(HES) hereby submits this correspondence documenting the installation of two, groundwater monitoring wells at the subject site. These monitoring wells were installed at the request of the North Carolina Department of Environmental Quality — Division of Water Resources (NCDEQ). The location of the subject site is depicted on Figure 1 and Figure 2, included in Attachment A. On May 14, 2019, and prior to well installation, a meeting was held with the NCDEQ, Mr. Kevin Davidson (of Agri -Waste Technology, Inc. — AWT), and Mr. Joe Beaman (of HES). During this meeting, representatives of the NCDEQ identified two areas for the proposed monitoring wells. These proposed locations are depicted on Figure 3. Following this meeting, Mr. Joe Beaman (of HES) visited the subject site on May 16, 2019. During the site visit, it was determined that the proposed location of one well (MW-1) was very heavily wooded; and that the location for the second well (MW-2) was within the Duke Energy right of way, and too steep for safe drilling operations. As such, two alternate monitoring well locations were identified, and a diagram depicting the new proposed well locations was submitted to the NCDEQ in the Groundwater Monitoring Plan submitted by AWT on June 27, 2019. These proposed monitoring well locations are identified on Figure 1 and were included in the subject permit (#W00009826). A copy of the permit is included in Attachment B. MWA and MW-2 were installed at the site on August 15, 2019. The locations of the wells are depicted on Figure 4. At each boring location, solid -stem augers were utilized to advance a borehole through unconsolidated soils at the well locations. Mr. Andrew Francis (a geologist with HES) supervised the well installations, and logged drill cuttings during advancement of the boreholes. Once auger refusal had been attained, the augers were removed and the borehole was allowed to rest for a minimum of 20 minutes. At the end of the 20 minute period, the borehole was checked for the presence of groundwater using an electronic water level indicator. Groundwater was not detected above the bedrock surface in either borehole. After checking the boreholes for water, the drilling method was changed to air rotary drilling. Air rotary drilling was selected due to the presence of competent bedrock. In accordance with guidance from the NCDEQ, since groundwater was not detected in either borehole, the borehole was advanced 10 feet into competent rock. At this point, the wells were checked for the presence of groundwater using an electronic water level indicator. Groundwater was not detected in either well after construction. Boring logs for the respective wells are included in Attachment C. Both monitoring wells were constructed in accordance with 15A NCAC 02C .0108, with the exception of the following: .0108(p) — well development — the wells were not developed due to the absence of groundwater. .0108(q) — wells associated with underground storage tanks (USTs) — the wells were not installed for the purpose of monitoring or testing for the presence of liquids associated with one or more USTs. .0108(r) - vapor wells associated with USTs — the wells were not installed with the intention of monitoring vapors. Well Completion Records (prepared by the well driller) are included in Attachment D. HES notes two minor corrections to the Well Completion Records as follows: • For MW-1, the latitude and longitude for the well is presented as 36.27653N and-80.77724W on the well completion record. The latitude and longitude should be corrected as 36.276719N and .80.777098W . • For MW-2, the latitude and longitude for the well is presented as 36.27653N and-80.77724W on the well completion record. The latitude and longitude should be corrected as 36.276374N and -80.777863W . Because the "groundwater table" was not encountered in either well, in accordance with Schedule #3 of the permit, HES recommends that the wells be properly abandoned. If you concur with this recommendation, HES will prepare a written request to the NCDEQ petitioning for permission to properly abandon the wells.
