Pactiv Packaging, Inc. (Pactiv) owns and operates a packaging facility at 1447 Enterprise Boulevard in
Kinston, North Carolina (Kinston facility). The Kinston facility is currently operating under North
Carolina's General Permit to Discharge Stormwater under the National Pollutant Discharge Elimination
System (General Permit). The North Carolina Department of Environmental Quality (DEQ) has issued
Certificate of Coverage No. NCG050397 (effective June 1, 2013) for the facility.
Effective November 1, 2016, Pactiv will change its name from "Pactiv Packaging, Inc." to "Pactiv LLC".
With this submittal, Pactiv requests that Certificate of Coverage No. NCG050397 be updated to change
the company name to "Pactiv LLC" and the facility name to the "Kinston facility'. The State Stormwater
Permit Name/Ownership Change form is included as an attachment to this letter.

6. The $40.00 processing fee. If this is an initial transfer from the original permittee the processing

Hey Jen,
I spoke to Scott earlier today and we don't see any problems in issuing this permit.
It's so hot in here that we have about 10 fans placed at strategic locations inside the building. Needles to say these fans
are making such a noise that it feels like we are on an airport.

I just spoke to Scott Hazelton at Dopaco — he said they have just been acquired by another company and they did an
Env. Audit. Their auditors told them they were worried about the uncovered dumpster and diesel fueling area (which I
guess is around back out of sight by their AST). He spoke to Ken and Ken told him to be on the safe side to apply for
NCG05 — they are going to request rescission of NCGNE.

Hi Al, Samir, Scott and Thom,
Hope you all are well. We've received an NOI from Dapaco, Inc. for their paperboard packaging and
polystyrene lids facility in Kinston (Lenoir County) for coverage under NCG05 (paper, rubber and apparel
permit). The site discharges Stormwater into a creek — however I'm not positive what it is at this time as
our GIS maps are down. The NOI is attached.
So - Their application, says they have an NCGNE0036 — it says its an active permit in BIMS but statutory
as of 2/22/10... hmm...
Now they've applied for this NCG05. Does anyone in the WaRO have any idea why they might have applied for this
permit if they already have an NCG05? I've called them to see what is going on, but just wondered if you all might know.
