Permit: WQ0001863 Owner - Facility: City of Mount Holly
Inspection Date: 06/24/2019 Inspection Type : Compliance Evaluation Reason for Visit: Routine
Inspection Summary:
MRO staff met with Adam Brigman (Synagro) and Doug Shoudt (Mt. Holly) to review active land application at site
NC-CL-12-01. The site conditions and application appeared to meet program requirements at the time of inspection.
This permit is currently in the renewal process with Central Office Non -Discharge permitting staff in Raleigh. Some existing
sites in Lincoln and Gaston county were viewed roadside to assess area changes detected with on-line mapping. It appears
structures in question were either tractor or warehouse storage, so will not impact setback requirements.
You will receive separate communication related to the renewal application. Please address renewal specific questions to
the DWR assigned permit writer, ranveer.katyal@ncdenr.gov. Contact maria.schutte@ncdenr.gov should you have
questions about this inspection report.

Comment: Used current data in the apDlication renewal Dackaae as the auarterly samDlina for the land
application event viewed; however, the Pathogen data is missing 3 of the fecal analyses. It
appears the lab report was two-sided and only the odd pages were copied. In addition, the
Vector Attraction Reduction data is from 1-3-19. Sent 6-24-19 email request for: the complete
lab report for all 7 fecal results, the current quarterly 30day bench analyses & estimated
PAN/truck loads for the NC-CL-12-01 application event today. Data was received by email on
6-24&25-19, from Adam Brigman (Synagro).
