Dear Mr. Kemp:
You are receiving this correspondence because you are currently listed as the owner of a
home/property covered under the subject permit for the treatment & irrigation of domestic
wastewater.
The State requires the Division to perform a Compliance Inspection at least, once every 5 years.
This office has been unsuccessful in several recent attempts to contact you to schedule a
Compliance Inspection for the subject wastewater system.
In accordance with Permit Condition V. (3) any duly authorized Division representative may, upon
presentation of credentials, enter and inspect any property, premises or place on or related to the
wastewater treatment and irrigation facilities permitted herein, and Permit Condition VI. (1) failure
to comply with the conditions and limitations contained herein may subject the Permittee to an
enforcement action by the Division in accordance with N.G. G.S. 143-215.6A to 143-215C. Please
call Mr. Justin Henderson at 336-776-9701 at your earliest convenience to schedule this inspection.
