Dear Mr. Todd:
On January 28, 2020, staff of the North Carolina Division of Water Resources (DWR) Winston-
Salem Regional Office performed a compliance inspection of the subject facility. This inspection
was conducted by DWR staff persons Justin Henderson and Patrick Mitchell. Mr. Michael Todd,
Hatchery Manager and Mr. Jim Brown, ORC were present during this inspection. The compliance
inspection report for this inspection is enclosed for your review. A separate DWR office file review
of self -monitoring data for the period of January 2019 through December 2019 was also conducted.
While, the inspection reflects compliance with Permit No. WQ0020793, the following items of
concern warrant your attention and action:
1) Permit Condition III. (5) states: public access to the non -discharge disposal sites and
wastewater treatment facilities shall be prohibited. While signage was present in two
locations adjacent to the infiltration/evaporation lagoon, they were not clearly legible due
to fading, and there were no access restriction measures present to prohibit public access
from the parking areas adjacent to the Airport Rd. facility entrances. Please ensure adequate
access restriction measures (i.e. fencing or signage) prohibiting public access, to the
wastewater treatment and disposal facilities, from all potential access points are
implemented and properly maintained to comply with this permit condition.
2) Groundwater results reported on form GW-59 for monitoring events, conducted during the
review period indicated above, continue to detect exceedances of the 15A NCAC 02L
standard for Ammonia (NH3), in Monitoring Well #2. Please submit a Corrective Action
Plan (CAP), describing the facility's plans to address this matter in accordance with Permit
Condition IL (6) and 15A NCAC 02L .0106.

Inspection Summary:
