Dear Ms. Berger:
Division of Water Resources' Central and Regional staff has reviewed the application package
received September 16, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on January 9,
2019.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy.

A. Application (15A NCAC 02T .0105):
The storage lagoon appears to not meet the required 50 foot setback to property lines (per 15A NCAC 02H
.02190)(5) which were the rules in place at the time of the original permit). Address if the existing easement
allows for a setback waiver. If a waiver is required, provide.
B. Site Map:
Provide an updated site map in accordance with 15A NCAC 02T .0105(d) that shows the following:
1. Legend, north arrow, and scale
2. Stormwater drainage controls
3. Property boundaries within the treatment and disposal area
4. All facility -related structures and fences within the treatment, storage, and disposal areas
5. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and
other surface drainage features within the waste treatment, storage, and disposal site(s)
6. Identification of all storage or treatment earthen structures that are part of the permit
7. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved
permit)
