APPROVAL of 401 Water Quality Certification
Davidson County Integrated Solid Waste
220 Davidson County Landfill Road
Lexington, NC 27292
Dear Sirs:
You have our approval, in accordance with the attached conditions, for the purpose of a temporary road crossing, as
you described in your application dated March 4, 1999. After reviewing your application, we have decided that this fill is
covered by General Water Quality Certification Number 3103. This Certification allows you to use Nationwide Permit
Number 14 when the Corps of Engineers issues it. In addition, you should get any other federal, state or local permits
before you go ahead with your project including (but not limited to) Sediment and Erosion Control, Coastal Stormwater,
Non-Discharge and Water Supply Watershed regulations. Also this approval will expire when the accompanying 404 or
CAMA permit expires unless otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application. If you change your
project, you must notify us and send us a new application. If the property is sold, the new owner must be given a copy of
this Certification and approval letter and is thereby responsible for complying with all conditions. If total wetland fills for
this project (now or in the future) exceed one acre, compensatory mitigation may be required as described in 15A NCAC
2H .0506 (h). For this approval to be valid, you must follow the conditions listed in the attached certification.
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must
act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to
Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh,
N.C. 276 1 1-7447. This certification and its conditions are final and binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean Water Act. If you
have any questions, please telephone John Domey at 919-733-1786.

Dear Mr. Shrone:
Reference is made to my letter of June 17, 1991. This letter confirmed
the May 24, 1991 wetlands survey verification of the proposed Davidson County
Land Fill adjacent to an unnamed tributary of Hamby Creek and Rick Fork Creek,
east of Lexington, Davidson County, North Carolina.
Reference is also made to the conceptual design and permit application
(PDN) for the aforementioned project received from Mr. Mark N. Hargrove of
Camp Dresser & McKee (engineering consultant for project) on July 16, 1992.
This proposed project design showed jurisdictional impacts of approximately
1.6 acres on unnamed/above headwater tributaries of Hamby Creek.
For the purposes of the U.S. Army Corps of Engineers' Regulatory Program,
Title 33, Code of Federal Regulations (CFR), Part 330.6, published in the
Federal Register on November 22, 1991, lists nationwide permits.
Authorization was provided, pursuant to Section 404 of the Clean Water Act,
for discharges of dredged or fill material into headwaters and isolated waters
provided: _
a. the discharge does not cause the loss of more than 10 acres of waters
of the United States;
b. the permittee notifies the District Engineer if the discharge would
cause the loss of waters of the United States greater than one acre in
accordance with the "Notification" general condition. For discharges in
special aquatic sites, including wetlands, the notification must also include
a delineation of affected specific aquatic sites, including wetlands; and
c. the discharge, including all attendant features, both temporary and
permanent, is part of a single and complete project.

Indicate Which of the Following apply:
0 There are wetlands on the above described property which we strongly suggest should be delineated and surveyed.
The surveyed wetland lines must be verified by our staff before the Corps will make a final jurisdictional
determination on your property.
0 Because of the size of your property and our present workload, our identification and delineation of your wetlands
cannot be accomplished in a timely manner. You may wish to obtain a consultant to obtain a more timely
delineation of the wetlands. Once the consultant has flagged a wetland line on the property, Corps staff will
review it, and, if it is accurate, we strongly recommend that you have the line surveyed for final approval be the
Corps. The Corps will not make a final jurisdictional determination on your property without an approved survey.
The wetlands on your lot have been delineated, and the limits o the Corps jurisdiction have been explained to
yom Unless there is a change in the law or our published re uladons, this determination may be relied upon
or a period not to exceed five years from the date of this notification.
0 There are no wetlands present on the above described property which are subject to the permit requirements of
section 404 of the Clean Water Act (33 USC 1344). Unless there is a change in the law or our published
regulations, this determination may be relied upon for a period not to exceed five years from the date of this
notification.
0 The project is located in one of the 20 Coastal Counties. You should contact the nearest State Office of Coastal
Management to determine their requirements.
Placement of dredged or fill material in wetlands on this property without a Department of the
Army permit is in most cases a violation of Section 301 of the Clean Water Act (33 USC 1311). A
permit is not required for work on the property restricted entirely to existing high ground. If you

