On November 3, 2009, the Division of Water Quality (DWQ) received your application dated
October 13, 2009 for the above referenced project. The DWQ has determined that your application was
incomplete and/or provided inaccurate information as discussed below. Please provide the following so
that we may continue to review your project.
Additional Information Requested:
1. Application fee missing: Water Quality Certification 3704 states: "any proposed fill or
modification of wetlands and/or waters, including streams, under this General Certification
requires application to, and written approval from the Division of Water Quality (the
"Division")."
2. "In accordance with North Carolina General Statute Section 143-215.3D(e), written approval for
a 401 Water Quality General Certification must include the appropriate fee." Please remit $240
made payable to Division of Water Quality (DWQ).
