Fourth Street Wendell, NC 27591 Subject: REQUEST FOR ADDITIONAL INFORMATION Wendell Falls Outfall D Dear Mr. Collins: On October 7, 2019, the Division of Water Resources (Division) received your application requesting a 401 Water Quality Certification and Authorization Certificate from the Division for your project. The Division has determined that your application is incomplete and cannot be processed. The application is on -hold until the following information is received: 1. In the Neuse River Basin, intermittent streams, perennial streams, lakes, ponds and estuaries depicted on USGS maps and NRCS soil survey maps are subject to the Riparian Buffer Rules. Impacts to the riparian buffers must be identified on your plans [15A NCAC 02B .0233 (8)(b)(iv)]. If a stream is depicted on either map it is subject to the riparian buffers. [Note: if the permittee believes the maps inaccurately depict surface waters he or she must consult DWR to secure a formal on -site determination. If DWR has provided an onsite determination, please e-mail the confirmed determination letter to Stephanie Goss at stephanie.goss@ncdenr.gov.] 2. On page 5 of the PCN, part 6. Of the buffer Impacts, the table was left blank. Page 7 of the PCN, part 6e needs to be filled out and the total buffer mitigation itemized in part 6f. 4. Please provide an additional soil survey map. As the map currently stands, it is not decipherable. [15A NCAC 02H .0502(b)] 5. Please provide documentation for review that the U.S. Army Corps of Engineers has verified all stream, wetland, and/or open water locations for the project. [15A NCAC 02H .0502(a)(7)] Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the proper consideration of the application. If all of the requested information is not received in writing within 30 calendar days of receipt of this letter, the Division will be unable to approve the application and it will be returned. The return of this project will necessitate reapplication to the Division for approval, including a complete application package and the appropriate fee. Please respond in writing within 30 calendar days of receipt of this letter by sending four copies of all of the above requested information to the 401 & Buffer Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617. Please be aware that you have no authorization under the Water Quality Certification Rules for this activity.
