Chip Mill facility in Elizabeth City, NC The facility exceeded the benchmark concentration for Chemical
Oxygen Demand and will begin to implement a Tier Three response All other pollutants were below
benchmark concentrations Additionally, the facility did not use greater than 55 gallons of new oil per
month Therefore, no Vehicle Maintenance Area Monitoring is required

Enclosed is the Tier Two Response Discharge Monitoring Report for the month of September 2015, for
our Chip Mill facility in Elizabeth City, NC The facility did not meet the benchmark concentration for
Chemical Oxygen Demand at Outfall #1 The facility has implemented Tier Two Responses, but upon
further review the facility did not previously meet the requirements to implement Tier Two Response
and does not currently meet the requirements and will revert to semi-annual sampling Additionally,
the facility did not use greater than 55 gallons of new oil per month Therefore, no Vehicle
