Re: Engineering Certifications 2002 —2006
In accordance with your October 22, 2019 instructions concerning the above reference, I am
providing the following information for each of the projects listed in your letter:
Permit#WQ0021358
Kendrick Farm Phases 1 & 2
Attached is a copy of the Engineer's Certification and transmittal letter dated Feb 13, 2008. We
do not have any record of the State's response. The wastewater collection system for this
project have been owned and maintained since the completion.
Permit#WQ0021543
Fites Creek Subdivision
In our files we did find the NCDER Final Approval letter for the Water System. However, we do
not have the sewer approval letter. This project has been completed and the sewer collection
system accepted and maintained by the City.
Permit#WQ0022622
Southgate Meadows
This project was completed in 2005. Water and sewer facilities have been owned and
maintained since completion. We do not have any files and/or records concerning this project.

Taylors Creek Sanitary Sewer Extension
Attached is a copy of the Engineer's Certification and transmittal letter dated November 16,
2004. We do not have any record of the State's response. This sanitary sewer trunk line
extension was completed and construction certified by an outside engineering firm. This line has
been in service and is owned and maintained by the City.

Creekside Estates Phase 2
In our files we did find the NCDER Final Approval letter for the Water System. However, we do
not have the sewer approval letter. This project has been completed and the sewer collection
system accepted and maintained by the City.

Rhyne's Trace/ Rhyne's Estates
In our files we did find the NCDER Final Approval letter for the Water System. However, we do
not have the sewer approval letter. This project has been completed and the sewer collection
system accepted and maintained by the City.
Also attached is the State's sewer certification acknowledgement for Phase II of Rhyne's Trace.
This project is attached and part of the overall Rhyne's Estates/ Rhyne's Trace project.

In our files we did find the NCDER Final Approval letter for the Water System. However, we do
not have the sewer approval letter. This project has been completed and the sewer collection
system accepted and maintained by the City.

Permit Rescission Request
The Permittee requests that the subject permit be rescinded and certifies that the permitted sewer system has not been and shall
not be constructed nor made operational.We understand that,in the future,should we wish to operate a non-discharge sewer
system,we shall apply for and receive a new Wastewater Collection System Permit.
