
Annual Report.
