Dear Mr. Crews:
On November 22, 2019, staff of the North Carolina Division of Water Resources, Winston-Salem
Regional Office (DWR) performed a compliance inspection of the single-family NPDES
wastewater treatment system located at the subject property. This compliance inspection was
conducted by DWR staff person Justin Henderson. Mr. Damian Crews was present during this
inspection. While the system is currently inactive, as the residence is unoccupied and reportedly
has been for approximately seven years, this inspection reflects non-compliance with the subject
permit. The following deficiencies require your attention and action:
1) Failure to submit the NPDES Certificate of Coverage (CoC) NCG550000 Ownership
Change Form to have permit transferred into the currently property owner's name as
required by Permit Condition Part II. Section E. (4.).
A copy of the Ownership Change Form was provided to you via mail prior to the date of
this inspection. Please complete and submit the form to request the subject permit be
transferred appropriately.
Please be aware that should the subject wastewater system become active in the future, then
effluent monitoring, operation, maintenance, and records shall be conducted in accordance with
Permit Condition Part I. Section C. and Permit Condition Part I. Section D.
