Route 1, Box 168 Horse Shoe, NC 28742 SUBJECT: Permit No. NCO069671 Authorization to Construct Modification Mills River, Inc. Mills River Village, Phase I Wastewater Treatment Facility Henderson County Dear Mr. Lindsey: A letter of request for an Authorization to Construct modification was received November 14, 1988, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 0.015 MGD wastewater treatment facility which consists of a bar screen, a flow splitter box, a '41 GPM wetwell influent pump station with 16 GPM duplex submersible pumps and high w•^.ter alarm, 50 linear feet of 3-inch force main, a 15,000 gallon wastewater treatment plant which consists of a 15,050 gallon aeration tank, a 2,680 gallon clarifier, a 1,400 gallon sludge holding tank, dual 75 CFM blowers, a tablet chlorinator with a 315 gallon chlorine contact tank, a 'Y' Notch weir for measuring flow, and all piping, valves, and appurtenances to serve Mills River Village, Phase I in Henderson County. This Authorization to Construct is issued in accordance with Part III paragraph C of NPDES Permit No. NC0069671 issued November 3, 1987, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NCO069671. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph C of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215. 1 and in a manner approvabJe by the North Carolina Division of Environmental Management. The Asheville Regional Office, telephone number 704/251-6208 shall be notified at least twenty-four (24) hours in advance of operation of the installed facilities so that an in-place inspection can be made. Such notification to the Regional Supervisor shall be made during the normal office hours from 8: 00 a.m. „Jl until 5:00 p.m. on Monday through Friday, excluding Stare Holidays. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733 7015 An Equal Opportunity Affirmative Action Employer In the event that the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits, the permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities: A set of approved plans and specifications for the subject project must be retained by the applicant for the life of the project. The Permittee shall provide for the pump station and force main the following items: a. Pump-on/pump-off elevations located so that 2-8 pumping cycles may be achieved per hour in the pump station. b. An air relief valve located at all high points along the force main. c. A screened vent for the wet well. d. Fillets located in the wet well at the intersection of the flooring and sidewalls. e. Three (3) feet of cover (minimum) over the force main or the use of ferrous material where three (3) feet cannot be maintained. f. Sufficient devices which will protect the pump station from vandals. g. Flood protection if the pump station is located below the 100-year flood elevation. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a profe��.sional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications.
