Please find enclosed five copies of the revised Pre-Construction Notification Application Form for Calypso Storm Drainage Upgrade. I am also enclosing two full size copies of the revised project plans and three reduced size copies of the revised project plans and a check in the amount of $240.00 for the project fee. The project is located in town limits of Calypso, Duplin County. Revisions to the plans include, removing the requirement to dredge 700 linear feet of Dicks Branch and adding the requirement to remove trash and debris from the branch. The requirements for replacing the headwalls at 2nd Street and 115 linear feet of bank stabilization at 1 st Street remain unchanged.
