Driving directions: See file. II. EXISTING FACILITIES: MODIFICATION AND RENEWAL APPLICATIONS 1. Are there appropriately certified Operators in Charge (ORCs) for the facility? ❑ Yes ❑ No ® N/A 2. Are the design, maintenance and operation of the treatment facilities adequate for the type of waste and disposal system? ® Yes or ❑ No Explain anything observed during the site visit that needs to be addressed by the permit, or that may be important for the permit writer to know (equipment condition, function, maintenance, change in facility ownership, etc.): This is a rental property. The mobile home that was present has been removed and the system has not been in use for several years now. As a result, the system has not been operated or maintained in some time. A tree has fallen and knocked the system controls box and alarm off the post, disconnecting the power. Repairs must be made and it is unknown what other portions of the system function properly versus not. Recommend a schedule condition be added to the new permit that requires notification to the WSRO to schedule a startup inspection and written gpproval received from the WSRO before the system is returned to use. 3. Are the site conditions (e.g., soils, topography, depth to water table, etc) maintained appropriately and adequately assimilating the waste? ® Yes or ❑ No 4. ADDITIONAL REGIONAL STAFF REVIEW ITEMS �onacw.00d On December 19, 2019 WSRO staff conducted an inspection of the subject facility. The facility was found to be vacant and the mobile home that was present removed. The system has been out of use and therefore has not been properly maintained for some time. Repairs are needed to the system controls and alarm, and the entire system needs to be tested before it is returned to use. Attached to this staff report is a site map showing the approximate location of the system components and a copy of the easement for the system that was recorded with Randolph County Register of Deeds. Recommend that language concerning the easement be included in the permit conditions related to setbacks to the WWTF and Spray Field. The system was found to be not in use and no home is present on the property. The Permittee indicated that they plan to return the property to rent sometime soon, but no date provided. System was found to need repairs (e.g. system controls and alarm knocked off by fallen tree, disconnecting power. The entire system needs to be tested and inspected by WSRO before returned to use.
