Below is an email we have received from Mr. Castillo of LKQ Salisbury.
He is claiming a discrepancy in contact information for the facility located at 3720 Burlington Road, Greensboro.
I have attached a Permit Contact Summary for Stormwater Permit NCG100040 issued to this location.

Is this facility still in operation? Has it been sold or shut-down? Does this permit need to be rescinded?

For some reason there is a second annual permit invoice being sent to this location, requesting payment.
In fact we are, and should only be responsible for permit number NCG100080.

Permit number NCG100040 belongs to a location in Greensboro.
I am unsure as to how or who to contact at that location (3720 Burlington Rd. Greensboro NC, 27405) but the permit for
that location should in no way be attached to us here at LKQ Salisbury, 1212 Webb road.

I have attached the necessary documentation to make changes to this location.
Again, I am unsure as to who should be contacted at the Greensboro facility, but please address this other bill to their
address, C/O current Manager.