Nationwide Permit No. 26 (PDN) A U G 4 1992
Action ID. 199201317
Davidson County
C/O Mr. L. Norman Shrone
Court House Annex
Post Office Box 1067
Lexington, North Carolina 27292
Dear Mr.
This proposed project design showed jurisdictional impacts of approximately
i.6 acres on unnamed/above headwater tributaries of Hamby Creek.
For the purposes of the U.S. Army Corps of Engineers' Regulatory Program,
Title 33, Code of Federal Regulations (CFR), Part 330.6, published in the
Federal Register on November 22, 1991, lists nationwide permits.
Authorization was provided, pursuant to Section 404 of the Clean Water Act,
for discharges of dredged or fill material into headwaters and isolated waters
provided:
a. the discharge does not cause the loss of more than 10 acres of waters
of the United States;
b. the permittee notifies the District Engineer if the discharge would
cause the loss of waters of the United States greater than one acre in
accordance with the "Notification" general condition.

The surveyed wetland lines must be verified by our staff before the Corps will make a final jurisdictional
detemlination on your property.
The wetlands on your lot have been delineated, and the limits o the Corps jurisdiction have been explained to
you. Unless there is a change in the law or our published r-0ulations, this determination may be relied upon
or a period not to exceed frve years from the date of this notitcatiQIL
If you
have any questions regarding the Corps of Engineers regulatory program, please contact

Analysis
- Determine Peak Flow Rate to Drop Inlet:
Use Rational Method (QP = CIA)
- Determine Input Parameters:
- HWALLOW = Allowable Headwater Depth (ft)
- N = Number of Pipes Used
- QPIPE = QP /hl (CfS)
- D = Culvert Diameter (in)
- Type of Culvert (i.e., Concrete, CMP, etc.)
- L = Culvert Length (ft)
- s = Culvert Slope (ft/ft)
- n = Manning's Number
- ke = Entrance Loss Coefficient
- d, = Critical Depth (Use Critical Depth Figures) (ft)
- Find actual HW for the culvert for both inlet & outlet control: (The condition with the
greatest HW governs.)
- For Inlet Control:
Enter Inlet Control Nomograph with D & QPIPE and find HW/D for the
proper entrance type.
Compute HW. If HW exceeds HWAI.LOW, try larger culvert.

- For Outlet Control:
Enter Outlet Control Nomograph with L, Ke, & D.
To compute HW, connect the length scale for the type of entrance condition
and culvert diameter scale with a straight line, pivot on the turning line, and
draw a straight line from the design discharge through the turning point to
the head loss scale H. Compute HW from the following equation:
HW=H+ho-LS
d +D
where: ho = 2 or tailwater depth, whichever is greater.
If HW exceeds HWALLOW, try larger culvert.

Stream Channel Crossing CHKD BY:
Obiective:
Using Manning's Equation, determine the average flow of the stream channel as observed and size
the appropriate culvert size(s) to handle the determined flow.
Manning Equation:
Q= 1.486 A R2J3 5112
n
Where: Q= Flow (cfs)
n= Manning roughness coefficient (NC E&S Section 8.04)
A= Cross-sectional area of flow (ftz)
R= Hydraulic radius (ft); A/P; P=wetted perimeter
s= Longitudinal slope of the water surface (ft/ft)
Assumptions:
1. It has been observed for the last two years that the stream channel flows dry for the majority
of the year.
2. A series of beaver dams and other debris cause stormwater to pond upstream of the
road crossing area.
3. Average flow through the channel has been observed to be approximately 6-12 inches in depth
across the width of the channel (20 ft.). To be conservative assume a flow depth of 2 feet.(HAx)
Flow Cross-section:
w= 20 ft
d= 0.5-1 ft (Typical)
d= 1-2 ft (Peak)
