Facility Located at 1915 Rolling Green Court, Forsyth County
Dear Mr. and Mrs. Thorpe:
Our records indicate that your property is covered under the above referenced wastewater discharge permit
under the previous owner. In our letter dated May 20, 2019 (copy enclosed), we informed you that this permit is not
transferrable to new owners without notification to this Division. This information was also emailed to you following a
phone conversation on July 5, 2019. As of this date, the Certificate has not been transferred.
Please note that NC General Statute 143-215.1(a) requires a valid permit in order to discharge wastewater to
waters of the State. Pursuant to NC GS 143-215.6A, a civil penalty of not more than twenty-five thousand dollars
($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or
requirements of any permit issued pursuant to GS 143-215.1. Your immediate attention in this matter is appreciated.
This Notice of Deficiency has been issued for the issues noted above. Please submit the enclosed
Name/Ownership Change Form within 30 days of receipt of this letter. If you have any questions, please do not hesitate
